Depends on the school. Know some that don't require Recommendation letter prior admission. So just check in with your Graduate advisor.

same school, same sevis code, you don't need to make any new payment.

Hello. Its not too early. You can go for interview 120days to your program start date.

My visa was approved today and my class starts on Monday, school have refused to give me an extension, should I just defer and if I defer will I need to apply for another visa and go for interview.

defer to Fall session. You're not going for another interview, just collect a new i20 from the school. You can also enter 30days to your program start date. Congratulations!
