1a) the CO did not have guards and outposts around the barracks.
and
1b) the CO did not have his tank crew at the highest level of alertness.
This would be gross negligence. Boko haram has overrun military barracks even in Maiduguri.
What would make a CO in a hot zone not prepare for this.

2) the perimeter guards were either surprised or compromised.
