The few ones I know is this:
If she lost so much blood while trying to deliver naturally, and blood transfusion is needed but because of poverty from both couples, the husband might go for a cheaper blood. And cheaper blood means blood that's not been screened. Such blood can have any form of infections. A close friend of my neighbor died through such means.

If she's hypertensive and her bp is not monitored all through the pregnancy and during labour time, it can skyrocket and voom! She's dead after delivery.

If the CS is not done properly, she could die as well.

If her cervix is small and cannot push out a baby but refuse to take the option is CS. She and the baby can loose their lives or the baby can be suffocated and killed while forcing it out from the narrow cervix , the natural way .
