Imanuelle: In certain situations, people improvise, especially when it has to do with life. A first aid treatment for a start, even on the floor could have had her alive till they can do the needful. If a facility is filled. First aid on the floor doesn't cut it. It probably means that the capacity of the healthcare workers, equipment, lab services etc has been reached. Would it have been better if she was placed on the floor and there were no nurses available to see her or Doctors to care for her, or even no lab facilities for her care?? We do more harm by providing inadequate care. We need to see the big picture. You can't use plaster to fix axe cuts. |

A society where you can walk into a "pharmacy" and order any drug you want without regulation.
I think introducing Morphine to a Nigerian market where there are no laws is a big mistake and will create a a lot of drug addicts in the near future.
BTW, Morphine is not more effective than other pain killers. Pain medications work in a different ways and sometimes, other meds are better for pain management. |

We can achieve this by comparing the organisms found in a 'healthy' mouth versus hers and use a selective culture media for that 'out of place' organism. It is DEFINITELY NOT an infection.
Infections don't last 10 years.
Due to the blisters, she might sometimes develop a superimposed infection.
But the primary problem that she has had for 10 years is not an infection.
It is impossible for anybody to make a diagnosis online, so you are probably better off taking her to a good hospital.
Hereditary telangiectasia however can cause recurrent AV malformations which may be the reason for the bloody blisters in her mouth.
You can google that and see if her symptoms possibly suggest that diagnosis.
And like somebody else said earlier, doing a swab, microscopy and culture is probably the worst suggestion because the mouth has so many organisms that are normally supposed to be there.
Bottom line, look for a good DOC |

The cost of a Nigerian Visa for American citizens is $160 and there are a bunch of miscellaneous fees that you also pay at the embassy. Regardless of what people might argue, Doctors are the head of the health team. And if we want to be CMDs, we should also be willing to take the overall fall for malpractices in the hospital. And we should lead the way in bringing about reform. And that is why my stand regarding this issue is that regardless of who was deficient, the Doctors managing are responsible for picking up on the fact that the said child was not getting better and try to make the necessary adjustments. I know some of the limitations in our Nigerian health system, and that is why I said they should try. |

Infections are a problem worldwide and that's how indians market themselves... the way they talk, you will think they can make the boy scarless. I dont know about lasuth dressing practices but there are rules... unless soaked, once or twice daily dressing is ok. If someone was now negligent with dressing, na their wahala but indians na winch ... do they have a video canera in nigeria to know where the infection is from?  If a wound grows MRSA. I know it was hospital acquired. Depending on cultures, you can determine where an infection likely came from. There are lots of organisms that can only be carried by humans. Nigerian hospitals lack basic facilities. Hand sanitizers, gloves. Even water to wash your hands. Let's not pretend like the infection couldn't have been hospital acquired. I'm not here to argue medical facts. Dirty wounds have a high chance of infection. I know that. And that is what we are there to prevent. And that is why the kid must have been admitted. Or else he would be home. Most wounds can be dressed once a day. But wound dressing will only go so far if the infection was so bad. And the bottom line is that if lasuth handled this matter the way it should have been handled, there wouldn't be any story. Its that simple. |

Any plastic surgeon can do this and do not need any special treatment in india or anywhere else for that matter. -The child was treated in a world class PRIVATE hospital in india and NOT an indian govt hospital. A world class private hospital here in naija that has a plastic surgeon in its pay roll will equally do a good job. - I'm sure when the poor boy was admitted in LASUTH, the father had nothing on him except a promisory note from Gov Fashola that the boys medical expenses will be paid by the govt. Now, anybody who knows how govt works in naija will know that fashola's statement was merely political and will most likely not be honored in any govt establishment. Rather, the boy's father will be expected to provide for the dressing materials and antibiotics needed to treat his child. Something he failed to do. if at all he will be reimbursed by the lagos state govt, that will not be immediate due to bureaucratic process that delays such payments. Neither LASUTH nor the medical staffs will eat promisory notes, but they wont complain. Fashola is their employer, thus they will continue to do whatever they can to help the poor boy. Obviously their effort was too weak to have any effect.. The boy was flown to india with an adequate treasury chest backed by delta state governor. if he had gone to india with promisory notes, he would also have been rejected. pls, lets be fair in our comments. thanks.
note, I'm not exonerating our medical staffs, but as far as this case is concerned it has financial constraint written all over it.

armadeo: A badly infected wound would always smell. We don't k ow if the infection was hospital acquired or not. There lies the issue. The fact that the wound was badly infected while the kid was getting health care (Mum complained about the foul odor) and the infection was not adequately controlled until the mother had to take the child out of the hospital is the issue. If the wound got better, and mum wasn't misinformed, she wouldn't have a bad story to tell. Did the kid get the care that he was supposed to get? Healthcare in Nigeria is in pieces and we have to take responsibility to make it better. |

We don't have the culture results, so we can't really talk about what organism was grown to make the Indian Docs believe it was hospital acquired. I'm pretty sure one of them has taught you (where did you do your residency, let's start from there). Please park very well.
The Diclofenac case, the doctors she met first didn't even ask her if she had ulcers, one of the doctors I mentioned had to come over and take over her case.
The Appendix for constipation happened in Surulere, the doctor had to lewere going to hit him with malpractise.
The Asmathic was on her first trip as a teenager to Nigeria, she went to Delta state and was brought in wheezing and complaining of shortness of breathe, they took her temperature and declared it malaria
How can you rule PID without a single pelvic exam and base your diagnosis on a urine analysis which showed Staph and nothing else? Which is what that slowpoke did and was mouthing off just like you, until I calmly made a phonecall and he was forced to explain to one of his former specialists s how he got that diagnosis. Not once did that slowpoke refer me to a Gyno.
Was it not at Unilag Medical centre I took a friend there (she had fallen into a gutter and was still cracking jokes, so I knew it couldn't be worse than a fracture) only for the doctor there to POP her and declare a broken bone. I carried her home and took her to a very high up doctor at Igbobi, who looked at us and went "but this is a hairline fracture"
I do not walk into any hospital in Nigeria without getting a 2nd opinion. If my immediate family members (who used to teach doctors) are not sure of their current crop of colleagues, I see no reason why I should say "all is well with Nigeria Medical Association" I bet your stories are true. I have heard of lots of horror stories and I have seen shameful things happen too. There are good doctors that make bad mistakes and there are bad doctors everywhere in the world. What is missing in Nigeria is accountability. Every professional (not only Doctors) has to be held accountable for their actions, otherwise, we will continue to do things the same way. And the way we currently run things is barely getting us anywhere. Everything needs to change. |

Dr Bashing, Naija's favorite past time.Is the practice here without flaws, no, but then I hope we are all aware one of the top leading causes of death in the US is medical error from prescriptions, yes Google that. No system is without its flaws and if you will criticize, please do it objectively and truthfully.
I"ll not bother typing out the problems in the Nigerian Health sector seeing its not the topic here but I'll discuss LASUTH. The personel (medics,nurses etc) patient ratio is alarming. The staff there are ill equipped and overworked, LASUTH almost never has bed spaces for new emergencies not willing to wait for this reasons, and that's all I'll say.
As for this case, they should sue the Apollo Hospital for such unguarded statements ,its not their first, its starting to be a habit.Its a calculated effort to win over naïve Nigerian patients,which if the referral letter truly carried the LASUTH Letterhead the relatives should be sued for fraud shikena. Yes its possible the wound was infected and they did not carry the relatives along on management and please what bite wound does not get infected, its a dirty wound for crying out loud on of the aims of management is to control and treat an infection on it, i can promise you there's no Nigerian nurse that will dress a wound five times a day in Nigeria's teaching hospital with the ratio of like 1 nurse to 20 patients sometimes. if you like write it as such the best you can hope for is to send your registrar's or house officers to dress twice daily, after "begging" the nurses to dress at least thrice daily. Add that to the fact that the hospitals where on strike forty percent of the time last year you start to get the true picture in a case like this
Nigeria has equally seen lots of mismanagement cases by Indian and foreign hospitals, you don't see anyone crying spilt milk on Indian hospitals/colleagues and for those who don't know its one of the codes of practice in medicine.Lots of cases diagnosed here were said to be wrong including cancers which turned out to be right, Dora comes to mind and I've seen other examples with CA that is.
Now For Nigerians: your medical teams nurses, Drs etc work round the clock with remunerations, settings and systems foreigners cannot imagine and definitely not produce results with you really should not pull them down but ask sincerely what are the problems, how can we help fix this. If we refuse to do that, the exceptionally good ones and the ok ones with any dignity left will leave, its happening already and its Nigerians that will reap the consequences with time. You have valid points, but for a system to work, we have to constructively criticize what was not done right in order to make it better. It is good to talk about the success stories, but in Nigeria, there are too many unnecessary deaths from very trivial issues. Some of which you have identified. The US health system is also absolutely imperfect, but it keeps on evolving, only because the Government, Doctors, Nurses and patients as well are held responsible for their actions and inadequacies. We need to change how we do things after identifying the root cause of the problem I agree. A lot of Doctor bashing goes on, especially by people who have no idea about the health system. And that will never end. It happens even in developed countries. It is just better controlled, and the docs are paid well enough that they can be empathetic with patients despite this.
