Bacteria is divided into Gram positive and Gram negative based on their ability to trap crystal violet .

Gram positive stains purple/blue. Gram negative stains pink/red.

What underlies the principle of Gram staining?

Gram positive bacteria has many layers of peptidoglycan in its cell wall. Gram negative bacteria has only one layer of peptidoglycan in its cell wall.
Make your smear. Fix your smear with 95% methanol, it dries. Then you flood with crystal violet...both Gram positive and Gram negative absorbs the crystal violet.
Then flood with iodine. Wash off. And 'decolourise' with 95% alcohol. Finally , a red dye called sufranin is used to stain the smear.

Gram positive bacteria has more peptidoglycan than lipid in its cell wall. So it will trap the crystal violet. And will not be decolourised by alcohol. It will retain the original purple/blue colour of crystal violet. Hence, the purple/blue colour.

Gram negative bacteria has more lipid than peptidoglycan in its wall. So alcohol will wash off the outer layer of the bacterial cell wall...thus decolourising the crystal violet. . Gram negative bacteria can now pick up the red colour of sufranin dye. Hence, red/pink.

Management of typhoid fever can be medical and/or surgical.

Medical management include :

1 )proper fluid and electrolyte monitoring and replacement
2) pharmacologic therapy (broad spectrum antibiotics) prior to isolation of offending organism (S.typhi in this case)
3) patient should be advised on proper hygiene, proper hand washing before eating, proper waste and faeces disposal

Surgical management comes in when complications of typhoid fever arise. A good example is intestinal perforation. In cases of multiple small intestinal perforation secondary to typhoid fever, small bowel resection is indicated.

Gold standard for diagnosis of typhoid fever is culture of isolated organism. Culture is 100% specific. Blood culture, stool culture, culture of bone marrow aspirate.

Widal test is not specific or sensitive for typhoid fever. Bless cool cool

Lagusta: typhoid fever is a serious illness caused by species of salmonella, mainly salmonella typhi, others include s. paratyphi A, B and C strains..... all are gram negative. rods!!!

it is contracted via eating or drinking contaminated food or water, no proper food hygiene, etc

symptoms include headache, high fever, general body pains, vomiting, diarrhea or constipation, abdominal cramps, rose spots on the trunk, then later, bloody stools, weakness, intestinal perforation, sepsis and finally.... DEATH!!!

widal test gives the diagnosis, for treatment, see your doctor!!
I just remembered one of my in course questions in medical microbiology..

"widal test is not a specific and conclusive laboratory diagnostic test for typhoid fever. discuss"

And did I add that Medscape has a tab for differential diagnosis for every disorder?

During my case presentations, it really helps me to rule out stuff intelligently.

It's always cute when I'm presenting and a consultant stops me halfway to ask " why did you ask so so and so?''

And I flash that 'medscape' smile way I don derive tight differentials. It's a must-have

Medscape is the stuffiest app ever for medicine. There's nothing in medicine you won't find there properly dealt with...conditions, drugs, references, procedures. Na wetin I dey carry shine

Then there's this app Merck's Manual. It's a paid app though. Its equally good.

There's Schwartz Manual of Surgery. A bit stuffy on the surgery side. Haven't had time for surgery in a while.

Joanna Sedley: Yeah, if your are a nurse, pharmacist and the rest then you are a med school dropout and not intelligent enough etc. This line is getting old and childish, people should just grow up.
@topic, i sincerely hope they know what they are doing, cos healthcare trends are changing. Nurses are gaining ground in most developed countries and you know we like copycatTING in Nigeria " they are doing so so and so over there, i think we should give it a try here". The strike is becoming incessant irrespective of whatever it is they are agitating for.
Let's play the scenario on how probably our politicians reasons, they are there cos of what they will gain from the government and now for the doctors to give them headache " "these guys are becoming a pain in the axse, let's start training other personnel to perform some of their duties" they make the law and bam bam that is it.
Remember when the CHEW's were introduce hahaha, nurses started struggling for supremacy over them, every other personnel relaxed and now they are getting degrees in community health. These professionals diagnose , prescribe, treat people at various communities using standing order, what if a law changes that and they stop using standing order.
Doctors should take note, in these case, you are not the one with the scapel the politicians are. As far as they are concerned we are all dispensable, they can take your job and give it to a willing opponent.
My perspective though.
