I'm still finding it difficult to understand how d 28 days sequence starts. From the moment U apply for admission to UK university to the issuance of CAS and payment fee deposit, when exactly does the countdown to 28 days begin? At what time interval would I know when d 28days kicks off?

Given that medical school programs are challenging and time-consuming, but your relative has graduated. (Thank GOD!). Why hasn't he or she finds it necessary to invest valuable time to ask questions regarding their about to start professional career? Just wondering.

This write up is a guide for Nigerian Medical Practitioners who want to continue their practice in the UK.

Upon graduation from Medical School, I was eager to contribute my quota to the Nigerian health sector. Few months in practice, my hunger to serve Nigerians was replaced by another type of hunger; the hunger to continue my medical practice abroad, this was after experiencing over 9 months nonpayment of salary, governments lack of commitment to the health sector and the outdated equipment available to majority of Nigerian doctors. I developed a global mindset of pursuing personal development and contributing to the global health sector - the world is now a global village.

I searched the internet about the available options and learnt about PLAB, a two-part assessment exam that international medical graduates from outside the European Economic area and Switzerland are required to pass before they can legally practice medicine in the UK. The exam is conducted by the General Medical Council of the United Kingdom.

Passing the IELTS exam is a prerequisite to writing the PLAB exam. IELTS is a test of English Language proficiency. It is designed to assess the language ability of non-native speakers who intend to study or work in the UK. The IELTS requirement for PLAB is a band score of 7.5 (not less than 7 in each of the exam modules – writing, speaking, listening and reading). To prepare for IELTS, I used materials from IELTS online, IELTS buddy, IELTS Cambridge. IELTS Cambridge materials are available on the video section of Q and A App.

After passing my IELTS exam, I registered a General Medical Council (GMC) account on the GMC website to secure a reference number. After securing a reference number, I registered for the PLAB 1 exam. In Nigeria, the PLAB 1 exam is written in March and November in five cities including Abuja, Enugu and Lagos. The pass rate (cut of mark) for the exam is dependent on the average performance on the day you sit for the exams.

After passing the PLAB 1 exams, I registered for the PLAB 2 exams in Manchester. It is a bit tasking to secure a VISA for this exam especially if you do not have a sponsor, but its not impossible. Drawing from my experience, you’d need to maintain a healthy bank account over a 6 – 7 months period. A healthy account means that you receive a regular monthly payment in your account from a legit source, one you can explain and maintain a minimum balance of NGN800,000. The account should have minimum withdrawals.

I did not subscribe to the services of any agent to process my VISA, it was when I got to Manchester I discovered that a majority of the other participants from Nigeria secured their VISA with the service or guidance of an agent. Whether to use an agent or go about obtaining the Visa yourself is a choice.

The PLAB 2 exam is quite costly and should be well planned. There are a number of tutors for the PLAB 2 exams, they include Samson, Swamy, Hammed….. I understand that Samson now has presence in Lagos, Nigeria. This is a good development for Nigerian doctors, you can save a reasonable amount of money on accommodation by enrolling with samson in Nigeria as against traveling to Manchester – you will eventually need to travel to Manchester to sit for the exams. Note that the PLAB 2 test is not a written exam. It is a practical exam that tests for your listening skills and how you empathize with patients. Again, the pass rate is determined by the average performance of the participants.

To give you an idea of the cost of the exams, note that this is a guide as at the time I wrote my exams (2018-2019).
IELTS – NGN 75,000
PLAB 1 – NGN 196,000
PLAB 2 – NGN 395,000
Visa fees – NGN 120,000 for priority, NGN45,000 for Regular.
Flight ticket - this depends on your choice of airline and vary from time to time.
Accommodation in Manchester, I spent about NGN255,000 (one month period)

Whatever the reason for which you have decided to take your medical practice to the UK, the routes are similar for all, whether you have studied in Nigeria, Pakistan, India or Sri-Lanka.

The apprehension of an uncertain future usually puts people off from taking the first steps, but being armed with good information can make all the difference. As things change frequently, relying on friends who have gone before, although very useful, might not always be the the most uptodate source of guidance.

Step by step guide getting into the United Kingdom’s general medical register

1. Now that you want to start your journey of a thousand miles, you must first ensure that your medical qualification is acceptable by the General Medical Council. If your medical education was in the United Kingdom, there is nothing to check, but if you have been educated outside of the United Kingdom verifying your medical degree may be worthwhile before you proceed. There are a number of ways to do this; firstly, you should check your university on the world directory of medical schools. Having found your university, you need to check the qualifications the GMC does not currently accept. You can do this by clicking on this link or going to the gmc website.

2. Having made sure that you are eligible to practice in the UK, your next step is to confirm your english language proficiency. The most usual route is to take the academic IELTS test. The current criteria requires that you get a score of at least 7.0 in each testing area (speaking, listening, reading and writing), and an overall score of 7.5. Because these criteria might change, you need to get details from the GMC website. There are IELTS centres in different countries. In Nigeria, the test is conducted by the British council with centres around the country. Other routes of confirming english language eligibility can also be found on the GMC website. The IELTS test is the first in the series exams you’d take. Keep in mind that your IELTS certificate is valid of 2 years, so you’ll need to pass your PLAB 1 exams within this validity period.

3. Now that you have confirmed your English language proficiency, you need to take the PLAB (Professional and Linguistic Assessments Board) tests. There are two parts to this test- PLAB 1 and PLAB 2. PLAB 1 test is in the multiple choice question (MCQ) format and can be taken in Nigeria and other countries whereas PLAB 2 is in OSCE (Objective Structured Clinical Examination) format and is done in the UK. Adequate and relevant preparation is the key to success in these exams. Many people pass them at the first attempt. The new GMC guideline stipulates you that from september 2017 you can only attempt each exam a maximum of four times. Details about each exam and sample questions can be found through this link and on the GMC website.

4. The next step after passing your exams is to complete your registration with the GMC. You have done all the hard work and this is just the icing on the cake. You will usually start the registration process on the GMC website, you’ll pay the fees and then you’ll book an appointment at GMC for a check of your documents. At the registration stage, you’ll also need to ensure you are registered with your home medical licencing authority and you will also request a letter of good standing which will usually be sent from your home medical council to the GMC directly. There is usually also the need to independently verify your credentials at this stage and this is also done via the EPIC website.
Once you have completed your registration, the next step is to take a year off and rest from all your labour. That would have been possible if you were already a billionaire and you were just doing this for the love of it. For all the normal folks, it’s time to secure a job. There are different ways to go about this. You could either apply for resident medical officer posts, locum posts, Trust grade posts or you can apply for a training post. Your decision will depend on a few factors like your immigration status, the state of your finances or the availability of jobs. But whichever one you choose, it is always good to give a thought to your career progression.

Generally speaking, to progress in this journey, it is always good to take one step at one time, get up-to-date and useful information and follow your heart. There will always be wonderful people along the way to help you.

You didn’t give this young fellow a good advice, to be honest.

Sgtponzihater1 gave the most practicable advice that’ll also be most beneficial to him. The route you’re advising him to take is like cutting corners, and he’s going to be wasting much of his time doing that.

The first reason being FY1 is quite competitive and he may end up getting frustrated searching for a job after successfully passing all PLAB steps.

Secondly, MDCN that I know wouldn’t give him a letter of good standing just for holding a provisional registration. They’ll say they don’t know him( they did this to a friend but he was lucky to have been licensed in his country of training and had done internship there, letter of good standing was gotten from this other country). Except of course MDCN has changed the rule within the past 2 years.
Besides, Ukraine does not license their foreign Doctors after graduation.

Thirdly, the one year spent acquiring masters would have been well spent getting a proper hands on clinical experience from Housemanship training.

More than 80% of the foreign trained Nigerian Doctors run back home to complete the requirements for full registration, there’s a reason for that. This includes the ones licensed in their countries of primary training.

My advice is for the dude to go back to Nigeria and use one stone to kill 3 birds. Within 18months, he’d have been fully registered, completed Housejob, and passed PLAB one if he’s a serious person.

If he is doing Masters (in Public Health for example) at UK, what is the guarantee that he would be able to do housemanship at the same time during studies in UK?

I had the impression that he would need to get into foundation programme .
To get into foundation programme(internship in uk), the doctor would need at least English test and both plabs.
The second option being suggested appears to be faster and less expensive. The doctor pass MDCN exams ,do internship in nigeria, write plab during internship and disappear afterwards to uk.

This second option is also reasonable too. I think that my relative could at least return to Nigeria and write the MDCN Exams on November 2021 and get inducted and obtain his license first. He could apply for a UK Masters program which could either start next year or be deferred to next year 2022 (or even further).

The refresher course and exam lasts maximum 3 months. On completion of the exam he will be inducted into the Medical and Dental council of Nigeria, he can then leave after his induction for a masters starting in January.
He should sit the english exam which has a validity of 2 years, in this covid times gmc extended to 3 yrs i think.
Within the 9 months of the masters he should attempt to pass both plab 1 and plab 2.
At the end of his plab 1 and plab 2 with success. He wud apply for gmc registration.
As he has successfully been inducted into the mdcn, he will obtain a certificate of good standing from them which the GMC will need to register him.

He needs to go back to Nigeria, get registered with MDCN and do his internship and have a full license to practice. It's on this basis and letter of good standing that he can then register with the GMC.

If he proceeds to do Plab1 without the above, which his possible, he would sadly not be able to register fully with the GMC. As a fresh graduate, he should follow the ropes as it should be and he probably knows this at least from interacting with others.

my relative studied medicine at Ukraine and graduated this June 2021 (still residing in Ukraine). But my relative just graduated and has not done Housemanship or house job (whichever name it is called). The Second pressing issue that I still don't know is that is housemanship a compulsory requirement or condition that must be met before a medical graduate can write the PLAB 1&2 Exams?

Boss, it's so unfortunate you break an obvious rule your payment and completed online application form before pof 28days is not good, I just wish you can cancel and reapply if possible, because your pof is not up to 28days. Nonetheless, Please try get others opinion and let's see how it can be fixed.

@justwise ,Scooby and others what can he do?, This forum is very informative, you will see people mentioning it over and over the importance of this 28days pof before application. I also got some info from a reliable source that if you apply before your pof is matured is an automatic refusal. (My opinion)

You're welcome madam. Lol. Pls don't pay while counting 28days ooo, it means you're applying before 28days rule, just keep your pof first for 28days unchanged (standard rule) after that on the 29th you can print, then apply online, make payment for IHS and your visa fee. The truth is that a visa application site can tempt one to just easily complete the form, make payment, biometric without considering the availability or maturity of the pof..which can deny you your visa. However, if it was the TB test one failed or forgot to do for one reason or the other while others are intact, you can still beg and gate crash because your pof after printed can still last for 31 days.

I still don't understand this issue of waiting for 28 days over visa applocation and the proof of fee payment (POF) or so

I dont know if there would be any restriction on a Tier 4 visa (the "no doctor/dentist training], but I dont think PLAB 1 will be classed as training, but let him find out.

A simple phone call to the GMC would be the fastest way to find out.

OK no problem... the reason i ask is because my cousin is currently in Ukraine where he just graduated from Medicine this June 2021. He plans to get Masters admission at a university in UK and intends to travel to UK direct if possible and isn't coming to Nigeria. So he wants to be doing masters in a medical program while he prepares for the PLAB 1 and 2 exams in UK but he doesn't know whether both PLAB 1&2 exams could be taken in Britain.. He wanted to be very sure about it

For PLAB 2, you have to be in the UK, and a visiting visa allows that.

If you have not paid, I advise you do your research well. I was about to pay my deposit & I was advised against it.

1. They are slow in responding
2. After paying your £4,500 deposit, you will be requested to.pay your full tuition fee before CAS is issued. I confirmed this from the school bu calling them on Monday.
3. Read the email sent to you again to understand the fee payment
4. I confirmed their delay personally by chatting with some students on the school website.

All Universities are good for medical graduates (MG) considering majority of MGs won't be needing the degree after they pass their licensing exams....so let your cousin concentrate on cost of tuition rather than university ranking.. I would recommend Robert Gordon University, Chester University, Glasgow Caledonian University etc
