In my own thoughts, I believe accepting an offer for lunch would depend on the setting. If there is still sufficient time before you face the panel. accepting is okay. I don't see anything wrong she did by accepting the offer for lunch after she had been delayed for about two hours. Also, for the stain on her shirt, I feel it's no offence if they give her a little time to clean up a bit. If her overall appearance is neat and decent, then an oil stain from a meal she had in the interview premises should be overlookable (since it's obvious she didn't arrive that way i.e she carried out due diligence to respect the organisation by arriving neatly dressed). However, it becomes an offence if the role she's to be interviewed for requires that she never accepts an offer of a meal under such conditions, or gets her shirt stained for any reason (even by mistake); then she has obviously given reason for a low score. There are a number of ways to think about this issue... On the other hand, the truth remains that every interview is a two-way thing: while the interviewer interviews the interviewee, the interviewee should also not fail to make a mental assessment of the organisation to know whether he or she would like to work there. Some organisations could be unnecessarily, strict or harsh, and trust me you don't want to be with them. For me, I don't think the candidate did wrong by politely accepting a meal nicely offered, and that shouldn't make he or she fail except they did not perform very well on the necessary indices.
