This is wrong. You now need ATVs whether you leave the airport or not. This will be checked during the check-in/boarding process and you won't be allowed to proceed. I transited through AMS when I visited Nigeria last year and someone who was meant to be on my flight was denied boarding due to the absence of an ATV.
Luggage (except hand luggage) was not seen or handled by passengers till we got to the UK.

If you book a flight that requires ATV, then it is Self-transfer and you won’t even be allowed to board the flight right from the original country you start your journey because it will be part of the checks that will be required before you get your boarding pass. Quite unfortunate that the person who brought out the discussion has already booked the flight as I was a victim in 2018 when I booked flight to Nigeria from London through Italy but it required me to have a schenghen visa before I could even start my journey. You still don't get it. It is UKVI that creates this numbers and assigns them to schools so once the CAS number on an application is correct, UKVI uses it to access the CAS details to check that they align with what you have on their system. Whether or not you submit your CAS statement to UKVI, they still need to conduct a check on their own systems to check that the CAS number exists and is not expired.

As long as you arrive on the 19th and not the 18th, it is all good. As far as I know, if the flight will arrive on the 18th, the airline will not allow you to book. If they erroneously allow you to book and board the flight, you won't be allowed entry into the country since your visa isn't active yet.

Yes, you can if you meet the job criteria but you might need to cut out some things from your CV and adapt keywords to suit the job applied for. While at it, you can work towards your GMC registration.

You can apply at VI or Ikeja, not seeing a crowd there doesn't mean that there are issues. Most people probably opt for Ikeja so as not to pay the extra fee charged to use the VI centre. Also, it only makes sense for people to apply in the locations close to them hence, the crowd at Ikeja.

Depends on the school. Usually around 2-3 weeks or more. When applications are many, could be up to 2-3months for some schools.

It won't. The rate used in converting your funds is the rate on the day of your online submission. If the rate goes up even a day after your submission, it doesn't matter.
