There are so many causes of infertility. Some female, some male related. The fact that both people involved have had babies before is somewhat reassuring.

They both need to get a comprehensive interview with their doctor, who will then determine what tests will be required.

Your symptoms sound like a partial intestinal obstruction, just like you have been told.

Big question is, where is your obstruction? Small bowel or large bowel? And what is the cause of your obstruction?

It is very important to determine the cause in order to know what the treatment should be.

Associated weight loss may be concerning for a cancer, but there are also other causes of weight loss.

You need lots of tests to be done - including A CT scan of your abdomen, a Colonoscopy and maybe an upper GI endoscope.

Make sure you follow up with whatever hospital you can in order to determine the cause, and go through with your treatment. Your symptoms could be due to a serious medical problem.

Pain with urination is usually due to urinary tract infections(UTI). May also be due to irritation of the v@ginal area by urine if you have labia that are irritated by yeast infection.

Do you also have v@ginal itching or a discharge?

Speak to your doctor about your symptoms. UTIs in women are common and can be treated emperically (without any tests)

Skin problems are very difficult to diagnose, especially when the rash is not seen. Best thing will be for you to go have a doctor take a look at your skin.

Staph aureus does not usually cause infections in the female genital tract. Like somebody else mentioned, it is likely a contaminant.

The common organisms that will cause a discharge can not be grown with usual cultures.

Possible common causes of a discharge are

1. Neisseria gonorrhea - Requires special cultures. There are rapid tests for diagnosis. I don't know if these are available in Nigeria.
2. Chlamydia trachomatis - Very difficult to culture. There are also rapid antigen tests for this. Don't know about availability in Nigeria
3. Yeast infection - Can be cultured in the right medium. But diagnosis usually doesn't need a culture. Causes white clumps in discharge with itchiness.
4. Bacterial vaginosis - Not really an STD. Caused by overgrowth of the normal bacteria in the V@gina. Causes a fishy smelling discharge especially during inter.course.

All these problems can be treated with antibiotics, but first of all, you need to have a diagnosis.

Some of the problems with healthcare in Nigeria is that Doctors and Labs do not have the diagnostic tools needed to make this happen.

If you tell me what antibiotics you have been prescribed and how long you took them, I can make an educated guess about what has been treated.

Also, another possibility is re-infection. If you are still engaged in unprotected inter.course, you may be getting re-infected from your "source".

If you truly have blocked tubes, there isn't any medicine that is proven to reverse it. You just might be wasting your money and time!!!

Blocked tubes are usually due to scar formation from an infection, and the tubes are so small that once they are scarred, they are usually blocked for life.

I don't mean to disappoint you, but also, it is unfair to give false hopes to your patients.

Like somebody said earlier, find a reputable gynecologist who may be able to determine the extent of blockage that you may have. If it is not severe, it may be correctable by surgery. Otherwise, start to consider other methods of conception.

If TB means tuberculosis, then there is no way to determine TB level. You either have the active infection or you don't.

Some common symptoms are chronic cough (more than 3 weeks), blood in phlegm, weight loss, night sweats, fever.

If you think you might be infected, get treatment early to avoid spreading it. There are lots of government clinics that offer free treatment.

Chyvy: Pls Doc, what are the ways of preventing UTIs? I've had 3 recurrent UTIs for the past 4 months. I notice its usually triggered by sex. During this time, I saw a doc, did some tests and took the prescribed antibiotics. It cleared off. But this time around, its coming back again after sex. Pls what do I do? This is really getting on me and my marriage. Should I avoid sex altogether for now?

I've read all the do's and donts online about peeing after sex, drinking a lot of water. And washing the Cores befor and after sex. They seem not to be effective. Pls help.
Keep on practicing the measures that you read about e.g peeing after s.ex, cleaning your privates before and after. Also, drinking cranberry juice seems to have some benefits.

If all fails, you can discuss about postcoital antibiotic prophylaxis with your doctor. Sometimes, in women with recurrent UTIs, that may be related to se.xual intercourse, taking a single dose of an antibiotic after intercourse will help to prevent UTIs.

Most likely a se.xual transmitted disease. You need to get screening tests from your doctor and avoid se.xual intercourse until you've been treated.

Depends on the severity and cause of his heart failure.

Viral causes may be completely reversible while Heart failure from myocardial infarction (MI) will depend on many variables such as the size of the infarction and other comorbid medical problems like Hypertension, Diabetes.

Your question is like asking how long it will take for the engine of a car to "knock". Too many variables to consider.

And life expectancy will definitely be best with regular use of prescribed meds.

I've heard that Coronary angioplasties can be done in a few of the new private hospitals in Nigeria. I don't think bypass surgeries are done in Nigeria.

These are very major procedures and are not done just because people have chest pain. There are indications for everything. And a Doctor's responsibility is to make sure that he is not harming his patient by the things he recommends.

Firstly, non-invasive tests like an EKG and an exercise stress test would be done to make sure that your chest pain is related to your heart or not.

Sorry, but I feel I have to jump into your discussion here.

First, It is unlikely that you will have untreated angina (heart pain) for 3 years and you will still be alive right now.

There are lots of problems that will cause chest pain that are unrelated to your heart.

Nitroglycerin is rescue treatment for people who may be having a heart attack. Basically, you take it and then you go to the hospital because like I said earlier, untreated angina is progressive and will kill you. Also, If Isordil gave you headaches, Nitroglycerine will most definitely do too.

Clopidogrel/Plavix is not supposed to be given without a proper diagnosis. It can cause life threatening internal bleeding.

Like Joshuapharm said, you need to go to a hospital, maybe find a cardiologist. You need to have a stress test (they may have you run on a treadmill while they monitor you vital signs and your EKG to see if the chest pain is truly heart related)

Atenolol which is a beta blocker is a great medication for hypertension if you do not have side effects from it. It is one of the medications recommended by the American heart assciation because it has been proven to prolong life in people with heart disease.

Also many medical associations recommend that males over the age of 45yrs old take a daily 81mg aspirin for cardiovascular protection.

You also need to have your cholesterol checked to see if it is high or if you need to have that treated as well.

Bottom line, go speak with your doctor about your problems.

Govt hospitals are bad too and the people living in those areas cannot afford the kind of hospital with enough equipment and manpower without good health insurance. Medical treatment cant be cheap and the government's proving it without providing alternatives or even training medical personnel to fill the void
The alternatives are the registered hospitals and clinics that were not shut down by the Government.

No matter how bad Government hospitals are, the devil you know is better than the "devil" you don't.

I am not aware that Comprehensive wellness check has a technical definition. It has nothing to do with how many "major human organs" are examined.

I think comprehensive wellness check is used just its English meaning. And this just means it is exhaustive enough to be able to identify medical issues that are important to the patient's age and status.

i really need ur advice, after searching for a job for almost three years, i was told to cme for a pre-employment medicals with a company, bt after the medicals i was told i wnt get the job anymore cos i am hepatitis B positive. I felt so bad cos i tot all my toiling in school to get a 2.1 has finally yield diviend. Since then i ve bin reading abt heb B and a possible cure bt frm my research, there is no known cure for nw so i have bin beleiving God for healing cos right nw i dnt even have enough money to feed talk more of treating hep B which i understand is very expensive. Bt the good thing is that, i feel fine and i hardly fall sick and even if am to treat myself, i need a job which i dnt have for nw. My dilemma nw is this, ave bin invited for another pre-employment medicals for a job in a financial institution, i really really need this job and am scared of being rejected again even though i believe i wnt be a health risk to others in the organization,
So PLS WHAT DO I dO? How to i convience the doctors during the medicals to consider my plight. Thanks everyone in anticipation for ur advice.

It is not right for you to be excluded from a job in a financial institution based on a diagnosis of hepatitis. However, na Naija we dey.

Hepatitis B is usually self limiting and most people clear the infection within 6 months. because it is a virus, there is really no great treatment for it. So, people who do not clear the infection may be tried on antiviral drugs which have a lot of side effects and may even be unsuccessful.

I don't know what you mean by convincing the doctors because they have to be ethical and professional. So, I don't think they will be able to exclude those results.

By the way, why do you need a hepatitis panel to be done in order to work in the financial sector? It shouldn't be a required test, but like I said earlier, Na Naija we dey.

Do you have a copy of the lab results? If you do, I may be able to tell you if this is a chronic infection, or it might be a recent infection which might clear up soon.

Also, hepatitis B is highly infectious through exchange of body fluids - Blood transfusions, s.exual intercourse. So please try to protect your loved ones from your body fluids, so that they do not become infected.

Morning sickness is a somewhat normal but annoying symptom of pregnancy. It usually resolves for most women when their pregnancy reaches its 2nd trimester (around 13 weeks)

Some women have severe symptoms which may be a predictor of a more serious problem e.g preeclampsia.

Also multiple pregnancies tend to have more severe symptoms likely due to increased amount of pregnancy hormones due to the larger placenta size.

Natural over the counter remedies that have been proven to work are ginger and vitamin B6 (pyridoxine 25mg up to 3 times daily)

My urine was declared Normal. However my Blood pressure is on the high side..not hypertensive yet.

but at times i feel full...feel musky in my eyes..and this hoarse tight feel in my chest and dizziness..sometimes feel like fainting.
please whats going on...i am 6.1 foot tall and weigh 108..which is bad...i want to start dieting..but cant in this condition..as im on drugs..i will start watching it after im ok..help i want to get back to 100% and be very vigilant henceforth about where i eat, how i eat...etc.
The information you have provided is so vague and makes it impossible for me to help you with any information.

If you feel very sick with chest tightness and weakness, you should be in a hospital.

Note that the only reason that you will get typhoid fever is if you drink water or eat food that has been contaminated with human feces. Also, you will most likely be having a fever.

If you have had a stroke before, then every symptom becomes important because you have higher risks of having another stroke which could result in more permanent disability.

It is normal to have some increase in your pulse rate with activity. However, in your cousin that has had a stroke, I would be concerned about heart conditions like atrial fibrillation, which will increase your heart rate, may give you an irregular pulse and will significantly increase his risks for a stroke.

What was the reason for his initial stroke? If that isn't been addressed, he could be at risk for another stroke.

oluplus: Hello doc, I woke up last week and fell down and started feelin dizzy, went to a hospital and was told sugar level too high, wt malaria and typhoid. Was given drips , injection and drugs. Was discharged d same day, though still very weak. Since that time, till now. Am still not balanced, though, no more dizzinessn but I still stagger once in a while as if I'll fall. Went to anoda hosp, and was told to go for E/U/Cr tests and ECG .Am going to a lab outside now to conduct the test. Pls what could be wrong wt me. What other tests can I do. The truth I don't take sugar at all. Pls help me. Tired of malaria drugs for a week without improvement
Sounds like you should be in a hospital.

There is limited help you can get on this forum here.

Infections usually present with fever. Also, there are a million reasons for feeling tired, and a lot of them could be life-threatening.

I will advise that you go back to a hospital and speak with your doctor, so that you know the details of why you are sick, and also ask to know when he expects that you should begin to feel better.

Are you urinating large quantities frequently?

Do you have trouble with your urine flow, or the flow is normal?

Do you have pain when you urinate?

How old are you?

There are many reasons for urinating frequently, and if you do not give your doctor a detailed history, you will end up with a wrong diagnosis.

Some possibilities for urinating frequently include urinary tract infection, benign prostate enlargement, Diabetes mellitus.

Note to all Nairalanders here. . . Medicine is not magic. Doctors are trained diagnosticians that use the information you give them to arrive at the most logical conclusion.

A penile discharge in males is usually due to an infection. Most likely se.xually related.

YOU NEED TO ABSTAIN FROM SE.XUAL INTERCOURSE until a diagnosis has been made to prevent spreading the possible infection.

You will need urine tests for gonnorhea and chlamydia, which are the ccommonest STDs. Also, you will need a urine culture.

These will help the your doctor determine what organism is causing your symptoms, so he can treat you with appropriate antibiotics. Because specific antibiotics work for specific organisms. There is no one antibiotic for all problems.

In summary, you need to see a doctor. Physically.

The usual cause of this depending on your age will be an enlargement of the prostate (an organ within the male reproductive system)

This is common in black males from about the age of 50 years.

It is usually benign (not cancerous) but sometimes, it could be cancer.

Younger males may have problems with urination due to urethral strictures (scarring inside the urethra). This is mostly due to infections or could also be due to a previous injury to the urethra.

You will probably need a prostate exam to determine the exact diagnosis and rule out dangerous causes for your symptoms. Benign prostate enlargement can be treated with medications

I know a lot of Nigerians do not do this, but it is important to have routine wellness checks with your doctor every year.

Unix 1: Good evening doctors in the house pls I would like some advice on my ailment
I gave birth in the month of FEBRUARY OF this year about 3months after it I started noticing hottness in my lower stomach when ever I want to stool, later the hottness became permanent in my lower stomach,I went to a hospital near by after complaining the doctor gave me ampicillin and multivitamins told me to drink a lot of water eat a lot of vegetable and fruits 2 days after taking the drugs the symptoms disappeared for 2 weeks in a row and later came back as cramp in my legs.
I went to another hospital I was asked to go to the laboratory and do SWAP/HVS/,URINE & BLOOD
Swap came back and I have CLORIFORM ORG, in the URINE I have moderate growth of ECOLI & BLOOD test was negative. the doctor placed me on anti-biotics for 4days with other injections according to the drugs written on my lab result which ciprofloxacin drip but even after taking the drugs and spending days in the hospital,I still feel the hottness around my leg area that is from my labs,help what can I do about it?
What do you mean by "hotness" in your legs? Do they hurt? Do they feel numb? Is the sensation in your leg abnormal? Is it on leg or both legs? Do you have trouble walking? Does the "hotmness" happen all day r it comes and goes.

An infection is unlikely to go on for that long without treatment. Are you taking any medications.

Do you still feel the "hotness" in your belly? Your symptoms are quite vague and it would be difficult with the information you have provided for any doctor to help with some insight.

There could be a million reasons for your symptoms but I do not have enough information from you to be able to make any diagnosis.

The antibiotics probably didn't work because your urinary tract infection was not the reason for the problems in your legs.

Some antacids cause diarrhea. Milk of magnesia is a classic example that can be used for treatment of heartburn as well as constipation. The magnesium component of it specifically.

Nobody knows it all!!!

The internship programs were diploma courses offered by Schools of Medical Laboratory sciences (usually attached to teaching hospitals). The change was enacted by law (Act 11 of 2003) involving change of the names of the regulatory body then from the Institute of Medical Laboratory Technology of Nigeria; whose graduates were called Medical Laboratory Technologists (diploma holders) as against the current nomenclature: Medical Laboratory Science Council of Nigeria who among others, give licence to graduates of Bachelors of Medical Laboratory Sciences, referred to as Medical Laboratory Scientist.

Sorry about your predicament which unfortunately seems like a lack of proper and effect career guidance before and at tertiary institutions, resulting many intelligent young men like you losing their way and ending up in an occupational cul-de sac.
For many, there seems to be no easy way out except taking the longer tortuous route of going back to university and taking the requisite qualifying courses.
But that shouldn't be a reason for eliminating Diploma courses. All that should happen is that the course be changed to a Diploma in medical laboratory sciences, and the MLSCN would supervise these Diploma courses and ensure that they meet the necessary standards.

An example would be scrapping polytechnic engineering courses because you can get engineering degrees from a university.

If there is a need for more Lab med scientists and Universities are not producing enough, then that would be a rational thing to do.

Unfortunately, I think because of the stagnation and lack of funding that we have to health in Nigeria, even though millions of people need healthcare, facilities are limited!!! And no matter how much manpower you have, a kitchen can only fit so many cooks.

The biggest hoax that I have experienced in the history of Nigerian medicine is Staph.

As a well meaning Nigerian, I therefore felt compelled to try to shed some light on staph.

First things first. . . Staphylococcus is a bacteria that is normally present on human skin. It helps to protect us from other bacteria/fungi.

Don't get me wrong. Staph can cause infections.

But what happens is that everything that happens to anybody is blamed on staphhuh But we all have staphhuh So, how can this make sense.

It is like accusing the devil of any bad thing that happens in the world. Sometimes, it is the devil, but most of the time, we actually did it!!!

Who goues to spend 6 yrs only studying anatomy. thats bull crap. there's no future there. in the US we have medical technology, technical courses and other assorted courses that serve as assistants to professionals in the health field. thats whats necessary. Secondly , wake up folks. this relieance on the oil industry as the saviour for jobs aint happening. they're drilling their own oil in the US and moving to electirc vehicles. apart for some minor exports to cover shortfalss in the future NJa will drink its oil. then what. everyone will kidnap everybody else for money? Think of starting stuff for your selves . I know the next excuse is no capital. then get some damn capital.if u gotta sell shit , work as house boy whatever it takes , make it happen. My mum started out selling mops in traffic in the mid 80's now she's independently secure in her own business . No govt contracts or other useless stuff. It can be done. GET of Your LAZY ASS and MAKE IT HAPPEN.
The capital required to make anything happen in the health sector is simply impossible. Except you are Dangote who got his money from huh?.

The Government needs to fund certain things. Nigerians should have a say in how our money is being allocated. And until they increase allocations to health from 0.1%, the best anybody can do by himself is going to be like working in the dark ages. Kinda like what is happening in the Health sector right now.

So, if una get popsie/uncle or sponsor wey dey politics. Help us relay the message to them.

Great discussion. Biggest problem is that Nigeria as it is right now, limits the potential of basic medical sciences because there are no research facilities available.

So, don't blame yourself for the passion that led you to study the course that you love. Blame the government that has failed to put down an organized structure for things to operate.

We are a consumer nation. Think of it, it this affects so many courses of study in different ways. We hardly can produce anything. We only consume what has already been produced by developed nations.

Even medicine, (which is gradually becoming irrelevant) would have been a totally irrelevant course if not that it is essential for every community to have physicians(healers of the body) of some sort because people will always fall sick.

Think of it. . . , despite the exploding population, hospital numbers/hospital capacities/ Medical labs haven't increased. If the basic function of the government is not being done, how would we expect them to put any funding towards research. Everyday, politician's salary is increasing and none of them understand that they shouldn't have to travel to Dubai for a rectal exam.

Forgive me, but I think it is a good thing to see that recently, even our politicians are dying of diseases that would have been well treated if facilities were available locally.

Basic medical sciences, medical lab science and pharmacological science are all attached to medicine at the hip. And as long as medicine and the health of the general population continues to rot, the situation will continue to worsen.

MDCN or MLSCN shouldn't be the enemy but allies. Like I said, we are all tied at the hip. The government is the real enemy here. Put money in the health sector, put more value into the health of the common man/average Nigerian and they will produce more smiles/less crime/ corruption.

I agree that sometimes people go overboard in Nigeria because rules are not enforced. However, there is nothing wrong with if Doctors who own "small clinics"/small businesses take on some of these extra responsibilities.

Just like you expecting a road side Akara seller to employ a cashier, a cleaner etc.

Make we no allow beef to cloud our judgement ejor!!!

The Docs training is broad enough to overlap into most of the other duties that you mentioned. The problem we have in Nigeria is that limits have to be set.

However, a usual trend in Nigeria is to put expenses before the value of human life.

A lot of deaths in Nigeria are preventable if only we did a little bit more.
