Call her and tell her to come over that you'd like to discuss something important with her. This would arouse her curiosity to know what it's about while at the same time, it'd prepare her to handle whatever you say. Now, ask her if she has ever removed a teeth or has ever had toothache...she'll know where you're driving at if she's also aware her mouth smells. From her responds, you'd suggest she gets a mouthwash from a dental hospital and suggest going with her for it to be done. It's not expensive if you really love her, 5k may do. Knowing you care and supportive would make her open to your suggestions than getting angry.

Why should it be difficult to tell her,honesty and communication is very important in a relationship. I have learnt to handle criticism from people,not allowing it to cloud my sense of reasoning. Just be plain with her and see how she will handle it. I once told my ex to start using perfume,in fact he asked me to recommend one for him,days later his friends girlfriend told him she liked how he smell and he felt happy about it and thanked me for it.
