It is highly unusual not to pick up the fetal heart beat by ultrasound at 10 weeks. Especially worrisome is that you had early pregnancy bleeding. Also, a transvaginal ultrasound will provide the best view for your pregnancy because it is early. If you had a transabdominal ultrasound, it might explain why the sonographer couldn't see much. Another option will be for you to get Beta HCG measurements serially. It is a hormone produced by the placenta of a viable pregnancy. If the levels are dropping, then most likely, you had a miscarriage, but if it continues to rise normally, then your baby should be fine. However, if you truthfully have a non-viable pregnancy, waiting is not a bad option, as usually, the body will attempt to expel a miscarriage. You need close monitoring while you wait. Complications of a missed miscarriage include sepsis, DIC and these are life threatening if not diagnosed early. |
I believe hospitals/clinics charge based on services that they have provided. And just like hotels have different rates based on overall quality, hospitals have the same. And if people feel that the quality doesn't match the price, then you should definitely go someplace else where you can get the best quality possible at the cheapest available price. But remember, good quality NEVER comes cheap. |

Dr young: Calm down, Sir.Like the above poster said, In Nigeria, patients should know that they have rights. And they have the right to know the name and position of whoever is treating them. They have the right to know what their diagnosis is and the names and side effects of whatever medication they are being prescribed by the doctor. Unfortunately, it is a lot of responsibility on the doctor, so, I will repeat again here, just like I said before that it is a good Doctor that will consult his colleagues, his pharmacist, books or other health workers regarding questions that he is unable to answer or he is unsure of. |

Physicians and Pharmacists have very different roles. The Doctor's duties overlaps that of the pharmacist to a certain degree, but well trained pharmacists are the masters of pharmaceutics. The Doctor is trained in making diagnosis and tailoring his treatment to the care of his patient. In civilized countries, whenever a pharmacist gets a prescription from a doctor, that he (pharmacist) thinks may be unsafe for the patient, his duty is to call the doctor to clarify first AND NOT TO REFUSE TO "DISPENSE". This is because the Doctor has a bigger perspective which the pharmacist might not see, and the pharmacist may have denied the patient of getting a life saving treatment. Medicine is so broad that the decision regarding the care of a patient is better handled when health professionals collaborate. For example if a doctor in the US prescribes a medicine and the pharmacist refuses to dispense and the patient goes without the medicine and dies or suffers harm, the pharmacist is liable. |
It is not news that the educational system in Nigeria is in pieces, and this affects every course that is being studied in Nigeria including Medicine and Pharmacy. Funds/Resources are lacking and the consequences are scary. Unfortunately, until we hold our Government responsible to certain standards, the worst is yet to come. Also, to make matters worse, the few well trained health workers that have the opportunity are trooping out of the country daily because they are not given opportunities to work due to the stupi.d politics that is being played in the health sector. They are overworked, poorly remunerated and are bullied by the Government (sometimes supported by the ignorant public) whenever they try to stand for what is right. About what happened to your wife, I would like to correct one notion that I think a lot of people have wrong. Medicine is about patient safety, so, it is a good health worker, who cares about the value of your wife and unborn child's life that will cross-check his prescription with his colleages, pharmacists, nurses in order for you to get the best/safest care. A lot of Nigerians think that when you ask others about something, then you are a bad doctor/pharmacist/nurse. But on the contrary, all health workers MUST check/cross-check and re-check everything they do. Because mistakes can be potentially fatal. |

Is this normal or what do you guys think, presently his BP is 130/80It sounds like your cousin is a young guy, and strokes are not very common in the younger population. I will try to be as simplistic as possible as possible in my explanation. There are many reasons why people get strokes, and in order for your cousin to be properly treated, his doctors need to figure out why he had one. Majority of strokes are called "thrombotic strokes". These happen when old, worn out arteries that go to the brain get blocked. This is kinda like when water pipes get blocked. The lack of blood flow to parts of the brain will then result in the stroke. Obviously, just like water pipes in the home, this will only happen to old/damaged arteries. For this reason, you will usually find this in older people with hypertension and or diabetes. The degeneration usually happens over a long period of time and it happens faster in those that have uncontrolled disease. For these types of strokes, treatment is usually aimed at controlling the risk factors I mentioned earlier - Diabetes, Hypertension, elevated cholesterol and patients may also need to be placed on an antiplatelet drug like aspirin. Younger people who use IV drugs may have accelerated damage to their arteries and may also have this type of stroke. Another type of stroke is the embolic stroke. this happens when a blood clot is formed in another part of the body (usually the heart) and it travels to the brain to block an artery. In this type of stroke, if the clot originates from the heart, patients may have rapid heart beats. This sounds like what you said your cousin gets. Treatment here is aimed at treating the "abnormal heart beat" and patients may also need to be on a blood thinner in order to "dissolve any blood clots" that may have formed in the heart. Please note that there may be other reasons for palpitations in your cousin. These are some common reasons for ischemic strokes, but there are other reasons for strokes in younger individuals. E.g forming clots easily due to genetic problems. Also extreme elevations in blood pressure can cause a bleed in arteries in the brain. Again, this happens easier in diseased/old arteries compared to in younger people. In summary, a diagnosis for your cousin goes beyond this Nairaland forum because there is so much to talk about regarding strokes. He needs a good doctor who has the capability to run the proper tests and tailor his treatment towards the cause of his stroke. |

About your question, your symptoms are vague and the answers could be endless or in fact, there might be no clear answer. Some possible reasons for your symptoms include an infection (viral vs fungal vs bacterial), photosensitivity, Heat rash, medication reaction, or maybe a systemic disease. Skin problems are not easy to diagnose without an examination and sometimes people may even need to do blood and skin tests. So, what you really need is a doctor's appointment so that you can be re-assured that it is nothing life threatening (I doubt that it is) |

I was on bromocriptine last year to reduce my prolactin level, but stopped and didn't get my flow from January to May when I had to go see d gynaecologist and he gave me an injection to induce my period. I honestly think DAT helped me conceive. I normally ovulate and get my flows with bromo. So should I wait a while before taking bromo or CN I star right away.Well from what you described here, it sounds like you have high prolactin levels. In that case, you probably need to go back on your medication to be able to conceive. Also, please make sure that you follow up regularly with your gynecologist. Best of luck!!! |

Let your body rest and try to conceive again naturally. Medications do more harm than good when they are taken inappropriately. Firstly, it sounds like you didn't get any blood test to measure your prolactin levels. And even if you did and it happens to be elevated, that is the body's normal response after a pregnancy. It is NOT safe, and you DO NOT have any business taking bromocriptine. You conceived without any problems previously, so you will be able to conceive again. You only need to allow your body return to its pre-pregnancy state. Allow your body to rest!!! |

Knowing fully well how it could progress into MI while awaiting diagnosis(sure I av no diagnosis). Aspirin75 mg has prolonged anti platelet effect and usage with clopidogrel wud just be perfect.. I was only advising him to manage it wif dis drugs.. So think next time b4 u call sum1 a quack... What side effects? The side effects of theses drugs are less probable usually less than 10% of cases. They are enteric coated, so no risk of GI bleeding.. Hez 22 yrs or so.. So we shudnt advise patients to use drugs cos dey av side effects? Which drug doesn't av side effect? *pfffftttt* abeg!!!!First of all, from the way you are talking, you are clearly practicing beyond your scope of expertise. Otherwise, you wouldn't spill the BS you just put up there. If you want to be a doctor, go to medical school. |
I suggest that mods should strongly discourage prescription of medications on this thread. A lot of innocent people come on here requesting for solutions to their problems. A lot of people who we do not know of their backgrounds offer suggestions. If you think that suggesting that a person should go and buy valium or morphine or codeine is wrong, It should also be wrong for anybody to suggest other prescription medicines because all medicines are potentially dangerous!! |

Albumin has no role in Neonatal jaundice. What happens is that your baby destroys so many red blood cells and produces too much bilirubin which damages their brain because their brains are not mature to manage high levels of bilirubin. If your baby is being considered for exchange blood transfusion, then albumin is not an option. Just like the name states, in exchange blood transfusion, the baby's blood is removed in small portions and exchanged with blood until most of the bilirubin is diluted out. This can't be done with Bilirubin because your baby will not have enough red blood cells left to live on. Make sure you have your baby with Doctors that have the ability to take care of your baby, and also time is very important because brain damage is essentially irreversible. |

But the truth is that there is nothing anybody can do without physically seeing her. Hernias are usually not emergencies when they are caught early. If she had an emergency, then it was most likely complicated. I don't know what complications she might have had, but the possibilities are endless. Emergency/complicated surgeries are not the best. There are usually problems, and her pain might be as a result of the complicated surgery that she had to go through. Because Doctors are not God, after surgeries and medical treatments (especially emergencies), things may never be perfect. However, If she is in a lot of distress, there might be some way to manage the problem and make her more functional. But like I said, she needs to see a doctor who will probably need time to make a good diagnosis and treatment plan. There is no quick cure for any condition especially when it has become complicated!!! (As an example, it is just like you are trying to fix a car with problems. Depending on how complicated it is, the car sometimes will never be back to new) Doctors try to treat the problem, but ultimately the body has to heal itself. Docs just aid the process. Find a doctor you can trust. PS. I am not a Doc, but i have a lot of experience in the health sector. |
Gboliwe: how do you know she has only 30percent chance of survival? Hernia is classed as an intermediate surgery, why all the complications? azubaby: Hello Dr n Mothers in the house.I seriously doubt that you have mastitis!!! Mastitis will not cause pain and ulcers on your n.ipples. Mastitis also responds to antibiotics. So, that explains why all the antibiotics you have used so far have not worked. Sore n.ipples is usually a mechanical problem (friction). Kinda like if you rub something firm on your skin, after a while it will hurt. Sore n.ipples usually happens to new mothers, and like some other posters have suggested, make sure your baby latches on well. If the problem is your technique, you will likely have soreness on both sides. I have attached a video that talks about some techniques, and there are lots of more resources on youtube.
There are other reasons for sore n.ipples and sometimes a fungal infection of the n.ipple is a likely reason for just one sided soreness. Yeast thrives in warm, moist, nutrient rich environments. In the case of this, first make sure your baby doesn't have thrush and if he/she does, go see your doctor for treatment. Stop using your sore side until you have healed and express from that side to prevent engorgement. But it is okay for you to continue to nurse on your normal side. Apply a topical antifungal twice daily. An example is clotrimazole or canestene. They are usually over the counter and don't need prescriptions. Keep the area dry as much as possible and apply petroleum jelly constantly to soothe the area. You can do this in between the antifugal applications. There are also lots of resources on the internet. khalling2008: Doctor in house pls help me, I have chest pain for the past two years now and I went for xray in january this year and from the result I have PNEUMONIA, since that have been using drugs which doctors prescribe for me but not cure it.If you have taken all these antibiotics, then your Xray should't show Pneumonia. If the medicine is not treating what it is meant to fix, then the diagnosis is probably wrong. Kinda when u change the fuel injector, but ur engine still backfires. Firstly, you need to stop taking antibiotics because they are clearly not helping your condition and will do you more harm. Next you need to find a Doc, who can help with a proper diagnosis. Too many things can cause chest pain. Some of them serious, others not so serious. If you are having cough and weight loss, a possibility is Tuberculosis. It is one of the lung infections that can linger for the duration you've been sick. In summary, despite all good intentions, the best anybody can offer you here is advice, but you still need to go see a medical doctor. Or you can try to find a Pulmonologist in one of our Teaching hospitals. |

Joanna Sedley: funny, this is a case where health faclities are not monitored appropriately to ascertain their credibility, level of professionalism, competence scope of practise and license which ultimately leads to proliferation of quacks. Trained medical workers refer patients who are at risk of something to a higher level of care not the other way round.My opinion is that even though it is important, it is extremely difficult to monitor every health facility. On the other hand, we as medical professionals (Docs/Nurses/Midwives/Pharmacists) should hold ourselves accountable for the lives of our patients. We should NEVER work above the level of our training. First DO NO HARM. And this implies to every medical professional. We know our limitations, and we should never be too proud to ask for another's opinion. On the other hand, you see everybody diagnosing/prescribing/dispensing. Medicine is extremely vast and it was structured to have Doctors/Surgeons/pharmacists/Nurses etc. Some of our roles overlap, but our roles are clearly defined. Unfortunately, just like an earlier post said, THIS SHOULD NEVER HAVE HAPPENED!!! |

While antibiotics have certainly benefited many in myriad ways, an overuse of antibiotics may be changing our entire bacterial makeup, says Dr. Martin Blaser, chairman of the department of medicine at New York University Langone Medical Center.Unfortunately, everybody plays Doctor in Nigeria. If prescription and dispensing of meds is limited to those that are legally allowed to do so, some of the problems with healthcare may be mnimized. I have seen lots of people taking the wrong meds for a wrong diagnosis. What can be done to correct this problem? |
gentlegg: Please am posting this to seek medical advice and response. The wife of my best friend in this world has been in coma more than 2 weeks now. She has been moved from FMC to Teaching hospital yet all to no avail. The problem started about 2 weeks after child delivery(her first). She experience recurrent and very frightening seizures/convulsion. The seizure can last more than an hour and she is incommunicado since then, for more than 2 weeks now she has not opened her eyes, has not sat down on her own, has not been talking, poos on the bed and urinates on the bed through pipe, has been just lying down and her breathing could be unbelievably loud, has been on oxygen over 90% of past 2 weeks. More than 27 lab tests were conducted at FMC Asaba but only saw protein in her urine and HBP which have affected her brain.A CT scan for her is very impportant and should have been done years ago. It is also the most important test that can determine whether she can be saved or not. It is quite unfortunate that most hospitals do not have these facilities on ground. |
Just like you said, it sounds like she had Eclamsia which resulted in the seizure. Seizures are bad and prolonged seizures can result in brain damage which is why Docs always try to prevent seizures, or stop them ASAP. From what you have described, the prognosis sounds bad because prolonged coma means she most likely sustained a lot of damage while she had the seizure. Another possibility is a hemorrhagic stroke which could have occured due to severe elevations in BP caused by Pre-eclampsia/eclampsia. At this point, a CT scan is a good idea to see if there are any things in her brain that can be fixed. e.g a significant bleed in the brain can be drained, also increased pressure in the brain can be relieved (Not sure if some of these things are available where she is) Besides some of these things I have mentioned above, the brain is one of those places that medicine can't do much to heal. Also the fact that she has been in a coma for so long puts her at risk of complications related to being unconscious (and also of medical therapies that she will receive) e.g Pneumonia, Sepsis, UTIs etc. So, the situation doesn't look good. |

The doctors administered a glucose drip to someone who was borderline diabetic. She slumped into a coma and never woke up. A simple blood sugar test would have made a huge difference. Which energy giving foods/drinks should the patient take inplace of soft drinks?First of all, the complications of hyperglycemia take a while to develop, so most clinicians are more aggressive in the treatment of low blood sugar compared to high blood sugar. So it is valid to give a sugary drink without checking the blood sugar. If the patient feels better, they most likely had hypoglycemia. In known diabetics however, you should always check their sugars. If you suspect hypoglycemia, you can give anything that will be easy to swallow and quickly absorbed by the body. And the options are any liquid that contains simple sugars like fructose, glucose etc. The best options I can think of will be fruit juices - Caprisun, Five alive etc. However, it is easier to get a bottle of coke/fanta/sprite in Nigeria. So, it looks like that is our best option. |

Don't waste your money!!! If you did any seminal fluid culture that grew staph and your lab tech made that diagnosis, tell him his diagnosis is wrong!!! The staph you grew is a contaminant. |
There is no specific reason for coke. Being sick puts you at risk for hypoglycemia and If you haven't eaten, you risk is even greater. It doesn't matter what you drink. As long as it has glucose in it. Orange juice, fanta, coke, tea. E no matter. Whoever insisted on coke is either a quack or has no idea about what he/she is practicing. Lots of quacks out there my brother. |

I swear little knowledge is worse than no knowledge. That is the problem with us Nigerians. Too many cooks. Abeg, make nurse stick to him job, make pharmacist stick to him job. Same goes for Doctors, Dentists, Med lab scientist etc. And the doctor coordinates all of the care. He has the widest perspective and is the head of the team. |
