You have precisely 3 Neurosurgeons in Nigeria. In GH Lagos Island, doctors can't do PCR test (so when Ebola came, all Lagos doctors were scared, those that trained in Uniben even more so, because no-one had taught them how to do the test, even though Edo state has the most experience with Haemoraghic fevers).
Nurses from School of Nursing nowadays are taught a lot of Theory and little practicals. And most are not very good.
The machines at LUTH are so outdated, CT scan machine either caught on fire (last year) or they will take light. Same with Endoscopy at GH Lagos Island, no-one knows how to use the machine.
It isn't just equipment that is your problem in the Nigerian medical field. Most of you are badly trained (I have had doctors prescribe Cimetidine at a child's dosage, when I was in my 20s, I've had doctors declare I had Cerebral Malaria, when it is more common in children and young adults, I have had doctors treat me for all sorts, doubling anti-biotics without being able to write a clear referral, suspecting I had PID without a single test to rule it out etc). I have seen a doctor take out a clean appendix because the patient was constipated, I have seen doctors not accurately know that a patient with the right symptoms and age was having mini-strokes, they kept prescribing painkiller upon painkiller. I saw doctors who didn't know how to do examinations on the pelvis of a patient who had been treated a year ago for fibroids and was having pelvic pain be prescribed so much Diclofenac Sodium she OD'ed and they didn't know how to do charcoal activation to clear her bloodstream and could merely put her on drip, I have seen a patient die of a thyroid sugery that is a one-day affair in US on a Nigerian hospital, last week someone broke his arm and was taken to Igbobi, this week he died of an infection. I have seen asthma treated as malaria and they doctors treating the patient didn't know how to use the nebuliser the patient's parents brought. The patient's parents then had to donate 5 second-hand ones to the hospital, so that mistake never happened to another child.
You guys have to be trained properly. I'm speaking because I personally, off the top of my head, can count 2 Consultant Pediatricians, One Dermatologist, 1 consultant Gynaecologist, One Surgeon, 1 Neurosurgeon, I can't remember the last 2 what they studied and all of them either taught at LUTH or worked at UCH, Uniben, GH or another major teaching hospital.
None of them think half the doctors in Nigeria should be practicing, half of them think most of the teaching hospitals should lose their license to train certain fields of study (like Surgery in Medilag). All of them wonder why their colleagues don't further their education. Most of them don't trust their valuable patients to nurses (when one had a patient who needed to be monitored overnight for internal bleeding and his wound drained, he told doctors to do it and forbade nurses close to the patient).
I am sorry, but I really feel that firstly no doctors should be allowed to graduate, and massive enforced training for all doctors should be done for 5 years, overhaul all the medical schools, with upgraded equipment. Yearly testing of doctors should commence for another 5 years. We need a govt that should assume we have no doctors in Nigeria and place the country under a National Medical Emergency so we take drastic measures.
It is so bad that none of my family members will be allowed even in the smallest surgery in Nigeria, not even childbirth is an option in the nicest private hospital in Nigeria. I have doctors in my family and when they hear you're pregnant, will ask you when you're flying out to have the baby because they have lost confidence in 70% of their own colleagues.
And that is a shame.
You believe you're knowledgeable about the Nigerian health System, you are not.Not about the state of service delivery and definitely not about the personnel. To adequately criticize the system you have to be within or have been within the system.

1stly Nigeria has at more than 11 trained neurosurgeons working in Nigeria not 3, and more qualify every year after 7 years of intensive postgraduate training working round the clock literarily.A medic treating asthma as malaria are u kidding with all the obvious breathlessness wheezing and other tell tale signs? as well as the fact that the patient is a known asthmatic? tell me they misdiagnosed cystic fibrosis, or an ailment not commonly seen here, and I'll believe you write with no malice but asthma?

someone overdosed on Diclofenac and your first thought Sir/Ma in Nigeria was activated charcoal in Nigeria, really? Like what are your primary concerns, Gastric mucosal ulceration with bleeding, an acidosis and renal damage perhaps amongst others and to buttress the fact you have zero clue of the Nigerian health system, your first point of concern was finding activated charcoal? where have u ever seen activated charcoal in Nigeria, remember its taken this person a while to be discovered and to reach the hospital.

On their inability to do a PCR (hope you are aware most of the lab strengthening in Nigeria for PCR was done via foreign organization donations? they are meager like a few per state and definitely not within the jurisdiction of the Clinical Dr's in Nigeria, they are not doing PCR yet "the others" are shouting "stealing" of roles .

An appendectomy for constipation, we've established you're kidding right? and Lastly, Please tell me the gold standard for diagnosing a pelvic inflammatory disease and how many Nigerians consent to that with the risk of a negative result and we'll come back and discuss why you probably had just blood tests and Vaginal swabs with an abdominal scan before a diagnosis of pid was made.

Dr Bashing, Naija's favorite past time.Is the practice here without flaws, no, but then I hope we are all aware one of the top leading causes of death in the US is medical error from prescriptions, yes Google that, you see they discovered tired Doctors made life threatening errors(Yes even in the hallowed US) the systems response to that was to reduce the hours their doctors (residents) work to not more than 48-72 hours per week. No system you see is without its flaws and if you will criticize, please do it objectively and truthfully.

I"ll not bother typing out the problems in the Nigerian Health sector seeing its not the topic here but I'll discuss LASUTH. The personel (medics,nurses etc) patient ratio is alarming. The staff there are ill equipped and overworked, LASUTH almost never has bed spaces for new emergencies not willing to wait for this reasons, and that's all I'll say.

As for this case, they should consider suing the Apollo Hospital and other Indian hospitals involved in issuing such unguarded statements,its not the first,its starting to be a habit.Its a calculated effort to win over naïve Nigerian patients,which if the referral letter truly carried the LASUTH Letterhead the relatives should be sued for fraud shikena. Yes its possible the wound was infected and they did not carry the relatives along on management and please what bite wound does not get infected, its a dirty wound for crying out loud and one of the aims of management is to control and treat an infection in it, I can assure you there's no Nigerian nurse that will dress a wound five times a day in Nigeria's teaching hospital with the ratio of like 1 nurse to 20 patients sometimes. if you like write it as such the best you can hope for is to send your registrar's or house officers to dress twice daily, after "begging" the nurses to dress at least thrice daily. Add that to the fact that there is no accountability should the Nurse refuse to follow the orders or she dresses just once (which is why you send your interns)and the fact that Nigerian hospitals where on strike forty percent of the time last year you start to get the true picture in a case like this

Nigeria has equally seen lots of mismanagement cases by Indian and foreign hospitals, you don't see anyone crying spilt milk on Indian hospitals/colleagues and for those who don't know its one of the codes of practice in medicine.Lots of cases diagnosed here were said to be wrong including cancers which turned out to be right, Dora comes to mind and I've seen other examples with CA that is.

Now For Nigerians: your medical teams nurses, Drs etc work round the clock with remunerations, settings and systems foreigners cannot imagine and definitely not produce results with, they work unrealistic hours at the cost of personal health, family and work life balance.We really should not pull them down but ask sincerely what are the problems, how can we help fix this. If we refuse to do that, the exceptionally good ones and the ok ones with any dignity left will leave, its happening already and its Nigerians that will reap the consequences with time.

So the poorly Paid overworked Nurse actually did her weight, Bp and other vitals? The Dr patiently takes a history of her illness,thereby excluding all other possiblity including a "brain tumour from Fever" she fears probably caused in this part of the world from TB,or HIV which may have given her a "cough" or other symptoms he actually took time to rule out despite his obviously overworked state and her poor attitude whilst the same person in this facility apparently dispenses medications and all he gets his more attitude and Insults.

This article is more than sarcasm,its a Nigerian attitude,If your Dr Diagnosis malaria,nothing stops you from getting a second or Third opinion,but this culture of shaming Dr's and Nurses by people who aren't trained is classic inferiority complex that stems from an unjustified sense of adequacy...

To buttress my point,are examples, a man comes in with eye problems,the Dr Diagnosis an advanced brain tumour and recomends surgery,the relatives go around screaming ignorantly he came in with only eye problems o and they wanted to do surgery,we dismiss it to the anger stage of grief, a child with very high respiratory rate,u ask them not to feed orally explaining fear of aspiration,they say don't mind them they ave come again,child is eating well,they feed the baby aspirates ,dies u hear he only had pneumonia o... ,next you hear of assault to the health workers,one even drew a gun over matters they are not trained to judge,and it gets worse they shot practitioners who explained there was no space in the morgue, no sentence followed,In nigeria examples abound.

its more than grief now,ItS becoming a culture in Nigeria,of disrespect to the profession that gives so much despite its handicap,and those who still have any sense of decency would do well to condem this trend..

The Ekiti State Governor has actually accepted the unconstitutional decision,it is left for the speaker to take the matter to the court of law and knowing how slow the process ussually is, Knowing it will first be addressed at the court of appeal and later the supreme court.

This scenarios are an abuse of the system,one that became rampant only in our recent history (minority votes overthrowing the majority)seeing how it had not been addressed in the past,no group of persons prosecuted for it,led to its appearance again on the national stage ,well planned,well executed,Police gates ,teargass and all...... What they did not count on/for was the determination of the "other lawmakers" not to allow this evil go unchallenged immediately this time.

I believe there where more of instances like this in various state House of assembly in Nigeria (I hope nairalanders who remember this incidences can please cite this examples) however the point remains this incidence at National Level is not a new affair in our nation,the reps where wise enough to identify that and I believe whatever actions they took in their resolution of that matter was obviously targetted at proctecting the shards of our fragile democracy.

I feel really sad for your comment because you are a man that reason like a female. Our generation is different from that of our grandparents where the man work and the wife stay at home to raise the kids. The economy situation in Nigeria is different from that of our parents and it now require both couple to work so as to keep the family financial situation good. More women attend school and get good jobs unlike in the past. In a modern African home, it is now require both the spouse to help pay the family bills. Its no longer the sole responsibility of the husband to pay all the family bills. No man wants to marry a wife that will spend all day watching dstv. Women should also work and help with the home finances too. The days of our grandparents is over and we are in a mordern society.
Actually nothing has changed,in the era of this grandparents,women worked and had income,they were farmers,nurses,teachers and artisans,women made and kept their income,which was how they could singlehandedly send many of their children to school in a polygamous setting,it was not dependent on the man.

Why is this generation of men trying to shirk responsibilty is what I don't get.Some Nigerian men of today ask women to bring all their income,this much I'm sure of.I repeat nothing protects this women when his relatives or second wives come calling in the future,or when he excercises his african right and sends her packing out of his house.this is why what obtains abroad cannot be set as the yardstick for nigeria...the women should always remember this.

And for the men, Don't get married if you can't afford to simple.and definately don't start demanding a womans income to assist you make ends meet,its inexscusable in africa.

I'm passionate about topics like this because I detest injustice,and by virtue of my profession have seen a lot of women with issues such as this.

To prevent abuse and ensure responsibilty each spouse may bring a percentage of their income to run family expenses,say 50percent each or whatever suits their fancy.so if the man earns way less than the woman,he must still drop that meagre amount that forms the percentage,That's his contribution that's what makes him responsible.But to ask a woman to drop all her income on the table,must be a temporary arrangement and must be a request not an order.demanding for ALL in any other form whether she is a spendthrift or whatever reason is criminal.

A man like that is a lazy thieving opportunist sponging of the woman.He needs to get real and find work that will bring in more income and please let's all not endorse this wrong on this pages,pple who practice this evil are here seeking validation

We must as a society speak out and condem with one voice practices such as this that has impoverished families and women for generations.This issue is as real as it gets in our society.

It is common,very common that some yoruba women bring their earnings at the end of the month ,kneel and put it on the table infront of their husbands who decides how the money is distributed/spent,he can keep it all if he wishes,just like she kneels to serve his food etc.

It is a primitive culture certain sects or religious bodies actually still promote.

At the end the woman is ussualy exploited and taken advantage of.

In nigeria,this is a common scenario,mostly with the yoruba culture and amongst certain religious sect.

I blame the women for marrying such primitive men,I blame soceity and pple such as nairalnders that commented carelesly for its existence in present day soceity.

It used to be a sign of submission but it failed the prior generation.The men took the money enriched themselves abused their spouses seeing they had nowhere to go.

So a man and wife build a house with all the womans salary,and his relatives or his 2nd wife take over,remember she has no other "savings" hence totally dependent.Not just her her children too.

So in this day and age a man says to his working wife "bring all ur salary" in a culture that has no protective laws for its women and some folks come on here and say its submission? Its not submission its evil and theivery by a lazy man,men like that eventually find other places to validate their ego,they spend on other women who look at them as a rich provider...

I've seen and heard of a few like this,non ended with the man staying despite the woman agreeing to remit all her income.like it is with every bully,giving in never solves the problem,the demands just get worse as the woman looses all her respect completely.Don't these women respect themselves? To respect yourself means u carry yourself and treat urself in a manner deserving to be respected. women,learn from the prior generationn,look out for ur family,but look out for urselves too,have ur own egg nest.if not for u,for ur children.Ignore society,if you trust the man completely,fine but remember its your responsibilty to protect yourself and your children with your choices,be wise.choose well.

If they were worried about her welfare they should have taken this matter to the courts and not act as they did.

The comments I've seen so far on this thread is good reason this grandmother must be made to face the full wrath of the law in Nigeria.

Let me try and make things a bit clear.

Its a norm for volunteers to be paid a stipend an amount not comensurate to the service they actually offer,so Dr's that earn millions abroad are paid like 250k a month accomodation and meals by Volunteer ts like an honorarium to appreciate service.
So the question is what is the true value of this work they are doing.

The way to appreciate it is how much would u pay an artisan,an accountant,a musician,an officer of the military on outside posting for a job that would risk his life/inconvienience him and that of his whole family per day...

Then u get the perspective of how this 50k a day is actually a stipend.and that the govt still fails to pay it,or they have to request and be spoken to harshly, that anyone criticizes them even from the comfort of a computer screen is Distasteful,and disrespectful to the very lives these pple sacrifice as they come out for this job.You all should really be careful,I believe God Dey.

I must comend the workers local and expatraite combatting this deadly disease.May Their gallant efforts not go unrewarded.

That said,why is our minister of health sounding so unproffesional in this matters,one would expect as a Dr he would know better.

The Epidemic threshold for a disease such as this is 1.meaning when u record a single case,u put all your emmergency preparedness gig into full swing.Now is not the time to be saying we have "only one case left",one case is a big deal,because it means they are more unreported in the community.that's basic epidemiology.

Can anyone shed light to the statement attributed to this same ministry that ebola 1.9 b is released to the ministry for expenses and not for ebola in particular so will be used to procure other items since there was just one case left?

In the spirit of tranparency and common good,can the ministry pls give us a list of treatment/emmergency preparedness center for all states equipped evidenced with pictures and staffed...

We shouldn't joke with preparedness,that's the stage to make a real change in the statistics.And All states must be prepared,don't let your pple become data for ebola.

The solution is simple, take the emmergency response away completely from the govt hands because some clowns in govt will always sabotage it.

I prefer foreign org,but a serious NGO needs to stand up now and set their own sys... Accomodation for staff,feeding,transp sys,drugs,med supplies,isolation tents etc its not cheap and we'll have to donate to it

Truth is for every epidemic threshold crossed in naija recorded in the last 10yrs,it took foreign intervention,msf esp to bring it down,meningitis,cholera,lead poisoning etc...

They used mostly local med staff with few foreiners cordinating....

To stop this plague,take it away from the govt,they should give the funds to an org not the ussual naija govt offices.

That said containing this thing isn't cheap @ all,bio hazard suits 20,000 non reusable.serious donations will be needed from all coporate bodies too.

They say u know how civilized a society is by how they treat those that are disables,those that are helpless.

So our fathers that were serious,the ones that refused to take bribe,that insisted on due process and integrity in the prior generation? Most died uncelebrated and their efforts unrewarded,the charlattans and crooks,the ones that cut corners soceity celebrated.

It continues till date,no issue is pursued to a reasonable conclusion,the list is long.... why then should this generation take anything or anyone seriously.

Moving forward, all this returants,public offices and places should be mandated to get proper touchless handwashing device abeg....,flowing water should be compulsory,etc.

Its situations like this that make society better,we really shouldn't let this crises go to waste.

I like an earlier comment,minister of health should take a shift @ one of this centers,Oya LASG commisioner of health,over to you,take a shift for one month.

What will u want in place before you start in this highly infectious unit and as u return home to your family everyday.

The truth is ebola just butresses the point of nigerian Dr's.Health workers are always exposed to illnesses we've lost friends,colleagues to infectious diseases they had no time off to treat,to lassa,some have got HIV ,heppatitis on the job,etc.

Hazard allowance cannot be 5k,and it is so wrong to ask anyone to take risks you the leader of the team will not be willing to take,so commissioner,lead your team.

Meanwhile,how many hospitals have been closed in lag for ebola?

Lastly,

Since we're trading blames on roles,its not the pharmacists that should ave found a cure for this.

PHD Dr's are awarded their PHD for research,and its expected they continue their research throught life.

In nigeria they want to be "consultants" and see patients.its not their fault really,its the goverment that failed to give grants for research,so the greater percentage of research projects in the country over 80 percent is funded by foreign aid/grants and self funding,so the whole consultant brouhaha is just them seeking relevance.

This chaos,will be a good time for NMA and Johesu to team up write everyones roles and duty in gold,write demands and expectations from the goverment,write the plan to ensure sanction for erring members,it can be bussiness as ussual,and then we will see peace.Its that easy....

You all should make good use of this crises.its will be like the phoenix born from adversity.

So I'm a nigerian trained Dr,and I have refused to practice clinically in Nigeria with any Nigerian institution public and private for yrs now. I must say when it comes to clinical medcine in Nigeria,it requires ingenuinty,diplomacy and tact,where all that should have been needed was protocol,discipline,teamwork.It erodes one's dignity and sanity,no one appreciates it,no one respects you.All good you do,and we do a loooot of good,we tell ourselves we do for "God" what I feel is deep bitterness,a sense of so not worth it,except taken as a calling.

So this same state and Nation,that has betrayed our trust and our profession again and again in all ramifications is asking come volunteer and they throw money to solve the prob,50,000 a day,here's why I won't and this are my recomendations.

I won't because,

Frankly Nigeria/Nigerians does/do not deserve it.They've disrespected their clinicians and their roles over the years and I simply refuse to "re-join" the choas now.

No one I repeat No one will look out for the clinicians as ussual,- so they've offered a life insurance policy,I would have expected serious pple to offer residency training slots in new infectious disease dept,training abroad,etc but so you know their tots.

Summary is I don't trust the Nigerian govt and her pple to look out for her professionals they never did,I doubt they'll start now,Red cross,MSF,The UN,and other ,look out for their staff in such great detail... They ave a security dept that safeguards with security report,all threats not to talk of actual assaults physical or verbal which are attended to propmtly and addressed,ure treated with respect,everyone understands their roles and responsibilities,and should u get infected or injured in any way on the job,(though you've signed an indemnity clause),they still do all they can to ensure your safety and comfort,and take responsibilty for your treatment should the need arise (unlike here where u're on your own to treat yourself,you don't even get time off,hope u all know lagos state does not pay for any time off for their locum staff including maternity leave )those guyz I will trust to volunteer with,they've earned it...they pay peanuts by the way,so its not about money.

My question and recomendation for lagos and Nigeria,

Now will be a good time to bring in those "cuban" Drs we always hear them boast about,I'm sure NMA will have no issue with it.

Get more quarantine centers before u need it.I ave said this one before now,so you know I'm right they are unserious jokers

Housing for this team of volunteers,hope u don't expect them to go home dly seeing what risk they are taking,and pls ensure they are really comfortable during this period.

Short volunteer periods,max 1 month which is wat others do,its easy to concentrate on protection for a short period ,with a long time frame,lapses occur.After this short period,quarantine and test them before letting them go home.

Lastly respect those that actually turn up,dr's Nurses,pharm etc.no state official or secetary of oga should turn up and start spewwing rubbish.

Lastly,it will cost money,no short cuts.So pls get enough protective wears and supplies,no we've run out of cotton wool or protective wear,pls manage...

Its not enough to learn new words and their pronunciations,you have to learn how best its used.While reading you discover phrases and words used in different ways,witty and interesting too,you get to use those words and phrases in future and people think/say wao that guy has a great vocabulary,but its just "a thousand" books speaking.

the more you read the more comfortable u get socially,because u'll have the right words to say just what you want said in most situations,and gradually you start to pick the writing style of different authors,as the voice in your head finds uninhibited expression.

so the trick Sir,is to read.
