ISSUE 1:

What's happening to nairaland? Sometimes, responses made on threads are really irritating. Am a dr by profession and am particular about the health threads.

I don't know about you but I believe that people post new topics in the health section because they are looking for real solutions to their health related problems. But what happens?


Lets take the thread about the guy looking for a cure for stammering, for example. He posted the thread because he stutters or knows someone who does. Guess what? The guy really wants to find help.

I believe its okay for example, to say "I don't think there is any cure for stammering pele" @ ~`Sissy~

Its also okay to go ahead and encourage and/or advice the poster based on what you know or based on personal experience, which is also fine, I'll go on to say that I was impressed 'Riskie' knew about the SpeechEasy Implants,

However, its irritating to read some of the responses.

Stuttering is NOT comedy. I don't stutter but a lot of folks, myself included don't appreciate it when you make comedy out of something that brings so much frustration to someone else. Is that the purpose of the health section - Comedy?

Stuttering is a serious problem and people like the poster want to end their suffering and that's why they come to the health section of nairaland. It may not be stuttering but questions you want to ask anonymously.

If these people need comedy, they'll let us know but till they do ask for it, please DON'T MAKE YOURSELF LOOK STUPID ON THIS WEBSITE.

Most doctors who visit nairaland want to really help people and thats why they are here. Am sure most of them get pissed off when they read stupid comments and they don't even waste their time replying to that thread.

Please find a moderator that will do a good job with deleting clutter from the health section of your site.

Dear Poster,

I won't go into the pathology of stuttering but anyway, you have the following options:

1. Go visit a speech pathologist/ board certified fluency specialist and enroll in a speech fluency program. Am not aware there are any in naija but if ur in the states, then that won't be a problem.

2.There are anti-stuttering devices. I know its highly effective even though I understand its doesn't work in a small percentage of people. There are a few in the market and one of them is SpeechEasy(c) antistuttering devices.

3. There are certain drugs that reduce the activity of dopamine in the brain, but lets not get into this.

Anyway, do your research and it be best you meet with a speech pathologist. Good luck to you.!

Not trying to digress here but when will the pay of doctors become commensurate with the degree of stress that the job entails; forget all the bullshit about medicine is a humanitarian course.

In my view, it would take a very long time cos if doctors in nigeria want higher pay, the money has to come from somewhere and that cost would most likely be borne by the patients, US pays doctors very well and that's one of the MANY reasons why the US has one of the highest healthcare costs anywhere in the world. The masses in Nigeria ain't ready for that.

I bought The Guardian newspapers last week or so (can't really recall) and there was this advertisement placed by UCH for various positions including consultant opthalmologist and cardiothoracic surgeon and i remember seeing a salary scale of CONTISS 13 (1.4-1.9m annually). I was like WAT D f@#k?

These figures may not be exact but they are sure close to the reality and this isn't only in the telecoms sector. Aviation, Oil and Gas/Energy pay well but medicine is a different ball game.

For God sakes, it takes almost a decade of postgraduate residency to become a cardiothoracic surgeon.

I have a brother who flies with Bristow Heli (NG) Operations and he earns much more than a so-called consultant. And he's only been flying for so long. So I ask myself all the time, why go through all the stress and years of residency when I can put my talent to better use in corporate Nigeria - to the benefit of  the company and myself?

Reason Number 3: Girlfriends who are physically abused may also stay in a relationship for fear of what the person could do to them if they walked out (but I wouldn't b surprised if those who fall under dis category are very few)

Sorry, its really been a long while since I read this thread, Its possible that your genotype has always been AC and that there were discrepancies with the second test results, One thing you must understand is that in terms of geographic distribution, the hemoglobin C allele is found at the highest frequencies in West Africa and it has been associated with protection against malaria.

A good number of people have the C allele but are unaware of it.

Also note that most people with the AC genotype show no symptoms,

About your financee who is also AC, well, if you put the stats together as I explained in my earlier posts:

For every child you'll have together, there is a 25% chance he/she will be AA, a 50% chance he/she will be AC and a 25% chance he/she would be CC.

Maybe I should explain my rship with the family. I have a good rship with everyone in the family except the dad - whom I have met just once(although, he is aware his daughter is dating someone). They live in a different state from where I'm based and visiting them isn't feasible. She schools in the same state I live so when she goes home for holidays, I visit her and her folks every 2weeks. I believe her dad is sought of a tribalist but I'm thinking that is not to say that he would say NO to a foreigner like me (from another tribe) just that he would prefer his tribe person, The other person only became an issue when he and his dad (who happens to be a friend of her father, and from the same tribe) came to the house to talk of marriage.

She has also met everyone is my family except my dad and they all love her. My mum is crazy abt her.

Walai, talai. I thought of the part (how can you just be gisting with someone and end up sleeping with the person? What sort of conversation were you having? when you are not retarded) I confronted her about this yday night and she told me just about 3 things and she didn't want to continue the conversation ("I don't want to continue this conversation". I believe she gave me a beautifully edited review of what happened or maybe she was too ashamed to continue.

I have tried to rationalize and think about how it could have happened? Several things came to my head: My gf is a slot; she isn't a slot but was on heat at the time; she was druged;the guy was more successful than i; the guy raped her; etc. The more I thought, the more the thinking became more painful. The painful thing was that from my discussion with her, this guy works in some stupid government petroleum agency and isn't worth crap - so it definitely wasn't the money.

This experience made me realize something - that people change. When we started going out, she showed no love for material things and that made me spend more on her - it's just unnecessary to begin to talk about what and what was done or was not done. I just keep asking myself? What didn't I do for her? Was it love, my time or money? I believe with all the spending, I wet the seed of materialism in her but even then, only God knows.

I wanted to break up on the grounds that since the dad's friend had already brought his son to the house, it would only be a matter of time before she would graduate and then they would return again. I raised this with her some days back and she maintained her position of not being interested in the guy and that it was a mistake. It's me she wants. I believed she was sorry.

Let me add, I was like "fine, it happened. Promise me it will never happen again because I want to trust you completely. I believe that trust is so important in a rship blablabla, " Tell her to think abt it, and like a day later, she's like she's afraid to promise me and fail again. ----That is a true statement but it could also mean, If it could happen this time, I'm not sure I can control myself when next it happens. As someone pointed out earlier, she could have made up the story to make me break up with her but I don't understand why she would do that when she could have just told me what she wanted. I'm a very easy person to deal with. Besides, we talked about it and she says she doesn't want me to break up with her and she's really sorry it happened. That's the summary.

Even if we were breaking up, I wouldn't collect any of the things I bought for her. First off, its not me; secondly, I have never believed that the material things count. I appreciate the anonymity of Nairaland in helping me out in this matter 'cos this is something I can't even tell any of my friends or family otherwise, I know what they'll say. Plus they'd base their decision more on the material gifts, which in the big picture, are irrelevant.

I don't know but with this experience, I'm beginning to think that women are all about MONEY and material things which just irritatingly pisses me off. Including this experience, I've had 2 in the past that didn't go well because of the "I'm freaked abt money factor".

thanks so much for your encouragement. I remember back then in university, when these sort of things happened to people around you, its just so easy to tell them to break up with their girlfriends but the situation is really different when it hits you in the face, particularly when you're looking to get married to the person. People, please help me out here. It happened about 1week ago and although I've forgiven her, I've been asking myself why it happened? So what's my story, please read on:

I've been going out with this girl for the past 2 years now and we have always been very happy with the intention to get married in another 2yrs after her NYSC. I really did love her and made sure she lacked nothing (the most recent was the N150k laptop I got for her about 1 month ago), even if that meant a little inconvenience. Before this experience, I believed she really loved me also and we certainly never had any major problems except for very minor arguments and stuff. The only difference between us is that we aren't from the same tribe.

Okay, she happens to be a student who went home for hols. Last week, she calls me to tell me that some old man - a friend of the father and from the same tribe comes to their house with his son for , you know, "this is my son, he is working and is interested in marrying your daughter". According to her, her dad tells the friend and his son that his daughter is still a student and to wait till she graduates.She didn't tell me about the incident until 2weeks after it happened. All this time, I call her at least once daily. About a week ago, during a phone call she tells me that she had sex with someone. WHHAAT I was mad with anger on the phone - she sees this and then tells me she was just kidding and wanted know how I would feel about it.

I'm still trying to get over this "experiment of hers" when a few days later, she resumes school, comes to see me and then tells me that it really happened. Can u please imagine that? I ask with whom she did it with and after some hesitation she tells me it was with the guy (who came with his dad) to come see her father. Apparently, her folks ask her to go and visit the guy (at least that's what she told me) and during that visit, they were gisting and from gisting, it happened and that was it. She says it was a mistake.

She literally broke my heart and I was insistent upon a breakup. She begged me not to; I thought about it for a day or so and decided to forgive her on the premise that we are all human and sometimes we make mistakes. I have forgiven her but walai, forgetting things like this is not easy. I have never cheated on her and I have been trying to rationalize things in my head.

People, please how the hell could something like that happen? Do women make stupid mistakes like that. I really need your candid opinion in this matter.

I wrote this in the hope that it will benefit someone who reads it. So what really is menstruation? why does it occur? what happens during menstruation? These are questions a lot of men and a few women - i guess - on nairaland want answered. In answering some of these questions, I have neglected the unnecessary technicalities (the endocrinology is extremely complicated) which will only confuse the entire thing.

First off, a little about female reproductive anatomy:

Basically, all the female reproductive organs are either a part of the external Instruments or the internal Instruments. The external Instruments also called the vulva (which isn't important in this discussion) includes the following: mons pubis, labia majora, labia minora, vestibule and vestibular glands, and the clitoris.

The internal Instruments include the vagina, the uterus, a pair of fallopian tubes, a pair of ovaries. For the purpose of this discussion, please bear in mind that the ovaries contain little eggs in them. At birth, there are about 2 million eggs but almost half of these number are and will remain non-functional.

Why does menstruation begin when it does?

By the time a female baby is born, all of her reproductive organs (both external and internal) have been completely formed but menstruation does NOT occur, because before puberty, the necessary hormones needed to trigger menstruation are produced in extremely minute concentrations.

At puberty, there is a marked increase in the level of these hormones and they begin to have varying effects on the entire female body including the external and internal Instruments. So besides the development of the Instruments, they also "kick start" menstruation.

What is the purpose of menstruation?

Well, without it, the woman won't be able to conceive.

What about cycle length?

Cycle length varies from woman to woman but generally it is 28 plus or minus 7 days (normal range is 21 - 35 days). A cycle length is the time from the 1st day of one period (the very first day blood started flowing) up till the 1st day of the next period.

What happens during menstruation?

Many things happen but basically things that happen take place both within the ovaries and the uterus (or the womb). In the following explanation, I will assume that the woman has a 28 day cycle:

Ovarian Cycle: Within the ovaries, the important activities are:

Phase 1: Period of egg growth (scientifically called the follicular phase) - On day 1 of every cycle, 5-15 eggs within one ovary begin to grow but only 1 will reach full maturity. The others will regress and will remain non - functional. This phase occurs from Day 1 of the cycle up and by Day 14 when these phase ends, there is 1 mature egg within the ovary. 

IMPORTANT POINT TO NOTE: Please understand that the end of one phase marks the beginning of a new phase, that is why Phase 1 lasts from Day 1 to day 14, phase 2 occurs on Day 14 while Phase 3 begins on Day 14 and ends on Day 28.

Phase 2: Period of mature egg release (scientifically called ovulation) - So now that 1 egg has reached full maturity, it isn't doing anyting within the ovary, so it is expelled via a process called ovulation. In some women, this process is associated with pain (called a mid- cycle pain). The mature egg is released from the ovary and then enters the fallopian tube and REMAINS there - waiting for sperm.

Note the following:

Note that a woman has a pair of ovaries and fallopian tubes but most times, only 1 mature egg is released from one ovary (just imagine that the ovaries take turns in releasing a mature egg). There are certain instances when both ovaries will release a mature egg and both of them may or may not become fertilized by sperm. If fertilization does occur under this circumstance, it results in the birth of non-identical twins.

Phase 3: Called the luteal phase: Before the mature egg was released from the ovary, it was protected by a fence of cells but during the process of ovulation, this surrounding fence of cells was left behind inside the ovary. During this luteal phase, the fence of cells develop into what is called a a "corpus luteum" that begins to produce progesterone.

Uterine Cycle

So where does the blood come from, to understand we have to look at the Uterine Cycle. It also has 3 phases that correspond to the phases of the ovarian cycle.

Phase 1: Menstrual Phase. Occurs from Day 1 - Day 5 of the cycle. In many women however, the period of bleeding varies between 2 and 8 days. We'll talk about what happens here later on.

Phase 2: Called the follicular phase. Unlike the follicular phase of the ovarian cycle, that of the uterine cycle begins from Day 5 of the cycle and ends on Day 14 of the cycle.

Before going on, let me mention that the uterus has 3 layers: an outer layer of protective covering (perimetrium), a middle layer of thick muscle (myometrium), and an inner layer (endometrium). The endometrium is in turn made up 2 layers of cells. An outer layer and an inner layer.

Okay, so during the follicular phase, certain hormones from some part of the brain begins to stimulate the cells of the endometrium so they begin to gradually increase in number and size. During this phase, the growth of the endometrium is stimulated by estrogen.

What is the fertilizable period of a mature egg released during ovulation?

The answer to this is simple. Remember we said the mature egg is released from the ovary and goes into the fallopian tube waiting for sperm to fertilize it. After its release from the ovary, the mature egg will remain fertilizable (or fertile if you would call it that) for 24 hrs after which, it can no longer be fertilized by sperm.

Phase 3: Called the secretory phase. Remember that ovulation was on Day 14 and after this time, the fence of cells left within the ovary began to produce progesterone in increasing quantity. Well, the progesterone produced by the corpus luteum within the ovary stimulates cells of the endometrium and they begin to further increase in size and make the endometrium "more suitable" for the implantation of a fertilized egg (whether or not conception occurs).

Please note again that the corpus luteum can only remain alive for 9 days after ovulation (from Day 14 up till Day 25). After this time, they die off and so progesterone production stops.

Phase 1: Back to menstruation. Progesterone is a chemical that has so many fascinating properties including:

* blocks the production and release of the chemicals that cause menstruation ie. prostaglandins (responsible for the pain women feel during menstruation)
* keeps the muscle of the uterus in a relaxed state
* is thermogenic (that is, it causes a rise in body temperature)
* others

As from Day 25, the amount of progesterone present in the system begins to decrease markedly  while levels of prostaglandins that cause menstruation will steadily be on the increase. The prostaglandins cut off the blood supply to the innermost layer of the endometrium - poor blood supply means the cells present in that layer are killed. By Day 1 of the new cycle, the cells of this inner layer of the endometrium are shed off together with some of the blood that escapes from blood vessels underneath as menstrual blood.

Basically, we say that phase 3 of the uterine cycle begins on Day 14 and ends on Day 1 of the next cycle.

All of the above repeats itself in the next cycle.


What is the Menstrual Cycle?

The menstrual cycle is a term that refers to both the ovarian and uterine cycles.

Safe and Unsafe Periods?

Safe period are basically those days during the menstrual cycle when you can have unprotected sex without the risk of pregnancy. There are 2 sets of safe period in the menstrual cycle - a period before ovulation and then after ovulation.

Before ovulation - You want to exclude Day 1 - 5 or any of the days when the woman is bleeding and then you also want to exclude 3 days before ovulation. Why? Because when sperm enters the woman, they remain alive and active for an average of 2 - 3 days after then. So therefore, before ovulation, the safe days are Days 6,7,8.9. and 10.

After ovulation - You want to exclude 2 days after ovulation so you can definitely be sure that the egg is now unfertilizable. So safe days are Days 17,18,19,20,21,22,23  up till the last day of the cycle.

The use of safe and unsafe periods as a form of contraception is called the RHYTHM METHOD. The major drawback with this method is that you must be sure of the ovulation day.

Determining which day is the ovulation day?

This is critical for the success of RHYTHM METHOD and its very easy to do this.

Remember that the cycle length varies from one woman to the next but fortunately, the time from the day of ovulation to the 1st day of the next cycle is ALWAYS, i repeat ALWAYS, 2 weeks.

With this fact in mind, I guess it becomes very easy to calculate your date of ovulation once you know your cycle length and it is relatively constant from month to month. There are many factors that may make the cycle length vary from month to month but that is beyond the scope of this write up but 2 important are: emotional factors and use of oral contraceptive pills.


I do hope someone has gained immensely from this write up and not be confused about whether or not pregnancy can occur during menstruation?

Most people confuse eczema and another skin condition, pityriasis versicolor. A causative infecton cannot really be found for eczema however a fungal infection causes pityriasis versicolor. You'll need to consult with a dermatologist to actually tell whethe its eczema or pityriasis versicolor you have, For the latter, the standard treatment is:

- Tabs Ketoconazole 200mg (1 tablet is 200mg and is taken once daily) for 10 days
- Clotrimazole containing cream (such as Sabresten) rubbed over and around affected area (twice daily - morning and night) for 4 weeks
- Nizoral shampoo used 2ce weekly for 2 months, and then 1ce a week for 6 months. (Wash your entire body and leave lather on for 10mins. Wash off). The extended treatment with the shampoo is due to the high risk of recurrence with pityriasis,
