Paracetamol: a friend or a villain?

In general, when people think of paracetamol they think of a “friendly” medication that can be used without any possible problems and it is beyond doubt a very useful medication. As a matter of fact, almost everyone carries some ... just in case. Funnily enough, it is a general belief that it is good for everything and harmless. Unfortunately, this is not the case. Whilst generally safe for using at recommended dosages, even small overdoses can be fatal and people can die as a result of taking even a small dose of paracetamol. Even so, compared to other over-the-counter pain relief medication it may be less toxic when used chronically at recommended doses, but it is significantly more toxic in overdose.

Paracetamol, also known as acetaminophen, is a widely used over-the-counter pain reliever (analgesic) and fever reducer (antipyretic). It is a mild analgesic, commonly used for the relief of headaches and other minor aches and pains. But it is also a major ingredient in numerous cold and flu remedies and when combined with stronger analgesics like the opioid ones, it can also be used in the management of more severe pain such as post-surgical pain and in advanced cancer patients for palliative care as well.

Many people use paracetamol for inflammatory pain, although it only has a weak anti-inflammatory activity. People feel it works, as a result of the analgesic effect, but it is not treating the inflammation that is the base of the problem very efficiently. While it has analgesic and antipyretic properties comparable to those of aspirin, the anti-inflammatory activity is limited as it has no direct effect at the site of inflammation, but instead acts in the central nervous system. After oral administration, it is rapidly absorbed by the gastrointestinal tract.

Liver damage
Acute paracetamol overdose can cause potentially fatal liver damage. Things get much worse when combined with alcoholic drinks, and in chronic alcoholics or patients with liver damage.

Paracetamol is metabolised primarily in the liver into toxic and non-toxic products. Some final products are inactive, non-toxic, and eventually excreted by the kidneys. However, there is an intermediate product that is toxic and is considered to be responsible for liver problems, like acute liver failure, a killer clinical condition. It is even more dangerous for some people who have ongoing liver problems and for those born with sensitivity to paracetamol where it can progressively damage the liver function. Small children seem to tolerate the toxicity of high dosages better than older ones and adults. When used in adequate dosage it can be used safely for pregnant and breast-feeding women. World demand for paracetamol was estimated at over 35,000 tonnes per year.

For fever
The efficacy of paracetamol by itself in children with fever has been questioned and showed to be less effective than ibuprofen.

For pain
Paracetamol has analgesic properties comparable to those of aspirin, while its anti-inflammatory effects are weaker. It is better tolerated than aspirin in patients in whom there is excessive stomach acid secretion or prolongation of bleeding time. In some cases, the standard over-the-counter dose of ibuprofen gives greater pain relief than the standard dose of paracetamol.

Adverse effects
In recommended doses and for a limited course of treatment, the adverse side effects of paracetamol are mild to non-existent. Paracetamol toxicity is the foremost cause of acute liver failure in the Western World. Compared to ibuprofen whose side effects may include diarrhoea, vomiting and abdominal pain, it has fewer adverse gastrointestinal effects. It was not until 1887 that a clinical pharmacologist tried paracetamol on patients. Paracetamol was first marketed in the United States in 1950 and went on sale in the United Kingdom in 1956 under the trade name Panadol. Panadol was originally available only by prescription, for the relief of pain and fever, and was advertised as being “gentle to the stomach”, since other analgesic agents of the time contained aspirin, a known stomach irritant. In terms of quantity, according to the Portuguese National Pharmacy Institute, paracetamol is the most sold medicine, but nothing is perfect and the problem starts when taken in excess. We all know that there are no medicines that are totally innocent and, like in everything else, too much can never be good. It is our own responsibility if paracetamol turns … from a friend into a villain!

A baby was born to a woman with a heart transplant in the University Hospital Brno, which was only the second case in the Czech Republic, Petr Nemec, director of the Brno Cardiovascular and Transplantation Surgery Centre, told CTK today.

Both the mother and her new-born daughter are in a good condition, though doctors do not recommend to women with such a transplant that they be pregnant, Nemec said.

The woman, 31, who give birth to her daughter on Monday, had a heart transplanted 19 years ago as she suffered from a congenital defect of the cardiac muscle.

"The woman, who was 12 at the time of the transplantation, is only the second mother with a heart transplant in the Czech Republic," Nemec said.

Seventeen Iraqi refugees including nine children landed at the Vaclav Havel International Airport in Prague yesterday as the second of several groups of Christians from Iraq, of whom the Czech Republic plans to accept 153 this year.
One of the refugees, who presents himself as George told journalists that they have not come to eat and drink for free at the cost of the Czech state, but they want to work, each in their own profession.
The youngest member of the group arriving today is a six-week-old baby girl.
The first group arrived in late January, the rest are to come in the nearest weeks.
The first group or two Christian Iraqi families comprised of ten people altogether, have been living in the Okrouhlik recreation facility near Jihlava, south Moravia, since January 24.

The refugees come from Mosul, Iraq, which they abandoned over its occupation by Islamic State. They went to refugee camps in Irbil, Iraq, and further to Lebanon.

January 1, 2016: The New Bank Bail-In System Goes Into Effect In Europe

If you have a bank account anywhere in Europe, you need to read this article. On January 1st, 2016, a new bail-in system will go into effect for all European banks. This new system is based on the Cyprus bank bail-ins that we witnessed a few years ago. If you will remember, money was grabbed from anyone that had more than 100,000 euros in their bank accounts in order to bail out the banks. Now the exact same principles that were used in Cyprus are going to apply to all of Europe. And with the entire global financial system teetering on the brink of chaos, that is not good news for those that have large amounts of money stashed in shaky European banks.

Below, I have shared part of an announcement about this new bail-in system that comes directly from the official website of the European Parliament. I want you to notice that they explicitly say that “unsecured depositors would be affected last”. What they really mean is that any time a bank in Europe fails, they are going to come after private bank accounts once the shareholders and bond holders have been wiped out. So if you have more than 100,000 euros in a European bank right now, you are potentially on the hook when that bank goes under…

The directive establishes a bail-in system which will ensure that taxpayers will be last in the line to the pay the bills of a struggling bank. In a bail-in, creditors, according to a pre-defined hierarchy, forfeit some or all of their holdings to keep the bank alive. The bail-in system will apply from 1 January 2016.

The bail-in tool set out in the directive would require shareholders and bond holders to take the first big hits. Unsecured depositors (over €100,000) would be affected last, in many cases even after the bank-financed resolution fund and the national deposit guarantee fund in the country where it is located have stepped in to help stabilise the bank. Smaller depositors would in any case be explicitly excluded from any bail-in.
And as we have seen in the past, these rules can change overnight in the midst of a major crisis.

2012 - Veteran male porn star Rocco Siffredi told XBIZ Friday that there are “89 to 100” adult performers currently infected with syphilis in three different Eastern European cities where porn is regularly shot — Budapest, Hungary; Prague, Czech Republic; and St. Petersburg, Russia. Marcus returned to work on July 24, and due to his history and reputation in the industry he was not asked for the full test and just showed the producer a copy on his cellphone, enabling him to hide the rapid plasma reagin (RPR) section relevant to syphilis. For a different production, Marcus faked paper documentation by folding the page of the original to hide the RPR results and making a photocopy.

On June 4, 2013, Marcus was sentenced to 30 days in jail and 36 month probation for "knowingly exposing at least two co-stars to syphilis after testing positive for the disease days earlier." Also he is currently being sued by fellow performer Lylith LaVey because of the outbreak.

2010 - Derrick Burts, male porn star who tested HIV-positive said that he thought he would be protected from infection because he used condoms on gay porn sets and was tested before working in straight productions. Burts, 24, had worked as a “crossover” star in gay and straight films for several months before learning in October that he was HIV-positive.

2011 - Unidentified performer tested positive for HIV. This caused the Los Angeles-based AIDS Healthcare Foundation to being campaigning with ex porn stars to get a measure placed on the city's June 2012 ballot that would require adult film performers to use condoms for porn filmmakers to obtain L.A. city filming permits.In the 2012 election voters in Los Angeles were presented with Measure B ("Safer Sex In the Adult Film Industry Act" which require porn performers to use condoms while engaged in sex acts, to provide proof of blood borne pathogen training course, to post permit and notices to performers, and making violations of the ordinance subject to civil fines and criminal charges.The measure passed with 57% voting for and 43% voting against. Vivid Entertainment and California Productions as well as performers Kayden Kross and Logan Pierce sued the county in January to prevent the new law, the Los Angeles County Safer Sex in the Adult Industry Act, from being enacted. 1980's - In the 1980s, an outbreak of HIV led to 27 deaths among porn stars between 1985 and 1989 to include porn actors and actresses, John Holmes, Wade Nichols, Marc Stevens and Al Parker.

2000 - Female porn star Niki Lae was infected with HIV. She only did ten original porn films.

2001 - Female porn actress Tori Coca Flame was infected with HIV. She did a film with Eric Stone who died from AIDS in 1996. If Eric Stone did infect Tori Coca Flame, it apparently took 5 years for the Aids virus that must have been dormant in her system to become HIV active and show up in the blood test results, or she may not have bothered to get a blood test again until 2001. Eric Stone did both gay porn and straight porn. More straight male performers do gay porn under different names and are called crossovers.

I used to be in the "adult entertainment" industry. I did over 50 porn scenes, danced at strip clubs, and dabbled in "escorting" -- AKA prostitution. As a sex worker, I heard from countless young girls asking me how to get in the business and how to be "successful" in porn. Now that I am finally out of this business and seeing it for what it is, I ask why? Why do so many girls these days aspire to have sex on camera, or dance naked in the laps of strange men? What happened?

One only has to look back to the 90's and 80's to see how things have changed (and not for the better). Back then, women (and most men, for that matter), got into porn out of desperation. The majority of sex workers fell into the business and did it secretly, hoping that they could just make some quick money and get out. Now, girls actually look at adult entertainment as a viable career option.

This is dangerous and incredibly misguided. Young women should be aspiring to something real, something that will take them somewhere in life. Porn is often nothing but a dead end.

Here is the pattern I have seen over and over again in my 7 years in this industry: Girl gets into porn, shoots regularly for about 6 months to a year doing relatively tame sex scenes. Work starts to slow down, so girl decides to do more hardcore scenes (things like anal, multiple men etc.). Work slows down again. Girl now starts escorting and becomes "open" to doing just about anything on camera to get work. Eventually, there is no company willing to shoot her and porn work is dried up. Girl usually has no work history and often no schooling, and now is essentially stuck with escorting, stripping, webcamming and any porn work she might be able to scrape up.

I wish that I had aspired to more, dreamed of something bigger. Now I have to deal with my past for the rest of my life. We need to get THAT message out, that porn leads nowhere, that sex is something you shouldn't just give away to anyone and everyone.

I only hope that my stories and experiences can help someone think carefully before making a decision that can alter their life and ruin many future opportunities. Please understand that porn is not a legitimate career path. If you're lucky, you'll get a couple of years before you're "shot out" and used up. And chances are, you won't have much to show for it.

Boko Haram: Britain deploys extra troops to Nigeria

More than 35 British troops are preparing for deployment to Nigeria where they will take part in training exercises to help the Nigerian army tackle Islamist group Boko Haram, which operates in the country’s northern territories.
The deployment comes a month after Defence Secretary Michael Fallon announced a bolstered British effort to combat the extremist group. Boko Haram, usually translated as “Western education is forbidden,” is responsible for a number of terrorist attacks in Nigeria and Cameroon.

On Wednesday the group claimed responsibility for a suicide bombing in Cameroon which killed 10.

In March 2015 the group pledged allegiance to Islamic State (IS, formerly ISIS/ISIL) and began using its black and white flag as its symbol. It is also thought to have had links with Al-Qaeda.
In a statement on the deployment, Fallon said Britain is “united” with Nigeria in the common goal of defeating terrorism.

“We stand united with Nigeria in its efforts to defeat the murderous Boko Haram extremists.

“Stepping up our training efforts will help support the Armed Forces of Nigeria (AFN) for crucial counter-insurgency operations,” he added.

During 2015, around 130 UK military personnel were sent to Nigeria. The latest pledge could see that number double to as many as 300.

Last year the troops were involved in a range of operations, including training in “infantry skills, civil-military affairs, media operations, command and leadership, IED-awareness, and support to Nigerian military training schools and establishments,” the Ministry of Defence (MoD) said.

In December, the MoD also said 1,000 Nigerian troops had received training for counter-insurgency operations in the north east of the country.

“The training uplift announced by Mr Fallon supports work already carried out by the UK’s resident British Military Advisory and Training Team (BMATT). BMATT has also grown in size since the government announced last year that the UK would increase its training and capacity building in Nigeria,” it said in a statement.

It is estimated that the conflict caused by Boko Haram in Nigeria has displaced 2.3 million people, of whom 250,000 are estimated to have left the country. In 2014, the extremist group killed more than 6,600 people.

According to local press reports, the Nigerian army killed three high-profile Boko Haram members in the north eastern Nigerian state of Borno during December as part of its counter-offensive.

Unemployment among PhDs on the rise
A growing number of people with PhDs -- more than 2,000 -- are joining the ranks of the unemployed in Finland. Those with doctoral degrees in disciplines such as biology, chemistry and environmental science are most affected, while a forthcoming study suggests that not even the private sector is looking to hire those with PhDs.There are few bright lights on the horizon for the highly educated.

Unemployment among PhD holders has grown over the past 15 years and increased rapidly in the past few years.

Biologists, chemists and biochemists are having the hardest time finding work, according to economist Heikki Taulu from Akava, the trade union for those with university, professional or other high-level education. Taulu says that these were long considered the fields of the future and though they still may be, that future hasn't arrived yet.

There are currently more than 2,000 unemployed PhDs in Finland, which represents about 5 percent of all those with doctorates.

Maija Arvonen, Agreements and Bargaining Officer with the Finnish Union of Experts in Science, says that in addition to the economic downturn there has been a kind of chain reaction. If a student with a lower degree doesn't get a job they often pursue PhD studies because that guarantees them 4 years of work through to their dissertation, she says.

Many groups have repeatedly proposed to the Ministry of Education and Culture that the number of PhDs could be lower.

"If you spend 10 years working towards your doctorate and you cannot get work, it's not sustainable for the economy or an individual," says Arvonen.

Last year an unprecedented number of PhDs graduated in Finland -- more than 1,800.

In the next four years there may be limits placed on the number of PhDs. The Ministry of Education and Culture will start negotiations with universities to lower the number of PhDs to 1,600 annually. According to the ministry's Erja Heikkinen, that number could be even lower, but it's neither fair nor realistic to drop it too quickly.

In addition to significant cutbacks to university education budgets made by the current government, a soon-to-be released report by the ministry says that the private sector is also not interested in hiring those with Phds.
