Yes you will request for it and add the email address of your school (the new school you want to attend). They won't give it to you.

After graduation (let's say first degree), clearance and everything, your school will give you statement of result (pending when your certificate will be ready). Later, you will change your statement of result to certificate (with a fee of course). When you want to further your studies (masters or PhD) or start working, that school or your intending employer will request for your transcript from the school you graduated from.
