1) High fever is most typically indicative of a viral infection.
2) Lethargy, Malaise and Fever are prodromes of viral infection.
3) Coffee-brown vomit indicates a haemorrhagic aetiology.
4) This person is a traveller to a Tropical region and probably lacks immunity to it. This viral infection should be common here.
5) What's the commonest tropical haemorrhagic viral disease? Yellow fever.

Sir pc: ok, but he does not have difficulty in breathing. As it is a core asthma symptom
His airways are not narrow enough to cause a noticeable difficulty in breathing. But narrow enough to cause a wheeze. The same wheeze he hears after a serious laughter episode.

a middle-aged white lady came into the country. two weeks later, She developed fever and malaise, later progressed to abdominal pains, headache, high fever and vomiting with 'coffee-grrounds' colour. on examination, She was pale, lethargic, pulse: 121 bpm, RR: 45 cpm.....

what is your diagnosis??

what should the patient have done before travellinghuh
1- Yellow fever.

2- She should have been vaccinated beforre travelling.

He should see a doctor.

When he does, he should mention the laughter-provoking cough+chest tightness/pain+heart racing feeling+wheezing. The doctor may either do some tests or place him on a bronchodilator and if he responds well- he makes his diagnosis.

Also, managing the condition involves avoiding triggers. Off the record, I used to have childhood asthma. Back then- crying almost always triggered an episode- just as you described. So I was almost never spanked grin

There are many causes of scrotal pain. A varicocele is one of the many. You need to see a doctor to do a scrotal/groin examination and decipher the cause.

A varicocele would almost always occur on your left groin. You'd feel it as a dull dragging ache. A varicocele will be obvious when you cough or increase intra-abdominal pressure(what we call Valsalva maneuver). Your sac would feel like a "bag of worms".

Madam speaker: i experience loss of appetitie (or mayb fear of eating so dat i wnt go to d toilet),no fever e.t.c....i stool as many times as i eat..after a meal,some minutes after...my stomach will start singing,den i know its tym.

Sir pc: Happy new year @ all, my good friend is in unhealthy condition. According to him, when he laughs immersely, it will make him start coughing and as he coughs, his chest (heart) will be heating up so hard / painful but he does not cough out any tangible stuff. Doctors in the house / experienced minds, pls, what could be the cause of this problem and possibly it's solution/ remedy. Thanks in advance.
How long has he noticed this?

Does he have episodes of cough with chest pain independent of laughing?

When he coughs, what does he bring out? What's the colour? How plenty? Does it have any awful smell?

Does he have fever? Does he feel breathless?

Apart from laughing, what else makes his cough and/or chest pain severe?

If he takes in a deep breath, would he feel the chest pain?


Does he smoke? If yes, how often and how long? (Very important)

OTC= over the counter (just like panadol for headache).

How many times do you pass the watery stool in a day?

A 39-year old man has been taking 16 bottles of beer weekly for the past 9 months. Last week, he begins to feel a sharp pain in his epigastric region that radiates to the middle of his back and is relieved when he sits up and leans forward.

This pain has increasingly become severe on coughing, vigorous movement and deep breathing. He also has occasional episodes of nausea and vomitting. His serum amylase and lipase levels are abnormally high.

Q1- what is your most likely diagnosis ?

Q2- why would this patient come down with life-threatening respiratory insufficiency as a complication?

Cottage cheese, itchy, non-smelly vag-inal discharge in a 24 week pregnant woman is clinically diagnostic of vag-inal candidiasis.

You need to see your doctor. While preparing to do so, on no account should you take oral anti-fungal drugs. It could be potentially harmful for your condition. You could give shots at the intra-vag.inal or topical regimen( I'm sure this is what your doctor will do).

But you still need to see your doctor.

How recently?.

Make sure you stay hydrated. Drink plenty water.
There's this drug "Lomotil". You can get it OTC and save your life. It saved mine few days ago (I was in your shoes grin )

Cough is a 'protective' symptom that means something is wrong with the respiratory tract. So far as the cause of the cough is not eradicated, no cough medicine will remove the cough.

The best approach to managing cough is not to take cough medicine. It is to find out: what's causing the cough?. And then tackle the cough from the roots .

Which is why you need to see a doctor.

Bbeyond: thanks so much doc. Pls doc, i'm jst 25 n i'm having shaky/jittering hands n legs mostly wen i'm tired or hungry. Every little thing i do dat requires effort such as (exercise, long run, dance, even singing) makes me very weak n dizzy dat I even lose balance. Secondly, I hardly keep tempo or sing to any beat without getting confused and weak n i would go off d tempo, even to dance or snap fingers to a beat is a problem. Pls is there any how I can get rid of these embarrassing problem? Thanks.

If the PT was negative, then a pregnancy should be the least of your worries. How did you take the postinor 2?

Emzybrown: Dr lomaxx auramine-rhodamine stain is an exogenous fluorescent dye. It stains mycobacterium tuberculosis, an allows for detection using fluorescent microscopy. It is a very sensitive and specific assay. I dont think nigerian laboratories can afford such a costly fluorescent microscope now. Happy new year.
