i dont know if it is first or second list. All I know is dat EKSU was her first choice. Wen the first list came out, her name wasn't on the list or nor jamb site. but i checked again last weekend and saw her name on jamb site.

you have to download the document sent to you. Then, you fill the form as appropriate. d form is a MS word document, so you can save, copy and paste etc. Send the completed form to the email at the end of the document.
