We heard that the doctors are doing everyone's job at some hospitals. We should not worry as they have thrown the first stone. We will wait for them. Whenever they go on strike, we will also do their duties, which we are very capable of executing and excellently too, thanks to Pharm D. and consultancy status in other professional cadre, and refer surgical cases only to friendly healthcare facilities. If patent Medicines vendors prescribes and treats patients in their localities and make their money to build 1,2,3,4 attorney buildings, the Pharmacists are there to consult, prescribe and treat with better medicines as the only experts on medicines. The Optometrists are there to handle all eye challenges. The nurses would have to add to their curriculum Nurse practitioners course to enable them prescribe medications for patients. The Laboratory scientists are always there to assist with the diagnosis. The Radiographers are there as well. What s it that we don't have. The Physiotherapists nko? All on our side. Dental technicians are d people that do the work of the dentists and even do it better. Its only Surgical procedure only That we will refer as I said earlier. Period.

Consultancy cadre for JOHESU has been in practice in Niger state for more than three years now and there has been no single case of patient confusion or a JOHESite pretending to be a doctor. I have well travelled and I have seen what is obtainable in other climes, i do not argue based on sentiment. A humble house officer, for instance learns faster than those arrogant ones. Where you are not so sure of certain dosage, dosage form, strength or brand, the pharmacist is there to quickly help out. My cousin (F) who is asthmatic was prescribed cocodamol by a consultant O&G but on getting to d pharmacy, d pharmacist spotted ventolin inhaler and cocodamol tab on same prescription, he called d attention of the consultant who eventually changed it to celebrex. No human is all-knowing and no human is above errors no matter how intelligent you are, and committing an error does not reduce your intelligence. Two good heads are surely better than one. Team work gives better outcome, makes the work easier, faster and less stressful. This is d norm in saner climes. You cannot continue to do the same thing over and over again and expect a different result. This was the kind of strange argument that NMA had put forward for years suppressing others to the rank of deputy director, no non-medical doctor was allowed to get to the peak of his/her career (director) that if you have other directors (like director of nursing services, director of pharm. services etc) it will confuse the patient with the Medical Director. JOHESU went to court and won the case just few yrs ago, now you have these directors in the hospitals, have you ever heard of patient being confused or any of the directors pretending to be the Medical Director? You are probably not a civil servant, you would understand that there is almost no way that a pharmacist, for instance, would pretend to be a doctor in a public hospital setting and get away with it easily. Where is he going to consult from, his office? And with which folder? How is he going to admit? How is he going to get paid? How would he charge the patient? Even a first time patient would ask the gatemen where to go first not to talk of regular patients that know the procedure very well. You mentioned rural settings, professions like MLS, pharmacist etc do not operate at primary healthcare level, you find them from secondary level (like general hospitals) which is also very organized with SOPs. Just so you know, I have a postgraduate diploma in Leadership and Management in Health (USA) so I am not here to fight JOHESU vs NMA fight. If you need to know the standard world over, I would give you d design. |

He finally presented his son to hospital in hypovolaemic shock due to ruptured viscus and it was an auxillary nurse that gave it There are quacks everywhere and in every profession. The case of mistaken identity due to patient's ignorance can happen but does not affect patient care. Again the public health setting is designed in such a way that a pharmacist cannot do the work of a doctor and the nurse cannot do the work of a doctor whether you call them doctor or not pls leave sentiment aside. I have discussed this erroneous assumption with my colleagues (medics) including our able boss who is a consultant (family medicine) and they have all kept sentiment aside to agree with the real facts. There is a kind of strange medical practice in Nigeria that d doctor assumes he knows everything and he is above error, the consultants also oppress the juniors doctors even when he is wrong you dare not point it out to him (he is all knowing)..it's all about ego ....this training is not good for medical practice and by extension, patient care ...ask any of your colleagues who are practicing abroad to gist you more about the practice over there... |

For crying out loud, many nigerians dont know the difference between a doctor and a nurse, especially if the person is female. Ask most female doctors, especially those that work in the public setting, a patient has referred to them as 'nurse' several times, meanwhile i have personally seen a lot of male nurses being called 'doctor' by patients. How does calling you a nurse as a doctor affect patient care? It is your duty to educate them that you are not a nurse but a doctor or vice versa. Thank God you made reference to public setting, is it possible for a pharmacist to go into d doctor's consulting room and consult because he is a consultant? No. Every professional has their area and office of practice which cannot be substituted. Even a paediatrician for example cannot just go and consult say in opd just because he is a doctor. Public settings are organized settings with rules and regulations and sanctions as well. |

Again sometimes you need to enquire before making assertions, there is West African Postgraduate College of Pharmacist for instance, (not even established by Nigeria) offering fellowship programs and only graduates of this college can be appointed consultant and not yrs of experience as you opined. Nigerian hospitals would be much better if NMA carries everyone along. I don't think JOHESU are mad people not to agree to cooperate. For instance I am working with medics and other health personnels as a team of HSE officers in a firm. Our bond is so strong dat no alien can come inbetween us. The doctor who is our head does not go and fight alone for the salary or other benefits of medics. We have one salary structure with different percentages for call, shift, hazard allowances etc with d medic having highest percentages followed by pharmacist, MLS then the nurse. The head medical officer goes to top management meetings to defend the course of HSE and not medical doctors alone. You need to see him fighting for us, a leader of all, a father of all. He has also affirmed that NMA is not a good leader. |

The kind of things you see in an emerging economy where the govt is the largest employer of labour. The only practicable solution is for the government to conceed the running of all public hospitals to Public-Private Partnership arrangement. Allow private employers of labour to determine who is important in the hospital. Let private employers also determine who earns more. Aberrations like JOHESU are the hallmarks of a failed state. Nigeria needs a change in structure. I told my MBBS colleague of mine some days ago that if not for people like him I would think reading MBBS in Nigeria makes one myopic and stupid that you can't even use google to get information. We are both HSE professionals. We discussed at length objectively pointing out the issues, JOHESU has its blame no doubt about that but NMA as the head of the health sector has really bastardized the health sector. From the ministers of health to commissioners of health, CMDs, MDs, boards of health institutions etc. are all doctors yet our hospitals keep deteriorating only for NMA to turn and blame JEHOSU instead of holding their colleagues (who are leaders of the sector) accountable. Whenever I hear NMA saying 'international best practice' I wonder whether they have their own definition of the phrase. Because in developed world like UK, US, and Canada hospitals are headed by administrators who can be anybody with requisite administrative skills, the doctors and other health workers concentrate on offering clinical services as a team with mutual respect for one another. The USA surgeon general for instance, is a nurse, NMA cannot allow this happen in Nigeria yet still quote international best practices. The head of WHO is a biologist from Ethiopia. The consultant cadre issue, according to NMA would confuse the patient, as if Nigeria or JOHESU invented it; this is obtainable in the UK, US, and Canada that are ranked among the top 10 ..so which one is NMA's international best practices? NMA also does not allow any non-medical doctor to do MPH in Nigeria, only doctors are qualified to do it; a program that any science graduate can do even at Harvard and the likes. This is pathetic. You cannot continue to suppress and oppress someone and expect him/her not to react at some point. It is the right of every civil servant to rise to the peak of his or her career (which is director) but NMA oppressed other health workers for many years saying that if others get to the director level, it will confuse the patient. I dont know how! JOHESU had to go to court to get judgement some few years ago. I can go on and on and on ...... The crisis in the health sector is as a result of failure of our leaders aided by the NMA who has refused to carry the others along as the head of the team. Just recently the Canadian medical association rejected their pay increase insisting that the govt must increase that of allied health workers first. The opposite is d case in Nigeria. |

Johesu want the same salary scale with Medical Doctors. It's easy to claim entry point distinguishes them but let's look at exit point. A lab Scientist at 55yrs probably now at the highest grade does nothing but order Juniors around, yet a General Surgeon who has put his life on the line still does rigorous calls and theatre sessions at 58yrs. The stupid, insane and unbelievable new demand means that a lab scientist, Nurse and neurosurgeon at the same grade level should earn the same, despite not having the same responsibility, this is a shame, and the Country will pay for considering these Buffon's and clowns called Johesu. Nowhere in the world is this stupidity condoned. The average hourly work rate of each professional the world over can be browsed, in all cases the doctors' comes top.
My advice for Junior doctors is to run as far as ur legs can take u, this is the best time. Don't look back, doctors are hot cake and don't limit yourself to a shithole country.

Why don't you use Google to at least educate yourself before coming here to display your ignorance. Doctor's call duty is 4% of basic salary per unit while pharmacist or MLS is 2% . Now maximum units of call per month is 40 and in most cases others are not allotted max call units only doctors but let's assume all of them are taking maximum units of 40 at same grade level, say 17 (director) with basic salary of 400k. 4% of 400k per unit is 16k, for 40 units is 640k while 2% of 400k is 8k, by 40 equals 320k. How is 640k and 320k the same? This is just one example. Pls try and read wide and be enlightened. Ignorance makes even an intelligent person behave stupidly. |

Nurses collect shift duty allowance, how can a meagre shift duty allowance of GL 8 be the same with 4% of GL 12? A medical officer who enters on GL 12 as resident and becomes a fellow after 4 or 6yrs is upgraded to GL 15 ..it takes a pharmacist minimum of 12 yrs to get to GL 15 and nurse minimum of 18 yrs to get to GL 15. NMA is really not helping the Nigerian health sector. |

Codeine is an opiate narcotic, it is a restricted substance that is not sold over the counter, and by extension cough syrups containing codeine are therefore POM (prescription only medicine) and should not be dispensed over the counter. It is illegal to dispense it OTC without a prescription from a qualified medical personnel. |

MotorGuru: question sir, did you at anytime experience a huge jerk at high speed before this happened? Did your gear indicator indicate being stuck in gear 4 when put in manual mode? Is it only engaging in reverse? Has your fliud ever being changed( and what was its condition? Black?) Do you hear whining noise when drive or reverse is selected and you rev the car? Finally, you may need to scan the vehicle to ascertain the real cause... Pls let me know of any of these ever happened. Yes the jerk at high speed happened while travelling, it's stuck on gear 3 in manual mode, yes it's engaging well in reverse, the fluid has never been changed, the Mechanic checked its condition ad said it was ok. There is no whining noise neither in drive nor reverse mode. The vehicle was scanned and according to the electrician it was the gear sensor that was bad and he replaced it but the issue still persists. |

I am not a member of JOHESU but I expect Nigerian doctors to lead by example, what do you gain by suppressing others? I don't know if you have ever been outside the shores of this country to see how things are done, it would change your mentality about others. I work in a multi-professional team/firm (Drs, nurses, pharmacists, engineers, lab scientist, computer scientist, radiographers, seafarers of different fields and nationals) yet we don't have this kind of disharmony. By God's grace we have been sent on courses and have seen what is obtainable in other climes, I just wish the young Nigerian doctors would both be educated and enlightened and not toe the line of their older counterparts. |

I logged on to facebook to see if she replied the chats I sent her last night but NO, she didn't. It's obvious she is sad. It's her third year of applying for Medicine and Surgery and this time, she was offered provisional admission to study Food Science and Technology. I could guess, she felt her dreams has been shattered again.
I care for her a lot but I just couldn't find a means to pacify her so I kept my cool.
I've had the privilege of meeting prospective undergraduates on various occasions and their clamour for Medicine and Surgery is alarming. Sometimes it's very difficult to ascertain what fuels their desire.
Why do you want to Study Medicine? Most of them don't seem to have any answer.
They either grew up in an Igbo family where the parents so much believe that the way out of poverty and lack is for their ward to become a medical doctor or while in secondary school, they belonged to a class where 90% of the brilliant ones kept singing Medicine and because they didn't want to feel less than equal, they joined the bandwagon.
Every year, thousands of students apply for Medicine, yet, very few get admitted.
11,130 candidates applied for Medicine and Surgery in UNN alone and not more than 200 will be admitted. It's is already clear that more than 10,000 will be denied admission.
Same with other 24 FULLY accredited Medical schools in Nigeria.
Only about 1.6% of people who apply for Medicine in Nigeria get admitted yearly.
And only 56% of the admitted students end up becoming doctors.
If you successfully scale through the 6years spent in school, getting a placement for housemanship within the stipulated 2years becomes another bridge to cross. If you eventually end up not getting a place, you will have to resit your final exams this time conducted by the Medical body not your school and you will have to pay plus the high failure rate involved.
Looking at the stress in the journey to becoming a doctor, one should be able to vow that these guys should be the most successful set of people in the society but unfortunately, the reality is different.
Failure in Marriage: Whatever business, ministry or profession that takes a toll on your family life has made you a failure and that is the predicament of most Doctors. I read an article where an American Medical Body reacted to a medical divorce research by saying only 23% of Doctors have a failed marriage as against the 27% earlier proven, what difference does it make?
I don't have a problem with the statistics because in Nigeria, you will rarely find any divorce case but I've been to Doctors' houses and to be sincere, their wives suffer lack of attention and so do their kids. But the Nigerian culture has taught ladies to tolerate till Jesus comes. In the book 'for men only', 80% of the women interviewed said they would rather suffer financial insufficiency in their homes than lack of attention from their spouse.
He goes to work even on Sundays, he never thinks of taking his family on a vacation, he comes home by 9pm and is so exhausted to chat with his wife and as he lays to sleep, two hours later, the nurse is on the phone 'Doctor there is an emergency' and he rushes out by 11pm and that is the profession that thousands of deceived Nigerian students wants.
Financially: Doctors are not poor but very few of them are rich. 300k per month, you have a wife and children, you have house rent to settle and have other family members who has a perception that you are rich so they make demands on you. I've never seen a Doctor riding a Range Rover, you only see them on Toyota Camry, Corolla, Honda and other 1-2million naira cars.
Business: If they invest in other businesses, maybe things will be better. I was with a Doctor in the Medical Centre here and he began lamenting that even if he wants to do anything like business, he won't have time for it and people will end up taking advantage of him. Moreso, if at the workplace, they realize that you have investments, you will be denied certain financial privileges.
I am not despising the medical profession, as I can't deny the fact that the society can't survive without them. I am only worried that an individual spends 7years resitting Jamb simply because he wants to study Medicine.
Come to a gathering and introduce yourself as a medical doctor, nobody will bow to you, outside the school setting what speaks is your Success in life not your A in Pathology.
How many less privileged kids have you given a reason to smile by sending them to school? How many homes have your existence deleted poverty from their lineage. Therein lies fulfilment.
We need doctors, don't get me wrong but our quest for the medical profession is not worth it. It has crippled rich minds, buried companies that would have been providing 10,000 jobs today and hidden business potentials and leadership models in the chocking atmosphere of the hospital.
So guys, don't feel bad if you not admitted to study Medicine, settle for anything. It's your imagination in life that counts not your IQ. Make the best out your life wherever you find yourself.

I refused to apply for medicine in my JAMB against my dad's wish, it wasn't an easy battle but eventually I won and when I showed him my acct balance just after NYSC, he had to apologize and admit that he was wrong to try to force me into what wasn't my interest. Interestingly almost all my guys who went for MBBS are now consultants or senior registrars (we were intelligent boys and girls) and they are all my 'boys' 'for account balance level'. |

ON SALARY:
I wouldn't comment on that because that is the business of the the govt. But let it be on record that if someone should complain of salary, it is the doctors. Nigerian Doctors cannot talk in the presence of their colleagues practicing abroad. They are the most underpaid persons with relation to the value offered. You can imagine the amount Canadian doctors are paid that made them to protest last time. An average consultant doctor in canada receives about 500,000 (about 180 million naira) dollars yearly. A consultant in nigeria is struggling with 10million naira yearly. Hence the reason 90% of Nigerian doctors are jetting out of the country, with many on the way out also. Even the few that are staying, are doing so, due to some supplementation coming from private practice. Otherwise, with the rate at which other countries are ready to issue visa to doctors, Nigeria may soon run out of doctors. The truth is everyone needs a salary increase because cost of goods have increased, starting from teachers. However, the issue of salary still remains their business with the govt, but note that if your salary is increased to that of a doctor, the doctors will seek their own increase. It is natural to act that way.
ON HOSPITAL HIERARCHY
I heard they want to be given consultant nurse, consultant physiotherapist, consultant Med Lab Scientist etc.
People need to understand something. The hospital is structured in such a way that every treatment /instruction must be directed from the Consultant Physician or Consultant Surgeon. No patient receives anything in the hospital unless the doctor says so. The reason is not far-fetched. It is in the best interest of the patient, so that he/she is not malhandled. Imagine when we introduce a consultant nurse, a doctor will give his own instructions and the nurse will also like to execute his own consultancy expertise by giving his own instructions, at the end, the patient, is at the receiving end. The practice is that the doctor is and must be the head of the patient management team.
Most nurses are clamoring for consultancy because of the knowledge they acquire by virtue of working in hospital for years. They feel they can now challenge the doctor.
I will give an analogy: In Catholic church, we have the parish priest, we have the catechist. By virtue of working at the alter for about 10years, the catechist can comfortably celebrate the mass. Infact he can even celebrate more than the priest if given opportunity. He can also be more prayerful than the priest. But no matter the condition, the catechist can never celebrate mass or be equated to the priest simply because he can do what the priest can do if given the opportunity. There is formal way of becoming a priest, and it is open to everyone, thus, if the catechist wishes to be celebrating mass, he should simply enroll into the seminary, and boom, he becomes a priest. That is the formality.
Nurse vs Nurse Dichotomy
There are two types of nurses, we have those that attended school of nursing(Registered Nurse. RN). we also have those that are studying nursing in university (B. Sc). The truth is that the BSc holders are paid far higher than the RN holders. Why is it so? In terms of work, I need not tell you that, the RN holders are the ones that do the major suffering in the hospital, and JOHESu will not harmonize their salaries so that all nurses are paid parallel amount. But they want to be paid what doctors (mbbs) are paid. That's hypocrisy.
Medical Laboratory Scientists vs Medical Laboratory Technology
The way Medical lab scientist (Bsc) discriminate against the Med lab technologist (HND) is another case study. You and I know that those polytechnic guys are more practical oriented and cannot be equated with university counterparts. But yet, the Bsc holders still lord it over the HND holders. They claim superiority over them, and yet want equality with doctors. Another hypocrisy.
Med Lab Scientist vs Microbiologist.
I happened to study microbiology in my first degree. As a microbiologist, you cannot have a lab on your own. You must work under a med lab Scientist. Even when the knowledge base is the same among the two disciplines.
Last time on Facebook I saw some graduates of med lab from ABSU hanging stethoscope on their neck, calling themselves diagnostic doctors. Thats so laughable. And that has been the delusion that has been making them think they are equal with doctors. That you can see bacteria in urine in the lab does not make you diagnose a patient of infection. A doctor makes 95% of the diagnosis from history and physical examination, just right in the consultation room. The lab tests are to confirm diagnosis and also check for the other systems that the patient may not be complaining of. Also every med lab Scientist works under a Consultant Chemical pathologist, consultant micro biologist, consultant Histopathologist, or Consultant hematologist as the case may be.
The Way OUT
The truth is that, there should not be the need for all these fracas if everyone remains to his job description and prescription. There is already a job description for doctors, nurses, med lab Scientists etc. If you are not comfortable with the job you do, you can switch and follow the formality laid out for that profession. There shouldn't be need for being a nurse and wanting to do the job of a doctor, or being a doctor and wanting to do the job of a nurse. A doctor needs a nurse, a nurse needs a doctor, a med lab Scientist needs a doctor and vice versa.
In addition to that, every job has a salary scale, most times the salary does not justify the work they do, but only justifies the time and process it took to get there. The people that take the highest salaries in Nigeria are the people that do nothing. They only go to offices to enjoy the AC and go home at the end of the day. When the month ends, they collect millions. Its unfortunate society has made it that way. Let us have a rethink, and be human. Everyone needs salary increase, starting from teachers to doctors to JOHESU (except our senators ***winks***). The problem is getting it right at the electoral polls.

For instance, a RN and a BSc nurse on same grade level, say CONHESS 10, do not have different salaries....I don't know where you got dat nonsense thought from. The entry point of RN is grade level 7 while BSc is grade level 8. You don't expect grade level 7 and 8 to receive same salary. However, a RN on grade level 8 will receive same salary as a BSc nurse on grade level 8. |

There is absolutely NO NIGERIAN UNIVERSITY that comes even remotely close to University of Ibadan in academics with the exception of UNN and Covenant. And I am talking as someone who has deep knowledge of SCOPUS, Research gate, WoS, Google Scholar and Thomson Reuters and I can assure you that any ranking in Nigeria that does not place those three to be top of any academic ranking must be a complete pile of trash. You may do well to rely on webometrics because it is an academic ranking but the ultimate ranking system is ARWU, THE and QS.
However in the area of available academic facilities UI would be the only public university that would/might manage into top 5. The remaining position would be filled by private unis like covenant, AUN, PAU, ABUAD. However what UI lacks in some facilities it made up with in a good human resource base. UI still produces and retains academics like Gureje Oye who are well rated globally.
NB: I am not affiliated to UI in any shape or form. Just saying the truth. I don't know where you got this rubbish conclusion from but I know great works of people like Dr. Omalu from UNN who was the first to discover and publish findings of chronic traumatic encephalopathy in American football players, prof Jonathan Nok of A.B.U Zaria who worn the Humboldt award for his discovery of the gene for trypanosomiasis (sleeping sickness), just to mention a few. Just try and read about the above mentioned works and the international awards they got and stop spewing rubbish ...... |
