Hello Dr,

My Fiance's last period started on 24th August. She still had blood coming until friday 28th Aug. We had sex early on sunday 30th Aug but I didn't ejaculate into her. However, I suspect some pre-ejaculate fluid may have gotten into her somehow (still doubting that happened). She took a post pill on 01st Sept.

By 6th Sept. she told me of some light cramps she's feeling. By 7th Sept she told me she saw little blood in her urine.

She drank undiluted lime juice in the morning on 7th Sept and told me she started seeing blood like her regular period coming the same day.

8th sept, the blood still coming like she's having her period again. She just finished her period two weeks ago.

She took a home test in the morning of 09th Sept and she said she has a faint second line. Note:She took post pill about two days after sex (tue 01/09).

The cramps have stopped early on 10th Sept but the bleeding is still coming just like her normal period.

Is there a possibiity that she's had a chemical pregnancy?

Is it safe to say the bleeding she's having is as a result of the post pill she took last week?

Dr pls I did a recent scan and result shows a bulky and non gravid uterus in an anterverted position with heterogeneous myometrial echotexture due to multiple fibroid masses measuring 1:74mm x 52mm is pedunculated
2:64mm x 50mm in anterior lower uterine
3-5: 53mm x 38mm,18mm x 30mm,and 31mm x 42mm in posterior region.
Endometrium is distorted in outline, Adnexae nothing remarkable, Cul-de-sac free. Impression : multiple uterine fibroids.
Pls can u explain wat dis means? Since I'm ttc what do u advice? Can I get pregnant and carry to term with it or I should go for a surgery?

PCOS can be a stubborn condition to manage, but do not be frustrated.

You have already achieved alot in losing 13kg. What is your current BMI?.

Did your doctor place you on glucophage too?

Ovulation induction is usually with clomifene and hcg and this recommended to be taken not more than 3 - 4months at a stretch, although some authorities say maximum of 1yr.

Duphaston may help you to menstruate, help limit the adverse effects of unopposed estrogen on your womb as well as help prevent the miscarriage of a potential pregnancy due to low progeaterone, but it really has no effect on ovulation (which is the primary mechanism by which PCOS causes subfertility).

PCOS is really not straightforward to treat, but affected women still conceive with it.

Where are you? If in Nigeria, a Federal tertiary centre may be your cheapest option to access a Gynae.

Continue what you have been doing well. If BMI is not optimal yet, optimise it gradually. Exercise. Avoid taking prescription from any source or anybody whom you do not know their specialty. Consult with your gynae.

Subfertility due to unresponsive PCOS may justify assisted reproduction, but this should be the very last resort.

Your enquiry is not clear.

You said you are spotting and have much menstrual pain since about 3.5months after taking postinor?

Is it that you spot in between your periods or that your periods now come only as spotting?

Mycoten is good. Topical mycoten. I hope you complete the doses - 100mg for 1 week or 200mg for 3 days?

Elimination is possible, but may not be realistic in your pregnancy. It is very likely to recur as pregnancy continues as your present condition predisposes you to candidiasis due to lowered immunity and hormone changes.

Both of scenerios are probable.

Scenerio 1: It could be a pregnancy, as she was not entirely safe on the day of intercourse, depending on her cycle length. And precum has been proven to contain some sperms. Also the weakly positive Home PT.

Sceneerio 2: Postinor side effects.


What to do? As long as blood loss is not heavy, she should watch and wait, and take note of any changes. If blood loss becomes heavy, see your doctor. Do a serum PT by next week. If positive, treat as early pregnancy complication with your doctor. If negative, regard it as postinor effects and watch the symptoms.

The scan showed you have many average-sized fibroids in your womb.

These fibroids have distorted, or affected adversely, the space in your womb where the embryo/fetus is supposed to develop (the endometrial cavity).

Due to their multiplicity, average (significant) sizes, and the distortion of your endometrial cavity, conception may be adversely affected, and chances of miscarriage are high should conception happen.

Ususally, when there is no other explanation for infertility, fibroids treatment is advised. Surgical removal, by open surgery or if possible by laparoscopy, is the mainstay of treating fibroids.

Method of probing lies with the surgeon.

Give this report to a specialist and he shall solicit the nearest economical surgery to remove the mass(es).

Convert the 5.2In to the equivalent size in metre.Square the equivalent size.

Your BMI is the value arrived out by dividing your MASS by the ''square of the equivalent size.''

hello houz, pls save a soul , i have been bleeding for d past 8wks i went to d hospital and i was given antibiotics, m2tone but d bleeding just reduced for awhile and continue again with little clot serious cramps on my lower abdomen and waist. now am on contraceptives and still seeing blood. pls help me ooo.

My galfriend confirmed her 3 weeks pregnancy this week monday. She inserted 3dose of cytotec bt didn't bleed well so she performed a surgical abortion last 2days. Ever since den she has been complainin of stomach pains, feeling irritated and vomitting. Please doctors in the room should I take her to the hospital or she will be fine cause I'm scared.

i think you meant to say 5.2 feet. If so, your BMI is around 28kg/m2. More weight loss will help, to bring BMI closer to 20 - 25.

Like Lucky said, the surgical approach depends entirely on the operating surgeon's descretion.

The sonologist did not specify precisely the locations of the fibroids, ie whether intramural, subserous or submucous.

It is likely they are multiple intramural fibroids. Open surgery might be preferred.

Have you done a PT? If not, do one to rule out pregnancy complications.

What kind of contraceptives are you on? And what other tests have you done?

If your blood loss is heavy, please see a doctor asap. Iron supplementation is advised until a definitive diagnosis/treatment is found.

Hi,

A level of abdominal cramps is normal in the days following a D & C, and some of the drugs she may be taking can induce nausea and vommiting.

Is she bleeding? A little spotting is normal but bleeding indicates a potential problem.

Is there a fever? If yes, she needs urgent medical attention. Otherwise, watch her symptoms for these signs.

If her present symptoms fail to improve in the coming days, let her go for a check up still.

Menstrogen is not an abortion/emergency contraceptive pill.

Let her do a serum PT to rule out pregnancy.

Hi,

There are many causes of recurrent early miscarriage.

Submucous fibroids are common causes, as are luteal phase problems (progesterone defficiency). Others are majorly genetic causes.

Hi,

Ultrasound will start detecting pregnancy about 5 or 6 weeks from the last menstrual period for transvaginal and transabdominal ultrasound respectively

If you mean 2 weeks after she had missed a period, then ultrasound will pick it up. Alternatively, pregnancy test then will also be positive.

Iron supplementation is advised until a definitive diagnosis/treatment is found.dr thanks ,it all started with infection(yeast) treated by antibiotics then delayed period, did scan nothing found, then brownish spotting , later bloody discharge then bleeding till date. i did scan this week and they saw fibroid and submucous component. am so tired and worried.am on combination 3 contraceptives now.
