My 5-year old cousin benefitted from my diagnosis of laryngotracheitis one week ago.The diagnosis was missed by many before me.

I nearly lost my 14 year old brother to status asthmaticus roughly a month ago. I was able to salvage that terrible situation in the midst of a panicking family.

I met a girl for the first time some months back. We were supposed to have fun, but then she started her medical history tales. They were unusual for a girl of her age. She's been managed unsuccessfully for months now. Then something about her history was striking- I reached and felt her pulse, I wanted to check the radio-radial synchrony. Alas!! There was a significant radio-radial delay. That was the first time ever I picked a radio-radial delay. I made my provisional diagnosis.

Each time I think of the many people medical knowledge has helped through me, leaving is very guilt-driven. And that's the plight of many who have stayed back to rescue what's left of the situation. They're the real heroes.

Big rhetorical question: If we all leave, who will care for the ones behind? Just think about it.

University Students survive with evening and night class. On the grand scale of unending lecture hours, a busy social life and extra-curricular commitments, one can always fall back on this time-proven strategy to buck his/her CGPA up to maximum reality. To ensure that you don't attend evening/night classes in vain, be careful to apply the following:

1) ARRIVE ON TIME

Arriving for a night class by 11pm or for an evening class by 9pm (to retire to your hostel by 10pm) isnt a smart way to study in those circumstances. By the time you must have spent 20 minutes settling in, choosing a good spot and aligning your mind to the task ahead, you must have used a good chunk of time for preliminaries. Arrive at a very good time - the earliest time possible. You will achieve much and still have sufficient time for good rest.

2) CHOOSE A WELL-LIGHTED AREA

The amount of light in a study area (especially at evenings and nights) is directly proportional to ease of study, interest in the study material, and overall concentration for the time invested in study. Studying in a well-lit area keeps your mind alert and increases your brain absorptive capacity per time.

3) HAVE A SHORT-TERM GOAL

If you must sacrifice useful sleep time to make up for unattended academic work, you need a goal for that period of time. Short-term goals keeps you focused on the target, aligns your priority for that brief moment on the subject matter and comprehensively affords you the luxury of celebrating achievement for that brief moment. A popular maxim goes, "he who fails to plan, plans to fail"

4) DELETE DISTRACTIONS

Between study time, social networks, meaningless chats from passersby, unending phone calls and staring at pretty girls in class, time isn't a luxury you afford right now. The main reason you embarked on a night class is because you were short-changed on time. Wisdom demands that you use that time wisely to accomplish the goals you set out for. So unless you have an emergency to attend to soon, switch off your phones, stay off social media, keep greetings short and make ample use of short time.

5) STUDY SMART

Adapt study techniques that are beneficial to the time involved and study goals to be achieved. This is not time to impress yourself with pages of heavy study material. Your study target should be to amass points and knowledge of what you need to know- a course outline highlighting what you need to know at the end of a subject should come in handy here. Also, attending classes unfailingly enables you to study smart. Unlike those who didn't, you don't need to go over the whole material. You focus on what the lecturer taught in class. Remember- studying smart is key.

6) REVERSE YOUR ASSIGNMENT SCHEDULE

Doing assignments during a study time in night class isn't the best ways to invest your time. Try to invest available time during the day to do your assignments, term papers and seminars. Co-habitating study and assignments within microscopic time in on the all-time overbearing for efficiency and productivity.

7) EAT A LIGHT DINNER

Loading yourself with Fufu and Afang just before you go for your night class would not make your study more efficient. It is counter-productive by all standards. It will make you dull, heavy and resistant to mental flexibility.

8.) BRING SNACKS

You need to be high in energy to pull a night class successfully. Your brain consumes the most energy in your body and you shouldn't be surprised if you begin to get hungry soon. Snacks come in handy to build up your energy to optimum levels ensuring it lasts you the entire study period.

9) DON'T ENGAGE IN HEAVY EXERCISE JUST BEFORE THEN

If you must burn some fat, or add some muscle or keep fit with exercise, few hours to your night class is not the best time to pull this through. Engaging in heavy exercise just before night class will build up lactic acid in your circulation and cause muscle fatigue.

10) ENSURE YOU ARE WELL COVERED

Cold, and mosquitoes are sufficient indications for proper self covering during a night class. You don't want to be distracted by mosquitoes bites at random body parts or the cold trying to permeate your skin. This serves the purpose of keeping your focus on the study material and enabling you to achieve the goal you initially set out for.

Most respectful cephalo-caudal greetings to my chiefs in the house. Straight to the point, the issue of overseas training for the Nigerian doctor has been heralded by a myriad of questions- most unanswered and unattended to. This has necessitated a compendium of what you must know about overseas training as a doctor. I stumbled on an ebook to that effect and it is in the most bit very comprehensive.

So in my usual spirit of doing-the-little-I-can-for-my-fellow-Nigerian-out-there, I'll like to share with medical doctors and students who want to explore career possibilities outside the shores of Nigeria and hopefully come back one day to make the system better than the quagmire it is right now.

The countries covered are United States, United Kingdom, Canada, Ireland, Australia, and New Zealand.

Amhappy: Any kind doctor available. I do vomit a lot after eating esp swallow and soup or vegetables sauce and anything that crayfish is added to. Even meat and fish na trial. This started like 6yrs ago and i have been to 3 hospitals without solution. Even when i add enough pepper,it doesnt help. I only enjoy fruits,bread and yam/oil without problems. Pls what is the cause of this. I vomitted eveything i ate yesterday just because i took a mold of wheat and ate snail.
Do you have allergies?

-Anyone else in your family has the same problem? Immediate or extended family, I mean.
-Do you have a history of asthma, either in childhood or currently?
-Do you have other things you're allergic to ? Like pollen, dust, smell of frying oil, smoke etc?

Most likely a sea protein allergy. You're the second case of a most probable seafood allergy I've seen on NL. Bad news is that sea protein allergy overlap with most animal protein. That's why you react to meat as well.

Management plan? As with other forms of allergy, avoid seafood(e.g crayfish) Also avoid food, you're susceptible to. It may just be nausea and vomiting now- you wouldn't want to trigger an anaphylaxis sometime in the future. I you must eat these protein, fry them properly. The excess heat would denature the proteins and you'd most likely react. But it's best to avoid.

See your doctor for evaluation. Especially a gastroenterologist.

How're yo sure its an ear infection?
Augmentin has a good coverage for those infections. Make sure you take for 7 days even after symptoms have abated.

NSAIDS like Ibuprofen don't help?

It could be anything. A peripheral vascular disease in your right arm is a bit on the extreme- but it could be a possibility. If that's your fear, see a doctor for evaluation.

The course of Hepatitis B infection as to whether you would have a chronic infection or not depend on a number of factors. However, THE AGE AT INFECTION IS THE SINGLE MOST IMPORTANT DETERMINANT OF WHETHER YOU WILL PROGRESS TO CHRONIC INFECTION OR NOT.

95% of neonates(0 to 1 month) infected would progress to chronic infection. 30% of children under 5 will progress to chronicity while less than 5 percent of adults will have a chronic infection. This means that more than 95% of adults infected with HBV will clear the infection (amidst other variables).

I hope this explanation is satisfactory. There are markers to determine whether you're likely to progress to chronicity or you're clearing the infection.

All in all, stick with the recommended treatment guidelines as directed by your physician.

Invest in a good moisturizing cream. You may consider changing your soap to a mild one too. Be careful how you take Vitamin A. It very less likely a vitamin a deficiency is responsible for your dry flaky skin.

Neck swelling in an infant does not necessarily mean he/she has goiter. There are many causes of neck swelling for that age. However, there is a possibility that a 4 month old could have a goiter- howbeit very rare. So you need to know if the neck swelling is a goiter or not before taking further action. Wich is why you need to see a doctor.

The body's homeostatic mechanisms for dealing with those electrolytes must have handled that by now. However, the same cannot be said for those who were already predisposed to a salt-debilitating condition. E.g Hypertension.
