Did they collect your original WAEC result & not return it back to you? Ours was returned except for NPC attestation & police report that was collected. Please be sure and clarify.
At visa collection, you will only be given your international passport(s) & brown envelope(s) that you must not open.
If you have reconfirmed as outlined by G2glory and sure its with them, Send mail to LagosIV@state.gov
you may say for the 'attention of Visa Chief', then describe the window where you were interviewed and tag number.
It will be attached to your package

olivialuv: Dear visa officer, I want you to enlighten me on a denied visa condition because my dv lottery visa was denied, I was told in the embassy on interview day(last week 9th oct) that my wife and child was not included in my application which I argued with the VO but in vain . My invitation letter stated my beneficiary which include my wife and child and I sent their Ds 230 form to the KCC after putting a call across that the initial form I sent was omitted with my wife and child form at point of dispatch at the currier center , I was told to write letter attached the form with the winning notice and send it back to KCC which I did but to my surprise the VO said they did not see it , the question remain that , the VO said I did not include my family and they saw only my form yet I was denied under section 212a(5)a tag "improper filling " yet they collected 3 visa fee . Pls how can I go through this , is there way out? Thanks.

Greetings! Here's the thing: You state that your wife and child were omitted from your initial form / entry. That means that your entry could not qualify. You must include your spouse and all minor children at the time of your initial entry, or you cannot qualify for a Diversity Visa.

By law, KCC is not allowed to adjudicate Diversity Visa applications; they just collect the paperwork and forward them to post. So they are not the ones who can tell you that you're disqualified. By law, that has to be done at the Embassy or Consulate. It's a tough law, but it's one of the very few rules of the Diversity Visa program, and the applicants have to take responsibility to become familiar with these rules.

There are no refunds. The fee isn't to receive a visa; it's to be interviewed and have your case considered by a Consular officer.

By law, KCC is not allowed to adjudicate cases. So even when someone leaves a spouse or child out of the original entry, KCC is not allowed to say "Well, you're disqualified." They pass the application to us, and we do the actual adjudication. That's why it's so important to make sure you have followed all the rules before coming to an interview.

The NPC office is @ Babs Animashaun in Surulere area of Lagos but I'm not sure of any Law Court
that might be closer to it but I'll suggest u get their first or just go to Ikeja High Court for d affidavit and
find ur way to Surulere for the age attestation.

From ojota, just take a buss to Ojuelegba, board another buss going tru Bode Thomas to Bab Animashaun, ask or just look around for d sign post. Its also beside NYCE office.

Not too sure of d required doc but hold on, I'm sure my proff's (Obatide & Kevwe) in d house will get u more info on this.

You and @Lagbabe have already gave best advise to him, Just in Addition, Pls take more than 3 passoprts for all because federal High court collect 2 for affidavit from me and NPC collected 2 as well.
For Direction, I took IJESHA, along Mile2 express road, board a bus to AGUDA and Marwa (Napep) to NPC at the same address, it costs #100 to #120 from IJESHA to the NPC office.

From this, I wish to suggest that we try as much as possible to connect with one another, I know some of us have already, but it would be great if we continue to remain as one family. Nairaland is the platform with which we all knew ourselves at the instance of winning the DV lottery. Malcolm28: Medicals and Police Clearance certificate done, waiting for my interview on tuesday 3rd December. I'd like to share my experience at the Medicals (Consultant Practice) cos it might help someone. Got there around 10:20am after going through a bit of traffic. Was given a Zenith Bank Teller to go pay the total sum of 38k which is the consultation fees (12,000 each for 2 adults and 7,000 each for 2 children).
Having done that at a Zenith Bank just 5 minutes walk from the Hospital, returned to the Hospital where I filled out some consent forms for each member of my family. I was then queued to see the nurse. The nurse took the weight and height for all of us, drew blood samples from my wife and I and then injected my 3 year old daughter with something that they will watch and read after 48 hours to see if she has been exposed to TB. My 1 year old Son wasn't given that injections cos they said he is still covered against TB by his current immunization.
I and my spouse thereafter took turns going to take x-ray. After that we went upstairs to see a doctor in a consulting room who will determine what vaccines we will take. We waited for quite sometime as the process became very slow at this point. Eventually we were attended to. The Doc was very friendly so the session was cool. He took our BP after asking a few questions about our medical history. He said that he needs to take our BP before telling us about the vaccines so that our BP won't rise (lol). He asked for our children's vaccination records which we had handy, he looked through them and highlighted areas of interest to him. He commended us saying he hadn't seen such a comprehensive record of immunization for children in a long time. He then made his recommendation for the vaccines we are to take and calculated the amount on the sheet of paper (I didn't copy them cos I thought we will leave with the sheets later). The bill was as follows: me and my spouse 18k each, my Son 22,500, my daughter 25,000. Total for all (Vaccines+consultation) was 117,500.
Please for those with Children who have vaccination records, please go with 2 photocopies for the hospital. I had to go out a make copies somewhere before leaving. Also, go with your Int'l passport, the photocopies and 2 identical passport photographs. Be ready to spend sometime there cos there can be traffic there as the hospital also does medicals for Canadian Visas. We went back 2 days after for reading of the TB stuff they injected my daughter and I alone waited to collect the medicals at 4pm which is the time they release results.
I also did police report on the day I collected the medicals. The process took about 2 hours in all. Cost me 5k each as the police people refused to reduce the money even with the Presence of my mother-in-law who is a retired officer.
All is now set for the interview on Tuesday. House believe God with us for a swift, clean, easy interview. The horse is made ready for the battle but victory belongs to the Lord. Thanks for all your support especially my good friend I met on this forum @Obatide. God will perfect all that concerns us all.

Before any of them reply, Embassy has a stable excanche rate of $1 to 165 Naira

Fees may be paid in U.S. Dollars or the equivalent in Nigerian Naira, the exchange rate at the Embassy and Consulate General is 165 Naira to one Dollar. ....................

If you get married Legally, all documents after marriage will bear her new Surname.
So, I will advice she use her marrital SURNAME

The strong memebers of this forum took me with surprise, for more than 3 times that I wrote the circumstances that besiege my forms to KCC, I wrote that I was one the DV 2014 selectees, with case number 2014AF 000069** that my bio-data forms had been sent and received by KCC since July 27,2013 this year and that my case number have been current since November as published in their bulletin, up till now I have not receive my 2 notification letter, but to my surprise nobody in this forum cares to encourage me and provide the way forward. Please house I really need your contributions and helps.
so sorry for that, I belive its an oversight.
Do you confirm that they received your form?
If no, mail kccdv@state.gov to enquire about the status of your submitted forms quoting FULL NAME, DATE OF BIRTH & CASE NUMBER, whenever you mail them or you may call them.
Once they confirmed that (if yes), relax your mind and prepare your documents, once ur 2nd NL comes, time seems to be flying.(my thoght tho)

Your question has been answered Severally, in the process of this Dv Lottery, investing more time to read and go extral mile in searching for information cannot be over emphazised. So follow previous post on that subject matter pls, and you will be glad you did.
In summary, to be at safer side, DO the NPC ATTESTATION.

it is not a requirement to have had English at school, so the fact that you don't have English on your school certificate is not a problem. You must however be able to show that you completed 12 years at school.

I was told to bring my passport in and to pick up my visa. Thank you Jesus!!! the nightmare is over. My case was sent to administrative procesing Jan 15th and I got a confirmation that they can issue my visa yesterday Monday 25th it took 5 working days.
Today I realized no matter how old we get, we will always be full of joy and happiness as soon as we have the support of our friends and family,ALL THANKS TO THE MASTER PLAN.BABA GOD!.

Simple. Firstly, put your mind at rest.
E-mail is just a means of alert when 2nd NL arrived, so I will advice you to open another e-mail address, use it to communicate with them to confirm they don't require any further information on the forms you submitted, and Check the site (www.dvlottery.state.gov/ESC) where you printed your first notification from on a regular basis, that's where your APPOINTMENT/INVITATION SCHEDULE will be.

I see your predicament, I am advising you to use the correct age, which is what you have sent in the forms, do not change it as that is what is on her certificates and passports and birth certificate I believe. Tell your sister to tell the CO the truth at the interview if the CO raise an issue on it, tell him that an agent applied for her and made the mistake of putting wrong DOB and place of birth, They understand and know agents help people to apply and mistakes are made and they do not make issues on it so long u tell the truth. Believe me we give this people too much credit they will not note or see this difference in DOB or place of birth not to talk of raising issues about it. Tell ur sis that the visa is hers IJN amen. u will surely testify on her behalf IJN amen.

Beloved, my candid advice is fo you to get the NPC attestation of age. I am not sure if that delaration of age is accepatble (pls read the requirement). It will cost you just between 2,500 & 4,000 naira.

I had a birth certificate that was issued in the 80's despite that i was born in d 70's because i was not too convince if it was acceptable or not. i went for NPC certificate so as to be on a safer side. This opportunity worth such investment.

Short form birth certificates will not be accepted.

The certificate must contain the ffg:
Person's date of birth;
Person's place of birth;
Names of both parents; and,
Annotation by the appropriate authority indicating that it is an extract from the official records.

I would advice you do it now.
Its also advisable to get every other documents intact except MEDICAL & POLICE REPORT to ease your stress whenever your 2nd NL arrive.
Even if possible, Get your $330 processing fee ready in Dollars too, all these will ease your tension when you are current.

3rd in my WEAC 2005 I have only B3 in commerce in 2006 I wrote NECO EnGLISH C4 MATHS C6 GOVERNMENT C6 ACCOUNT C6 I also have national Diploma I mine good 2 go?

First of all, I will advise u read more of what people have posted on the thread from page 1, You will be doing yourself more good doing that.
For question no. 1. Ensure Your birth attestation bears all those names, with that, you are good to go.
2. Affidavit will be okay for that and your ability to convince the C.O that its an honest mistake.(read Jannut testimony today)
3. I'm not in position to say that your O'level has K leg, but be prayerful. Don't you av up to 5 credits on that Neco? At least any subject to make it 5?

KCC is currently scheduling visa numbers for your region for the month of January'' pls could dis mean they have skipped our case no and if not do we still stand a chance to be scheduled? if yes when is it likely? pls i knw you r not a consular but someone with similar case could attend to my questions.

Pls worry less, God, who has started it will definitely complete it.
It takes KCC between 6 to 8 weeks to complete processing of submitted forms according to their official response most times, but it could be more than or less in actual sense, though, mostly more than the 8 weeks.
Since you have confirmed that your forms have been received, relax yourself and prepare your documents ready for interview, don't wait till 2nd NL arrive b4 u start preparing pls.
Note that No amount of worry and Anxious can bring 2NL, don't give urself high BP to avoid children practice saying they see something in your chest grin grin grin

janut: Our interview was quite unique. You will know why @ the end.
We got to the embassy @ about 5:55, and met people there as usual. After some time we were told to form queues according to immigrant and non immigrant.We went through various security checks and they also checked their list to confirm if we had appointment for the day.
Getting in we paid N109000 I.e for 2 persons. Pls keep all the papers they give you before entering.
After paying we submitted our documents and was asked to wait for our interview.
At about past 9 am we were called for our interview.
C/o: please both of u should raise ur right hand and swear to say the whole truth during the interview.
He took our finger prints after which the interview started.

We thanked him and left.
In all of this the c.o did not ask my wife a single question even though she was right beside me.
Please alwz say d truth. I used a wrong date of birth during my initial application and the c.o asked me about it which I confirmed and he was satisfied.
I wish those yet to get their visa all the best. It can only be God.

Dont be too desperate
Be sure of the source of Any Documents You are procuring/presenting
Gather as much info as you can instead of presenting documents that are fake
Finally, beware or AGENTS/TOUTS
