please is it a good idea to write as a reason for denial "that i have no idea" i am really confused as i have no idea why i was denied. I was handed the blue paper when i applied for the tourist visa with my family. Please i would appreciate if some one could help me out.

When filling the financial verification form of the university, should he use his personal name or the name of his business?

Also during my visa interview, do I need to go with the CAC forms of his business or will his bank statement suffice?

Yes. You'd need the DS-160 confirmation number to complete your visa application.

fill and save the ds confirmation number. You can submit later. Latest, 72hrs to your appointment date.

That was actually my first EA request with this MRV. How did it get exhausted and pls, how many EA attempts are allowed ?

The EA has been exhausted since its no longer reflecting on your dashboard. Ideally, you should have one more trial at using it, don't really know what happened. Could be a system glitch.
