Although I don't have the answers to her healing but everything am seeing here smell spiritual attack.. Now it's spreading.. As if the drugs given are not working.. Op have seen this type before. Better buckle your belt and seek for a strong spiritualist that will deliver your mum or a strong pastor or strong Alfa.. Before she gives up, God forbid. That's my advice oooo

You need to act more urgently, if not the further the wound goes, the further they will eventually have to amputate. The problem is that her health might be too poor, this is why they are delaying it. This happened to an uncle of mine and his kids had to move him to a private hospital where they quickly amputated his leg and saved his life. Pray for your mum, this situation doesn't look good, can't you move her to a better hospital? Even the bed she is laying on looks so bad.

Try and build her blood level by giving her meals made with fresh vegetables and ask the doctors if it's safe for her to take blood tonic

They have a diagnosis but Kazeem can't see it as families are not allowed to handle patients/ relatives folder. Let him ask them their working diagnosis and differential diagnosis.
From the referral note, Mama is hypertensive with a blood pressure of about 180/110, it is such a huge risk for surgery coupled with her diabetic state. No sane doctor will tempt it. It has to be reduced and controlled before making an attempt.

I wish mama quick recovery. Amen.

What a country we live in! This is so sad. Her condition keeps deterioration by the day and the doctors are just acting at snail speed. This is so unfair. Someone's life is at stake here.

This is not snail's speed ooo. She may not survive it if she is sent to the theatre with the aforementioned deviant clinical parameters. They have to be stabilised.

Kazeem, you may have to ask for orthopaedic referral to Igbobi, at least to be on a safer side.

Can't they do anything to stabilize her? Give her medications? Maybe insulin with D5 running.

They can, which I sincerely believe is what they are doing. Giving her medications and diet control is part of the stabilization process.
From the Kazeem's writeup, it shows she has peripheral neuropathy on the affected limb.
As you may know, Peripheral neuropathy particularly affects sensation. It is a common complication for nearly half of people who have lived with type 1 or type 2 diabetes for more than 25 years. The most serious consequences of neuropathy occur in the legs and feet and pose a risk for ulcers and, in unusually severe cases, amputation. Diabetes is responsible for more than half of all lower limb amputations performed in the world. Most amputations start with foot ulcers.
Foot ulcers usually develop from infections, such as those resulting from blood vessel injury. Foot infections often develop from injuries, which can dramatically increase the risk for amputation. Even minor infections can develop into severe complications. Numbness from nerve damage, which is common in diabetes, compounds the danger since the patient may not be aware of injuries.

Then, here comes the complications
Diabetic Ketoacidosis (DKA) and Hyperglycemic Hyperosmolar Nonketonic Syndrome (HHNS) has to be ruled out.
Hypertension has to be treated to avoid renal damage.
Her Foot ulcer may require antibiotics, wound care debridement, hydro gels, hyperbaric oxygen chamber and total- contact casting but the sad news is, the last two may not be available in Nigeria.
The major factor here is her age....Anesthesia personnel will be hesistant, wary and cautious in administering an anaesthetic agent to an elderly woman with all this associated comorbidities

Hypertension is the first issue. If she is taken to the theatre she might suffer a stroke or the increased blood pressure might make her bleed out. She's also at risk of a cardiovascular accident(heart attack). So they need to sort that out.

If her glucose levels are not under control she might go into hypoglycemic shock or Diabetic Ketoacidosis/hyperosmolar non-ketotic coma( big words for very high sugar levels that induces coma) and not wake up. That on it's own is a sword of damocles.

Then there's the ever present threat of overwhelming sepsis from the necrotic/gangrenous/dead foot.

And then there's her age to consider. She's and elderly/geriatric patient and they dont do too well under anaesthesia/surgery if all parameters are not optimal.

The only issues i am having with the doctors is that they dont seem to be explaining what's happening to the op. Also i dont see the need for the other X-ray of the foot unless the first one wasnt clear. Unless it was a chest xray?

It may not be a spiritual attack but she definitely needs prayers.

P. S in my opinion that foot should go off now to give her a fighting chance. My colleagues may disagree.

The complications are so enormous and no anesthetist and sane Orthopaedic surgeon will like to touch the surgery with a ten foot pole until most of the major ones are stabilised. No wonder they keep on with the investigations. Keeping the medical teams on their toes to rule out all the odds.

But i think the family had the right to know what is going on. They should educate them on the reasons for the delay. But no they are just doing investigations. Its wrong

They said we should wait till 19th Monday to do xray to know where to start the amputation. What they are giving her now is more palliative so that it won't degenerate further. Below are some of the medicine they are giving her. They give her that mesoprin twice a day.

This one that she had used is countless. They are also giving her insulin, morning and evening.

Sorry, I couldn't on time. The one they sis this morning was 172. This after is 190. I have not asked for evening figure. The nurse said the morning figure is the real one.
