I have been on the same issue for a month... I filled and submitted one proprietor but after the site was upgraded, they sent back my form with three proprietors ( the same entry in three places!).
I received a query that I need to fill only one proprietor, but because I could not delete the other two, I did "back to sender". But they sent it back that I should resolve the last query!
