You really don't need this much columns.
How I did mine then: 1st column s/n, 2nd column Name of staff, 3rd column last 3 salaries of each staff, 4th column staff signature the last column is the accountant signature. That's all.

Which bank will pay money to another bank acct that's not linked to a bvn?

This is the essence of bvn itself; to check fraud.

You can't defraud with bvn, neither can you get any details needed enough to gain access to an account via bvn; BVN is to check fraud itself

Let me tell you what they will do with your BVN. With your BVN, a scammer can open another account and link with it. The person keeps checking nirsal website for approval. Once the loan is approved they can easily divert your funds to the new account created. Please be wise ooo. I don't know why some people are too gullible to send their BVN to a total stranger. The funds for the loan have recently being increased to 300 billion naira so it's likely that if you apply correctly you will get the loan. So we all have to calm down.

This is not a question of being corrupt or not it's about understanding the country you leave in. Ok tell me, how many msmes in Nigeria can pay between 30 to 50k to their staff? Very few right. What happens to msmes that pay below this range and find it difficult to pay salary due to Covid-19 pandemic? You don't expect them to sit and fold their arms, they would look for ways to creep into the scheme because the government was not fair to all in the first place.

Please I need opinion and clarification here. One of my employees was called by the Survival fund agent he was asked of his salary and he gave a wrong answer( he said #10,000 instead of #45,000 we filled on the site).
