It is not strange. It is normal to hang on below if you have erotics flying punches in your mind. You really don't have to feel awkward about how tumid your pe.nis buzzes on a female rocking your waist line. It's just one part of you to suffer the vibe from that level.

Your not been comfortable with it lies in your not hiding this differences when expressed physically. You will be comfortable, panties on before wearing a pair of trousers should at anytime you have a cross with a female friend, your pe.nis shape will be hideous. That awkward look from her eyes that causes you to feel terrible about yourself will be done away with by the pant. Then, your public show of affection will be okay and controlled. Keep at this for one month and you will see a difference all things being equal.

If you're a medic,you will understand that it is normal to have divergency in ideas and how we share them.
It is a fallacy to have me called an impostor when you should have equally answered a question thrown before you.
I want you to understand that the clinical scene is far more a reality than just the medical text.

I want you to understand as well that most women holding champ with you on calling me names are yet to have any of their tube occluded. I am very sure that if I should pick 3 out of 5 from here assuming to suffer hydrosalpinx, they will neither agree to putting in for salpingectomy(tube removal). But for the sake of diplomacy they will agree with you. When most of them will find themselves in such a mess they will come to understand my side view. They will also understand it is easily said than done. Quote me anywhere, on presentation with such a case to a surgeon except on severity salpingectomy is never the first opinion especially if she has had one of the tubes removed or she is not fit for IVF. Yet she is young and active.

Further still, regarding risk/benefit ratio is a normal thing to taking consent from patient. It is something a doctor makes available to a patient and she is dwelt to make fitting choices after proper elucidation, the cons and pros are well spelt and skirted. If she is okay to have it done, then the consent will be registered and signed in. Even the IVF on which promise and indication for a tube removal is worth has it risk/benefit ratio. We have seen cases where a woman loses pregnancy three times for the number of times she tried IVF. It happens but does not actually make its useless or not worth trying. Or, haven't we seen women coming up with one complaint or the other after a failed or successful IVF-pregnancy? These things happen daily but that does not actually mean it should be overlooked. It is always a matter of "let's us try " and through this we have done lot more in medicine.


With this I rest my case.

Did you even read what I write up there, you just want to show good face?

Clinic reality is far from the expected. You think it is easy to hear a tube of yours is to be removed?
What if that woman you are dealing with has had a tube of hers removed before now and she is threatened to have the other removed?

But you called her aside and gave her a glimpse of hope that instead of removing this tube, let's try if we can repair it and manage it for you to carry pregnancy.
Do you know how much a service you have done for her?

I'll assume that by TCS you mean salpingostomy. I'll answer based on that.

In medicine, there's what we call risk:benefit ratio. No doctor will opt for salpingostomy in this case because:

1. It will put the patient at a high risk of ectopic pregnancy.

2. It is common for scar tissue to regrow after a salpingostomy, reblocking the tube. And in hydrosalpinx, there's high probability that the fluid will build up once more.

So what exactly has been gained if either of the two happens?

You have only wasted money, time and effort of the patient. You have also exposed the patient to more risks which could be fatal in case of ectopic.

And that makes you a very bad doctor.

Because if you ever passed through medical school, you'd understand certain basics of pharmacoeconomics (where you opt for the cheaper yet beneficial management plan for the patient based on socioeconomic status) and also deontology aka medical ethics where you're taught Primum non nocere i.e "First, do no harm."

Stop relying on Google to feed your imposter self. It will confuse you and make you look silly at the end.

Remember, if google could make one a doctor then there would be no need for medical training.

First of all I must tell, you are constructive in your ideas and I understand every idea of pharmaeconomics in this very country of ours. You seem to have avoided the initiative that that on presentation with a case of this such one just zero down to removing the tube(salpingectomy).
Do you want to tell me if all cases of hydrosalpinx is being brought to you as a doctor the only choice on observance is to think of removing the tube?

Please, you really have to answer me on this one as to know your point of reasoning.

This is because not in all cases I supposed as a medical doctor to advise tube removal for hydrosalpinx. Other choice have to be exploited especially if the woman and her husband agrees to. It is always a matter of, "let us try if this can work". If the available is not turning out as expected then we have no choice than to remove the tube, it is so simple!

You are using the 9ja clinic scene in judging wider general medicine. When you must have done all you can, you still have to carry normal management and several HSG within stipulated time interval to see how far it's gone.

In medicine anything can happen, the field is full of choices sometimes but could sometime be only one way.

TCS is tubal corrective surgery which can be either salpingectomy (removing the tube) or a salpingostomy (clearing the adhesions to improve patency) though the latter has poorer results and isn't always successful.

No medically inclined person as you termed yourself will write all what you wrote up there.

Well, I won't argue a word with you. The fact TCS is still done for cases that worth trying. That it is not successful in some fraction does not mean it is not in practice. It is.

1. Did you say hydrosalpinx is not much of a gynaecologic problem to cry wary of?

On the contrary, hydrosalpinx is a very big deal. Has no medical treatment (drug) but surgical treatment.

Asides affecting tubal patency, the fluid is toxic and back tracks into the uterus affecting implantation of the embryo.

2. Did you say "have a surgery done to improve patency."?

For hydrosalpinx, you do surgery to completely remove the affected tube (salpingectomy) not that you do surgery to improve any patency.

3. The gold standard way to confirm the diagnosis of hydrosalpinx is by diagnostic laparascopy. If found to be true, the tube will be removed.

It can be misdiagnosed by depending on Pelvic scan or HSG only. While a few hydrosalpinx can be picked up during ultrasound, some can go undetected. Same for HSG.

Some perifimbrial adhesion diagnosis may even involve hydrosalpinx but go unreported due to oversight and human error.

It takes skilled and experienced hands to pick it up during the above. And in Nigeria where a lot of diagnostic centres use outdated machines, it can be a herculean task.

So telling her to do another pelvic scan to reconfirm diagnosis could mislead this patient including her caregivers if the new lab claims to find nothing when there's indeed something due to any factor above listed.

And that seemingly slight mistake can cause a patient to run in circles for months or years and even possible failed IVFs.

Yes, a misdiagnosis can be that grievous.

Therefore, my advice to you Horlar2019 is to proceed straight to laparascopy. That is the way to get a confirmatory diagnosis in the case of hydrosalpinx.

Another perk of laparascopy is that it will help your doctor view all of your reproductive organs and be able to pick other issues affecting fertility eg adhesion which they can quickly clean up in the same procedure.

Basically, what hydrosalpinx means is that there's fluid in your Fallopian tube. The left tube in your own case. Its a type of tubal blockage which prevents the egg from meeting the sperm on the left side.

If the right tube is patent, they could meet via the right and fertilize but because hydrosalpinx involves a toxic fluid, this fluid prevents implantation because it makes the uterine lining hostile and unreceptive to the embryo.

That's why it has to be taken off even if one wants to go the IVF route.

Treatment

Treatment of hydrosalpinx is surgical and it's called a salpingectomy.

However, one or two mamas here have claimed to clear hydrosalpinx using gynapharm and gufeibao alongside proteolytic enzymes like serrapeptase, astragalus, wobenzyme, xi mu cao etc.

It may work and it may not work. They're just about 2% that got results out of all the hydro ladies that used it. The statistics are low. You can research this yourself.

The definitive treatment remains salpingectomy which MAY be followed by IVF.

There's no harm in trying the supplements first, you just may be lucky.

Really sorry you have to go through all this dear. It breaks my heart considering you recently had a myomectomy.

But remember that you're a warrior and this too shall pass. Your victory over these issues is certain. You're a mother of nations and will definitely carry your own babies as the Lord lives.

I really don't find any contrary to your post. But as one in the field I have every right and knowledge exposition to putting things right as expected.

Firstly, an hydrosalpinx is a fluid blockage or occlusion to any part of the uterine tube. It could be the at isthmus(closer to the uterus), ampulla( for fertilization) or the fimbriated end close to the ovary. As a matter of fact, the fluid could be mucinous or serous. This means the fluid occluding the tube either be coming from the upper reproductive organ or peritoneum(abdomen or abdominal cavity). Most causes to this blockage is usually infection but commonly, PID. A good reason I called for a culture at my last post. Not to forget this fluid transduction is a thing of inflammation. From knowledge on pathology, this occlusion could be acute or chronic. If her occlusion is acute then she has hope, I mean a real hope but if it is the other one then she is really in for a salpingectomy as you said.

Now, she has hope! And I am empathic about that not that I really sympathize with her. The worse is either to carry pregnancy the other ways or adopt a child.

Here is it! I made mention of acute and chronic inflammation, right?
Now, the truth is that the blocked tube can be corrected by
TCS and it an expensive procedure. At least there is hope that latency can improve from there. It is only on the ground you can't afford TCS a surgeon is exposed to removing the tube. It has helped many people.

Why do I call for confirmation?
This is because so many women implicated to have had their tubes occluded were not on several HSG.

Lest I forget, I once had injury at the gym 2016 which I had pain at the same lower back, I complained to my doctor and I was ask to do an X-ray, same result was interpreted that particular year, I was prescribed some muscle relaxant drugs I used them and I was very much okay. But since I joined my present work January 2018 i have been suffering from these severe lower back pain again, the nature of my work I sit for 12 hours, one hour break, since the pain started late February 2018 I have not been comfortable at all. Now I find it difficult sitting for 30min at a stretch, I will have to stand up in between 30 minutes cus if I don't stand up I will be feeling the pain seriously like something inflammation at my lower back. Some weeks ago I even went to see a physiotherapist and I attended about 3 sessions but I wasn't getting any improvement. Due to the discomfort this pain is giving me I had to go back to my hospital I told the doctor that I am tired of using drugs that I need to see an orthopedic doctor so I booked an appointment with him, I met him, showed the x-rays I have done including the recent one and also explained everything I have been going through to him, he eventually placed me on drugs again; he gave me "Pregabalin drugs". I have completed the dose since two weeks ago now yet I am not seeing any improvement at all. Am just tired of everything please kindly help me out. I am just 29 year old

It's suggestive of a fluid in your fallopian, this means you have to confirm it again by running another one and if turns to be as presumed then you really have to seek a medical help. Hydrosalpinx is not too much a gynaecologic problem to cry wary of.
You may have to do special endocervical culture for N. gonorrhea and the Trachomatis species.

Should nothing work be open to have a surgery done to improve patency.

It could be a normal allergic rhinoconjunctivitis. If this is normal it will subside with 7-10days, hopefully. The main interest here is to keep off from triggering factor and get to use PCM to regulate your feverish and headache. I am sure this is normal except there is more to it. Kindly watch for at least 7days and if the cough hasn't come down with eye redness mention me again or better still see a doctor.

I think so, cos after taking antibiotics, one may likely have an outbreak of infections especially yeast infection due to the body's inability to defend itself at the moment.

You're right, except that this largely doesn't mean an immunodeficiency (state).
Humans are normal nidus to various microorganisms. Most of these microorganisms are probiotic and helpful in their parasitism, these organisms also add to the inate immune system of the body too. This case is different to one being in long term use of antibiotics because you have a serious health condition either with the spleen or bone marrow or liver or kidney. These are group of conditions fit for any doctor to prescribe drugs to boost immunogenic cells to help keep the body from deterioration.

The unsavoury demand for weight gain by using Apetamin is wrong though can be use to promote appetite on person's finding it hard to have a feel to eat food. This makes it handful to manage malnutrition from Cachexia to kwashiorkor or marasmus.
It is an anti-histamine. This means it should be prescribe only to patient having problems with histaminic allergy like hives, rhinorrhea/rhinitis, conjunctivitis and others.
By effect, it can cause frequency in dizziness and sleepiness and others. It does not in anyway cause a flesh fattening or butt largeness.
This is why most people uses it for weight gain:
1. It increase your appetite threshold in the hypothalamus.
2. After that it causes you to sleep or weakens your metabolic processes, some person through this can suffer constipation.
3.When you sleep you less consume energy, your stress hormone relaxes and your growth hormone increase making way for growth.
