Insulin injection comes in when the above drugs and dietary measures have failed to achieve adequate glucose control. |
Ninejaywon:Insulin is introduced in type 2 DM when dietary measures and oral hypoglycaemic agents have failed to achieve optimal glucose control. It is the last stage of optimal insulin control in type 2 diabetes. It is also used when they present with complications like HHS or infection/sepsis. Please read and read to get more information on things you're not familiar with. It's stupid and uninformed information like this that has been killing patients' confidence in Nigeria and driving them to the ignorant Herbalists. |

Cof1ex:He's talking about government jobs. There's no civil service job in Nigeria that makes sense. I resigned at the equivalent of level 15 in the medical field in a federal government teaching hospital. That salary could not fence a plot of land, let alone develop it. The guy he's encouraging may not even get to that level. The Nigerian civil service is only good for the females, who usually don't have a lot of responsibilities and high expectations. |

Travellerslodge:Your questions should better be directed to the doctors treating him, as they already have more information about him than whatever you can supply us with here. Are you referring to "coma" as "vegetative state"? What's his age? What was the cause of the bleeding? Trauma from RTA, fall, or assault? If this is the case, then he will need an emergency neurosurgery to evacuate the blood clots and stop the bleeding. Or was the bleeding spontaneous? Is he hypertensive? If he's hypertensive, then, it probably means he has what we call hemorrhagic stroke or subarachnoid hemorrhage. Either could be fatal depending on the extent of damage and bleeding. Truth is that there's nothing anyone on nairaland can do for this case. So, get back to the team treating him, and ask for explanation/clarification concerning the diagnosis, treatment plan and his prognosis. |

Confirm if there's no error in calculating the date given to you, as 17th of August should be the exact date. Though they can argue a week difference does not significantly matter, but protocols should be obeyed. |

Have you considered the amount of money you would make in Nigeria running your own pharmacy shops in multiple places, compared with what you would earn working for someone else abroad? Nigeria is a country where people usually don't go to the hospitals when sick, and prefer patronising the pharmacy shops, and also do not need prescription to purchase drugs from the pharmacies. These often times, places the pharmacist at a better position than a doctor who runs a hospital. The huge and skyrocketing population, though mostly poor, is also an added advantage. I think there are big opportunities for pharmacists in Nigeria, unless start up capital is a problem here. I'm a doctor practising abroad, and I still have plans of coming home to run and supervise my own businesses. We can't be working for others all the days of our lives. |
jrusky:I didn't see your post when you initially made it, but let me correct you and those who ignorantly liked your post. The postgraduate training/pathway is quite different from the masters/PhD pathway for other professions(you can only do masters in para medical and basic clinical courses, but not in core medical specialties). That of medicine is called Residency program, run in two divided components, lasting for a total of 4 to 6yrs, depending on the specialty. For surgery which the op is undergoing, it's for a total of 6yrs. After the first 3yrs of rigorous work and training, he wrote Part 1 exam organized by the West African College Of Surgeons or the National Postgraduate Medicine College of Nigeria, and a pass earned him a membership certificate of the related college. This certificate already qualifies him as a specialist in his specialty, and outweighs the masters you get in other professions. You can see why he didn't need your "ordinary" masters degree to earn that amount. Even MBBS is weighted above masters, as it places a medical graduate at level12 in the civil service. There's nothing hard to believe there, as I also earned the same amount after paying Part 1 of the same exams while undergoing postgraduate training in Nigeria. |

Cc. LegitDeals247 I've not been able to reply you with regard to my previous response on the above thread, as the anti spam bot has banned me on 3 different occasions while trying to respond. Below is my response: Let's start by explaining the results. HBsAg reactive & HBsAb non-reactive: means she has been infected with the virus, and has no immunity yet from it(hasn't cleared it). Note that reactive HBsAb means immunity has been acquired(hers is non-reactive) HBcAb(igG HBcAb) reactive: means she's already in the chronic phase of the infection. ie, her infection is already beyond 6months. IgM HBcAb(which was not done) means acute infection, which usually clears in some people. Hers is now chronic, which is usually lifelong. HBeAg non-reactive and HBeAb reactive: means her virus is no longer actively multiplying, conferring low infectivity. This means the probability of transmitting to others is low. But to conclude on this, she needs HBV DNA levels, which was not done. Diagnosis: Chronic inactive hepatitis B infection/Chronic hepatitis B carrier( until HBV DNA levels proves otherwise) The liver function test results were still within normal ranges. This means the virus hasn't caused any damage yet, lending credence to the low replicating state of the virus. But this test needs to be repeated and monitored regularly, preferably 6monthly. Now, concerning treatment, she can still continue with the Livolin---this doesn't kill the virus, but may help the liver cells functioning optimally. She should adopt a healthy lifestyle, take more fruits and vegetables, and avoid things like alcohol and herbal medicines. The virus has no cure yet, but there are certain antiviral drugs that help to reduce the viral loads and decrease the infectivity rate. In her own case, this is not necessary, as her non- reactive HBeAg status already means the virus is not multiplying and probably has a low count. If she must go for drugs, the approved one is Tenofovir, but may be scarce in Nigeria and expensive. It doesn't cure, but lowers the viral load. During pregnancy, the highest rate of mother-to-child transmission occurs during delivery, and can be reduce if special care is taken. Within the first 24hrs after delivery, the child should be given both Hepatitis B immunoglobulin and Hepatitis B Vaccine. Breastfeeding is not contraindicated, as the virus can't be transmitted via the breastmilk. As for you, to find out if you're really immune, you have to check for HBsAb and it's titre. If it's reactive, it means your vaccine actually worked, and you're protected from the virus, especially if the titre is above 10. If not, you'll need a booster dose or a total repeat.
