A REMINDER!!!

Men & Women battling infertility go through a lot already in their mind, don’t make it more difficult for them.
✔️They do not want your pity party/sad look.

✔️Don’t remind them they’re getting old-they know that already

✔️Stop focusing all your discussions around your kids, be mindful of their struggles. They are trying to stay happy for you & your family.

✔️Stop pushing financial responsibilities at them thinking/assuming they’ve got nothing to do with their money. Many of them spend way more battling infertility than you ever spend on your kids.

✔️Pray for them IN YOUR CLOSET not on their timeline publicly during their anniversary. Don’t be a Joy-killer.

Lord, I ask that you bless every womb desirous of children, Let the sound of Joy rise among many this month Oh God!!! Amen ��

Sweeping the membranes is a way to get your labour started as you approach or get past your due date.

For the doctor or midwife to do a membrane sweep a finger is inserted through the cervix making a firm, circular, sweeping movement.

The membranes are gently peeled off the lower segment of the womb internally therefore stimulating the release of prostaglandins, which could get your labour started.

Sweeping of the membranes, also known as stripping of the membranes, is a relatively simple technique, and it can usually be performed without admission to hospital.

During vaginal examination, the clinician's finger is introduced into the cervical os.

Then, the lower part of the membranes accessed through the vagina is detached from the lower uterine segment by a circular movement of the examining finger.

This intervention has the potential to initiate labour by increasing local production of prostaglandins and, thus, reduce pregnancy duration or pre-empt formal IOL with either oxytocin, prostaglandins or amniotomy.

The benefits and the Risks are:
• No increased risk of caesarean section
• Membrane sweep reduces the duration of pregnancy
• Membrane sweep reduces the frequency of pregnancy continuing beyond 41 and 42 weeks of gestation
• No increase in the risk of maternal or neonatal infection
• Discomfort during vaginal examination and other adverse effects (bleeding, irregular contractions) are more frequently reported by women who undergo membrane sweep.

Doc is saying labour will be induced on 20th August (EDD no longer 7th Sept) due to fluctuating glucose level.

Baby is lying well and jumpy. Madam still twerks frequently.

Please guys, do induction encourage vaginal birth (tho we're ready for CS), painful, any long term effects?
I was induced for my 1st baby ,so I Dnt see it as a big deal.just stick to whatever your dr advice,he knows best

Polyhydramnios

What is polyhydramnios?
Polyhydramnios means you have too much amniotic fluid in your womb (uterus). Most cases of polyhydramnios are mild or moderate.

The amniotic fluid that surrounds your baby serves as protection from sudden trauma like falls or blow to your tummy, from infection, and it is important in the development of the lungs.

Unborn babies would normally swallow amniotic fluid, and then pass it out as urine. This cycle is repeated till birth.
When this delicate balance is disturbed, the volume of amniotic fluid can increase rapidly.

Towards the end of pregnancy the amount of amniotic fluid usually decreases.

Polyhydramnios is usually diagnosed when amniotic fluid volume is more than two litres. It could be as high as 3 litres in severe cases.

If you notice that your tummy is getting unusually large and feeling more tight than usual, and that your skin is stretched and shiny , associated with being uncomfortable to breath, please kindly get a review.

Polyhydramnios could build up gradually, or very rapidly in twin / higher order pregnancies.

Symptoms of Polyhydramnios:
indigestion,heartburn,constipation,swollen legs,varicose veins, stretch marks

How is polyhydramnios diagnosed?
Ultrasound scans would provide a more qualitative assessment of the volume of amniotic fluid and can confirm a diagnosis of polyhydramnios.

The sonographer will measure the amount of fluid in four areas around your baby to work out your amniotic fluid index (AFI) or use the deepest vertical single pocket measurement.

Polyhydramnios is diagnosed when the AFI is more than 24 centimetres or if there is a single pocket of fluid that is more than 8cm deep .

The higher the AFI, the more severe the clinical problem is.

Sometimes the scan may also reveal a reason for the condition, such as a pregnancy complication.

What causes polyhydramnios and how is it treated?
The treatment you will receive will depend on the cause , severity of your condition and how far you are in to your pregnancy / how close you are to delivery.

In milder cases it could resolve gradually , but close monitoring would be essential.

If a specific cause for the Polyhydramnios is found in your case additional specialist care may be advised ; if so pls adhere to it.

Cause of Polyhydramnios

1. Uncontrolled Diabetes: This increases the volume of amniotic fluid your unborn baby produces; and you're not already being treated for diabetes In pregnancy I would advice your seek a diabetes specialist; because when you diabetes is well controller this would reduce the amount of fluid.

2. Infection : severe viral infections like could lead to increased am amniotic fluid.

3. Twin-to-twin transfusion syndrome (TTTS) : Twin pregnancy with identical twins whereby they share a placenta are at risk of a complication called twin-to-twin transfusion syndrome which could lead to polyhydramnios.

TTTS happens when one twin receives too much blood from the placenta and becomes overloaded with fluid.

This leads to the twin receiving the excess blood producing more amount of urine, which results in too much amniotic fluid.

Treatment could be to drain some of the fluid from around this twin to ease the pressure; and in severe cases laser treatment could be used to seal some of the placental vessels to the well-connected twin which could to even out the blood flow.

4. Developmental malformations like blockage in his food pipe (oesophagus) or a problem with his muscle control leading to impairment In the passage of swallowed fluid or inability to swallow amniotic fluid.

5. Chromosomal abnormality

A chromosomal abnormality, such as Down's syndrome or Edwards' syndrome, can sometimes be a cause of polyhydramnios.

Treatment of Polyhydramnios
Draining excess fluid In severe cases, treating any obvious cause like infections, diabetes, laser treatment in TTTS cases and considering c- section if closer to term.

♨️Try to remember that most women with polyhydramnios go on to have healthy babies, particularly if the condition is mild.

♨️Symptomatic relieve of heartburn due to Polyhydramnios are : Eat small amounts more frequently, not lying down straight after eating food, don't eat just before going to bed, Don't eat or drink anything that makes your heartburn worse, such as spicy food, some may find relief if they sleep propped up in bed, and you get antacid prescription from you doctor.

PREECLAMPSIA

Preeclampsia is when a pregnant woman develops high blood pressure and protein the urine after the 20th week (late second trimester or third trimester) of pregnancy. The exact cause is unknown but risk factors include:
�Obesity
�history of high blood pressure
�kidney disease
�Primigravidity (first pregnancy)
�Multiple pregnancy(twins or more)
�Age (Risk is higher if older than 35)

Symptoms may include:
❌Swelling of the hands and face/eyes (edema)
❌Sudden weight gain over one or two days, more than 2 pounds a week
❌Headache that just doesn’t go away
❌Belly pain in the right side, below the ribs (may present differently)
❌Vision changes including seeing flashing lights or spots, sensitivity to light and blurry vision
❌Irritability
❌Decreased urine output
❌Nausea and vomiting

If your doctor suspects preeclampsia, he/she will carry out series of examinations and investigations including blood pressure and abnormal swelling (some swelling in the feet and ankles is normal during pregnancy)others include: blood and urine test to check for series of things.
Ultrasound scan , non- stress test will also be required. These will help him/her decide if your baby needs to be delivered right away.

Women who had low blood pressure at the start of their pregnancy, followed by significant rise in blood pressure need to be watched closely for other signs of preeclampsia (even if the values are not that high)

�Treatment
The only way to cure preeclampsia is to deliver the baby. If your baby is developed enough (usually 37weeks and above) your doctor may want to deliver immediately so the preeclampsia doesn’t get worse. You may receive medicine to help induce labor or may need a C-section.

If your baby isn’t fully developed and you have mild preeclampsia, the disease can often be managed at home until your baby has a good chance of surviving. In that case, your doctor may recommend:
✅Bed rest, lying on your left side most or all of the time
✅Drinking plenty of water
✅Eating less salt
✅Frequent doctor visit to make sure you’re Ok
✅Medicine to lower your blood pressure

Sometimes a pregnant woman with preeclampsia is admitted to the hospital. Treatment in the hospital may include:
✅Close monitoring of both Mom and developing baby
✅Medicine to control blood pressure and prevent seizures and other complications
✅asteroid injections (after 24weeks) to help speed development of baby’s lungs.

�Delivery
You and your doctor will discuss the safest time to deliver your baby.
Factors considered include how well the baby is doing in the womb, severity of symptoms (preeclampsia can be very dangerous for the mother) and how close you are to your due date. (The further along you are, the better for your baby.)
The baby must be delivered right away if there are signs of severe preeclampsia. These include:
❗️Tests that show your baby isn’t growing well or getting enough blood and oxygen
❗️The bottom number of your blood pressure (diastolic) is over 110mmHg or consistently higher than 100mmHg over a 24-hr period
❗️Abnormal liver function test results
❗️Seizures (convulsions) or changes in mental function in mother (eclampsia)
❗️Fluid in the mother’s lungs (pulmonary edema)
❗️Severe headaches
❗️pain in the belly area
❗️Low platelet count or bleeding
❗️Low Urine output, a lot of protein in the urine and other signs kidneys aren’t working right.

�Complications
Severe complication include
�bleeding problems,
�separation of the placenta from the uterus before baby is born (Placenta abruption)
�rupture of the liver
�and in some cases Death (of both mother and child)

So call your doctor right away if you have any preeclampsia symptoms.

LEG CRAMPS IN PREGNANCY (Muscle pull)

If you have painful leg cramps in pregnancy, you're not alone. Many pregnant women have them in the second or third trimester, often at night. No one knows for sure why women get more leg cramps during pregnancy. It may have to do with changes in blood circulation and stress on your leg muscles from carrying extra weight. Your growing baby also puts pressure on the nerves and blood vessels that go to your legs. And some doctors say low calcium, or a change in the way your body processes calcium, may cause cramps. Leg cramps usually go away within a few minutes.

Call Doctor If:

❌You have swelling, tenderness, redness, or warmth in the leg.
❌The pain doesn't go away.
❌You have trouble walking.
❌You think you need to take calcium supplements.

Step-by-Step Care if you experience leg cramps in pregnancy:

✅Do calf stretches. Stretch with your heels flexed. Stretch before bed if you get cramps at night.
✅If you get a cramp, straighten your leg, heel first, and wiggle your toes. Gently massage your calf to relax the muscle.
✅Avoid sitting or standing in one position for a long time. Move around. Don't sit with your legs crossed or other ways that may hamper blood flow.
✅Take a walk every day or do other regular exercise to prevent cramping, with your doctor's OK.
✅Drink plenty of fluids to keep your muscles hydrated.
✅If you get cramps at night, take a bath before bed to relax leg muscles.
✅When you have a cramp, put a warm towel or hot water bottle on the area.

*HOW WILL I KNOW WHEN LABOR HAS STARTED?*

This is a common worry for first-time mums, but once labour really gets going you'll definitely know.
The early signs of labour are:
♦️A show (the mucous plug at your cervix comes away and appears as a pinky-brown jelly-like blob or in pieces.)

♦️You might feel pains that feel like strong period pains (these are the start of contractions)

♦️You might have pain in your lower back that comes in waves.

♦️You may find that your waters break, in which case you know that labour is, or soon will be, under way, or you might start by feeling contractions. These will be different from the practice Braxton Hicks contractions that you may already have had. They feel *stronger, deeper and more painful.* Look out, too, for waves of backache, as some women find that this is where they feel their contractions.

Once labour has started, your contractions will become *more regular and won't go away!*

When you are having a contraction every five minutes (and last 30-60 seconds), call your midwife, birth centre or hospital labour ward if you are giving birth there or go to the hospital as the case may be.

If this is not your first baby, or you are likely to have a quick labour, your midwife may advise you to call earlier than this.
If you're still not sure whether you're in labour, phone the labour ward or your midwife. They are used to dealing with false alerts so don't worry.

Omo let me rant small....
I can hardly get any good sleep at night these days. The discomfort is on another level. Turning is something else abi getting up to pee. Around 5am, my waist started doing me somehow. Heatwaves on my left side and leg only. Does contraction happen on one side of the body? Hmmm Baba God has the hour come? I rushed and took my bath ooo, bathed my lil Daughter, my hospital bags have been packed for God knows how long but something inside me kept telling me this baby isn't coming today....
Na so the heat wave just turn to muscle pull..O boy! I screamed. Hubby came into the room where I was thinking baby has landed and met me holding one leg up and hitting it....Omo na so the guy join me oo till it stopped. The leg is just doing me as if it's not my own again. To walk is a problem plus pelvic pain cos I can feel my baby's head down down there.
Pls bear with me....it's only someone wearing my shoes that understands hence my rant here....

WHAT IS FULL TERM PREGNANCY?
Pregnancy usually lasts about 40 weeks from the first day of your last menstrual period to your due date.

A full-term pregnancy lasts between 39 weeks, 0 days and 40 weeks, 6 days. This is 1 week before your due date to 1 week after your due date.

Every week of pregnancy counts for your baby’s health. For example, your baby’s brain and lungs are still developing in the last weeks of pregnancy.

Pregnancy usually lasts about 40 weeks (280 days) from the first day of your last menstrual period (also called LMP) to your due date. Your due date is the date that your provider thinks you will have your baby.

Babies born full term have the best chance of being healthy, compared with babies born earlier or later.

ACOG and SMFM use these definitions to describe term pregnancies:

�Early term: Your baby is born between 37 weeks, 0 days and 38 weeks, 6 days.
�Full term: Your baby is born between 39 weeks, 0 days and 40 weeks, 6 days.
�Late term: Your baby is born between 41 weeks, 0 days and 41 weeks, 6 days.
�Postterm: Your baby is born after 42 weeks, 0 days.

*How was the length of pregnancy defined in the past?*

In the past, a pregnancy that lasted anywhere between 37 to 42 weeks was called a term pregnancy. Health care providers once thought this 5-week period was a safe time for most babies to be born. In 2013, ACOG and SMFM updated the definitions for term pregnancies because research shows that every week of pregnancy counts for the health of your baby. Lots of important things happen to your baby in the last few weeks of pregnancy. For example, your baby's brain and lungs are still developing. Being pregnant for at least 39 weeks gives your baby’s body the time it needs to grow and develop.

These definitions can help more babies be born healthy by helping to prevent births that are being scheduled a little early for non-medical reasons. If your pregnancy is healthy, wait for labor to begin on its own.

Oga o. We will testify at the end. I am so hanging boot after this missionary journey. I have experienced a lot of things that were absent in my first pregnancy. I fainted in a public transport facility while returning home from work. Thank God for a sensitive driver. He was the one that poured water on me. He said he sensed I couldn't have slept off just like that. Help me thank God for my life, mamas.

Fibroids in Pregnancy: Overview and Effects

Introductions

Uterine Fibroids are fairly common among women. For some women, they worry about the effects of fibroids on pregnancy. They worry if pregnancy is even possible in the first place. Many women also may not even realize they have fibroids until after they become pregnant and the doctor finds them in an ultrasound.
Study showed that a lot of women of African descent have fibroids.

The facts are, in most cases, fibroids do not pose a problem during pregnancy, and however there are situations where they do. The effects of fibroid depend on the size of the fibroid and where it is within your womb. Some types of fibroids can be harmless for the whole life of the woman, while some can be problematic and must be removed.

#What are the likely effects of Fibroids on pregnancy?

1. Pain throughout pregnancy

Normally during pregnancy, hormones in the body cause the womb to grow and enlarge to accommodate the growing fetus. The increase of these hormones may also cause the fibroids to grow. Because of the fetus that needs blood to grow, the fibroids may grow beyond their own blood supply, which can cause one of two things to happen.

The fibroids may undergo what we called “degeneration” ; either red degeneration or white degeneration. With red degeneration, it bleeds into itself, and with white degeneration, portions may undergo cell death and become a foreign body. These can both cause temporary mild to severe abdominal pain, which can usually be controlled with over the counter medication.

2. Pregnancy complications

In some types on fibroid, there might be some complications during pregnancy. In the first trimester there may be bleeding and a slightly increased risk of miscarriage, depending on where the fibroid is in the uterus.

If it is located at the sub-mucosal, or grown into the womb cavity, it can prevent normal implantation of the pregnancy or disrupt the growth of the placenta.

In late pregnancy, there are three main concerns.

·●The first is preterm labor, which is more likely if the fibroid is large or if there are multiple fibroid.

·●The second, placental abruption (the abnormal separation of the placenta) can occur if the fibroid grows into the place where the placenta is attached.

·● Lastly, fetus growth restrictions are possible, but studies have not shown that it is any more likely with fibroid than without.

3.Delivery complications

During delivery, depending on the size and location of the fibroid, they may cause complications.

Including:

·● Causing the baby to lie in breech or transverse position

·●Blocking the progress of labor

· ● Expulsion of the placenta.

Please, note if it seems like the fibroid may cause problems, the doctor may elect to perform a Cesarean section. At this time, he or she probably will not remove the fibroid because the risk of excessive bleeding is too high. Typically, your fibroid will shrink as your uterus recovers.

#Summary

Normally fibroid should not cause problems during pregnancy, but you should make sure they are closely monitored by your doctor. You should be proactive and share all changes with your doctor. If necessary, there are options to remove the fibroid during pregnancy that you can discuss with your doctor.

In most situations, it is best to have symptomatic fibroid removed before conceiving.

What is breech?

When your baby is described as being breech what it means is that your baby is lying bottom first or feet first in the womb (uterus) instead of in the usual head first position. Most babies would be in breech position in early pregnancy, and in early pregnancy the breech position is very common. As the pregnancy progresses the fetus (growing baby) usually turns naturally into the head (cephalic) first position.

Most importantly, between 37 and 42 weeks (term), most babies are lying head first ready to be born.

It is important to know that, 3 out of every 100 (3%) babies are breech at the end of pregnancy. A breech baby may be lying in one of the following positions:

1. Frank/extended breech

2. Flexed breech

3. Footling breech

Why are some babies breech?

The reason behind babies being breech is not understood in all cases, it could just be a matter of chance, and why they remain in the breech position may not fully be known.

Some babies are in breech position due to clear stated and known reasons, therefore making it difficult for a baby to turn during pregnancy to the head down position.

For example the following reasons may predispose a baby to breech position,

1. If there is an excessive or very little amount of fluid (amniotic fluid) in the womb,

2. the position of the placenta, if the placenta is low in the womb, it could prevent the baby from turning head down (cephalic),

3. If there is more than one baby in the womb (twins, triplets or more), there is a higher chance of one or the babies being in breech position.

4. It the growing baby has an abnormality (like a cystic mass) it could also increase the chances of breech occurring.

It is important to note that most of breech babies are born healthy. For a few babies, breech may be a sign of a problem with the baby. All babies will have a newborn examination.



What can be done?

When you get to 36 weeks gestation and your growing baby is in a breech position, your obstetrician (doctor) or midwife should ideally discuss with you external cephalic version (ECV) which literally means (turning a breech baby in the womb), #externalcephalicversion

What are my choices for birth?

Depending on your situation, the available options for delivery in a breech presentation are:

1. Caesarean section delivery: this is a type of surgical operation where a cut is made in your abdomen and your baby is delivered through that cut,



2. Vaginal breech birth: where the baby is delivery in the breech position, some women would not prefer this and may opt for C/section.

There are benefits and risks associated with both caesarean delivery and vaginal breech birth and these would be discussed between you and your obstetrician and/or midwife, so that you could make an informed decision.

For example, if you have had a previous c/section and you now has a breech pregnancy, you would be advised to go for c/section, as ECV is not done on ‘previous c/section uterus’.


Caesarean delivery: Most respected reviews suggested that caesarean delivery is safer for the breech baby around the time of birth.

However, caesarean delivery carries a slightly higher risk for you, compared with the risk of having a vaginal breech birth, and it is important to note that Caesarean delivery does not carry any long-term risks to your health outside of pregnancy.

In a case where a patient chooses a caesarean delivery and then go into labour before the due date of the operation, your doctor (obstetrician) would assess the safest mode of delivery and whether to proceed with the caesarean delivery. Please kindly understand the reasons behind the doctors chosen method (as it is for your safety and the safety of your unborn child). There is no taboo in having a c/section, please.

In some cases, if the baby is close to being born, it may be safer for you to have a vaginal breech birth.

Vaginal breech birth: in some cases vaginal breech birth could be the ideal choice for some women and their unborn baby in expert hands. Every pregnancy is different and it may not be recommended as safe in all circumstances. Vaginal breech is a more complicated type of delivery, as the largest part of the baby is last to be delivered (in a breech presentation) and in some cases this may be difficult.

Vaginal breech delivery could be chosen in the following circumstances:

1. The person conducting the delivery is skilled and has the appropriate experience in delivering breech baby vaginally,

2. There are adequate facilities at the hospital for an emergency caesarean delivery (should this be necessary), this is a possibility,

3. There are no other obvious reasons why a vaginal breech delivery could be considered unsafe.

If your obstetrician determines the following in your case, s/he may advise against vaginal breech delivery;

1. your baby is a footling breech

your baby is large (over 3800 grams)
your baby is small (less than 2000 grams)
you have had a caesarean delivery in a previous pregnancy
you have a narrow pelvis (as there is less room for the baby to pass safely
through the birth canal)
your baby is in a certain position: for example, if the neck is very tilted back (hyper-extended) you have a low-lying placenta if there other medical problems in pregnancy like pre-eclampsia (blood pressure complications in pregnancy)

What you need to know.

Most form of pain relief used in used in normal delivery are ideal with vaginal breech delivery; in some circumstances, you may need an emergency caesarean delivery during labour.

Forceps may be used to assist the baby to be born in vaginal breech deliveries, as the head of the baby is the last part to emerge and may need to be helped through the birth canal.

In an ideal situation a paediatrician will attend the birth to check the baby as soon as the baby is born.

In cases where there are twins on board, for example if you have twins and the first baby is breech, your doctor (obstetrician) would usually recommend a caesarean section. The position of the second twin before labour is less important at this stage because this baby can change position as soon as the first twin is born. The second baby then has lots more room to move.

In areas where access to c/section may be difficult the skills met of conducting a vaginal breech delivery could be a real saving grace.
