In the case of covid 19, the site of the initial infection is the nasal cavity,the infection spreads to the throat, the larynx andthen travels down the lungs through the trachea. If it gets to the lungs it then becomes dangerous and leads to hospitalization and sometimes death.

Now, the infection can be stopped anywhere in between the nose and the lungs, by the bodies natural antibodies or with vaccine assisted antibodies. Either way, the body must be infected before the antibodies can be deployed.

What vaccines do is "trick body to think the virus is present so body generates antibodies". The body has to be infected first before the body can generate antibodies. The job of vaccines is to make the body deal with future infections better. Most people still come down with chicken pox or measles even after being vaccinated at a very young age.

I doubt CDC can claim vax are 90% effective of preventing infection, most probably that's an error of reporting. That's why CDC still mandates masks and social distancing despite being fully vaccinated. What CDC can say is that CDC claim vax are 90% effective of preventing hospitalizations and deaths from Covid 19.

Assuming an r rate greater than one, if 1 person had Covid start of last year multiplied by the r rate exponentially is different from a starting base of say 100,000 multiplied by r rate in 2021
